True Ending: Simply beat Sucsarius in the Forlorn Arena and awaken the dreamer. This is the only ending you can potentially lock yourself out of. This ending will be replaced by ending E if the requirements to do so are met.
Gates of Tartarus: This ending requires quite a few steps. You need to acquire 30 "strange coin" items from doing specific things. There are 3 actions you have to carry out, with each rewarding you 10 total coins. The events are as follows:
Kill Death - In order to kill Death, a weapon called "Limbo" must be crafted. The first component, the broken sword, can be obtained from a skeleton near the wooden door outside the entrance to Sucsarius's arena. The sword will only appear during an in-game full moon, so you might not see it the first time you spot where it would normally spawn. The second component, fractured life, can be obtained inside the sub-area in the Forlorn Arena where you find the earth emblem. It's hidden behind a very unusual secret door. The secret door is not next to walkable ground, but is clearly outlined by vines in the middle of a wall. You will likely have to use earth bridge or icarian flight to reach it. The final component, fractured death, is also located in a sub-area of the Forlorn Arena, this time where you would fine the water emblem. You should be able to find it just by hugging the walls. You'll know it's the right secret room when you enter a narrow room that appears to have nothing in it. If you look up, you should see an entrance towards the ceiling. You will also likely have to use icarian flight or earth bridge to reach this. Now just take the 3 components to Etna and craft "Limbo." Once you have the sword, go to the Mausoleum in the Accursed Tomb and wait for Death to spawn. It'll only take one hit from Limbo to kill him. Death's weapon is incredibly high in damage, but having it also locks you out of important areas. To be able to access these areas again, you must grind for weapon EXP on Death's weapon and upgrade it to revive Death.
Kill Jotunn - Once you've killed Sucsarius, Sheryl will sell the weapon "Jotunn Slayer." It's on the pricier side, but shouldn't require any significant grinding to buy. Once you have it, simply go to the Lake of Blood and find Jotunn. Like Death, they also go down in one hit.
Desecrate a Monument - In the Temple of Silence, there should be a statue surrounded by multiple sitting mummies. Just use a blood spell on it to break it.
Once you have all the strange coins, go to the Labyrinth of Ash. If you've destroyed all 3 of the music totems, you should be able to cross two wooden bridges hanging over a pit. Just make it to the end of the temple you'll find yourself in and open the door you see in the video.
Ending E - In order to get this ending you must obtain every spell in the game and complete the VHS questline. I'm not really going to go into detail on how to do this, since there are other videos showing how to do it and it would be a little hard to be descriptive enough on the steps in text. The actual boss fight present can be a little tough for some players, so make sure you either use both hands on your WASD keys or one hand on WASD and one on arrow keys.
Ending CD - Stare into the center of the puddle you see in the video. You do not need to be standing in the puddle. You might need to do other endings first for this to trigger, though I'm not entirely sure.
