MacBook Pro: Firmware lock bypass and USB boot issues 

I bought a used mid-2009 MacBook Pro in unknown condition. I had to buy a replacement charger for it (linked below) but discovered that it powered up but would not go into recovery mode. Turns out it had a firmware lock. This is how I bypassed the lock and installed the OS.
