I set up with my authority to be able to run over 150 miles as the timber industry took a hit from the storm so that i could go further if it goes bust can I use a time sheet instead of log book since I am only traveling less than 150 miles a day even though I am set up with fmcsa to go further?

Part 390 is General, not Insurance. Insurance is Part 387.

Ok, so just to make it clear, it is Ok for the company to create their own forms right? There is no type of specific legal form that one must use to compile all this information ???
