This is hands down the most informative video I have watched on the subject of dental bone grafting. I wish I could come to you to have my implant done, but alas, not feasible as I live in Ireland. However, I now feel armed with more knowledge thanks to your video.

Well said, doc, it's not a small step, and it takes an informed patient to have a trusted referral to get it done properly. Buy cheap, pay twice, more pain, heavy cost.

Dear Dr. Daniel Choi! I only wish I could find someone as knowledgeable and honest and kind and understanding of what patients need to know! Thank you so much for the incredibly helpful and detailed overview of the Dental Implant procedure including almost all the Mechanical Engineering which goes in construction site for the Perfect Implants to last, as well as the outline of the teeth and bone healing and the miracle of implants surgery. Sadly too many people think they could get an implant done cheaper at any dentist office, and ideally they should find only a board certified specialist like you, dear Doctor! Could you please advise which kind of Dentist would be the best for Oral Surgery and Bone Grafting for the entire all front and back teeth and do it in a way not to ruin the orthodontist treatment bite: Prostodontist, Periodontist, Oral Surgeon (but most of them do the Implant and don't do the Crown)? Also, can Orthodontic treatment cause bone loss under crowned root canal teeth or what typically causes infection and bone loss on the root canal crowned years ago teeth? Could heart desease or something like slightly elevated blood sugar (not Pre-Diabetes but just a bit below it) or is it Oral Hygiene, poor Nutrition and lack of Vitamins or bad Oral Health genetics or perhaps just not being lucky to be treated frequently and regularly by an experienced and honest General Dentist? Lastly for the Bone Grafting or natural Bone regeneration after extraction, would you recommend any specific Vitamins (either supplements or food sources) which would help an Implant surgery to be a lasting successful procedure?

Is it true that bone grafting accelerates healing from wisdom tooth extractions because it prevents food from getting into the socket and helps protect the blood clot?
@DoubleDogDare54
@DoubleDogDare54 Год назад
My old man had terrible teeth when he was young and had them all extracted when he was around 20 years old. That was around 1925. When he was in his 60s I remember him crunching on radishes and green onions before dinner. My mother had dentures as well and when she was in her 80s she could eat corn on the cob, ribs, chomp on apples. That was also 40 years on. Only thing they ever did was get a new set of "china chippers" every 15 or 20 years. Now dentists need to do bone grafts, need implants just to keep dentures in place, big expensive deal. Even for regular dentures they want to do bone grafts. I remember my mother telling me the key to getting a good fit with a set of dentures was finding a dentist that knew what he were doing. These days they don't have to know what they are doing - they just fill your mouth with bone grafts, drill in the implants, pop on the dentures and hand you the bill. If you opt out of the bone grafts and implants I wonder how many would know how to do a good job fitting regular dentures? Would they even try? Or would they deliberately slop it up so after you pay for the dentures things are so messed up they will tell you, see?, now we have to do those bone grafts and implants to get things right. $$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$++++++

,@Doubledog,, Yes you are right in everything you wrote, dentist these days don't want to bother with dentures and partials that's ole skool plus they probably don't have the know how to do dentures,/partials correctly or interested in learning the correct way ,I read a few comments on dentist office sites which some patients had dentures done and they have to come back to have them adjusted a couple of times , Dentist /periodontist want where the big big bucks at which is implants.

To be honest in my opinion I believe dentistry/periodontist wants dentures and partials to be the thing of the past. Make it obsolete. They interested only in implants. Money money money. It's a money pit for patients. United States is a money hungry greedy country especially in the medical industry. Just awful the prices they charge. Sickening!!!!!!

Hi Dr. Choi My name is Luis. Your videos are fantastic. Extremely instructive. If I can ask you a question. I need extensive dental work; couple of implants which very likely will need bone grafting, tooth extraction etc.,etc., I live in Los Angeles, CA I have dental insurance. Now the great thing is that within a 10 mile radius I have about maybe at least 20 periodontists I can choose from. However, I am not sure how to choose the best periodontist. I've checked the education and years of experience of each Dr. Any other advice that will help me make a better educated decision or should I basically take the scientific method of "eenie meenie...." Your attention to this matter will be greatly appreciated. Thank you.

Hi Doc, great videos. I have had a molar extracted in the back left of my mouth for 9 years...I am just now seeing gum recession around the surrounding tooth. Is it too late in your opinion for an implant and/or would a bridge be a better option, it affects my smile from a small amount but I don't want to lose the other teeth. Thanks again for this amazing video.
@northtexasdentalsurgery

hey cody. its a possibility that you have lost a quite a bit of bone but you need a cbct 3d scan to confirm how much bone you have before saying we can do a bone graft or you're better off doing a bridge.

Great info! I'm having issues with my lower front teeth, bone is quite thin at the front and gums are receded. Back side is pretty decent, though. Only the front side is concerning. What are my options in this case? Is gum/bone graft possible, would it make any difference? Or just wait for the implant, and if so, what is the success rate of GBR for lower front teeth with moderate bone/gum loss? Thank you.

I had tooth #8 extracted 3 years ago. My dentist filled the socket with Ossix allograft (the kind that you cut and it goes in as one piece to fill the hole instead of the bone granules) and he put in only one stitch but did not use a membrane. Afterwards, it felt like I was losing bone graft material out of the socket because there was no membrane to hold it in. Five months later the oral surgeon put the implant in. It wasn't too bad until the implant was loaded 4 months later with the abutment and crown because then I started losing alveolar bone on the palatal side. I have lost all the alveolar bone behind the front tooth which drives me nuts. Not sure why my dentist did not use a membrane to hold the bone in, but is there any way to get a bone graft placed behind and above the crown on the palatal side to replace the missing alveolar bone?

May I get your thoughts on a child who has hypodontia (missing adult teeth) for upper right and lower left premolars? Should we try to protract teeth to avoid implants or how does she prepare for dental implants in the future? I know that it will be years before she can do an implant but that to me means she will not have the proper bond density for implants and will need invasive bone grafting. My question is what is "invasive" and are min-implants now an option to build/maintain bone structure now vs getting bone grafting in the future?

Hello Dr. Thank you for your time and consideration - Question: Does membrane or something always have to be used now? Do you think I should have the bone graft right away or wait and see? Here is my story - I have an issue with tooth #3 it has a Root canal, Crown and an Apicoectomy now a cavity under the crown. I choose to remove the tooth and replace it with an implant from Mexico. I visit there regularly and have already had 1 successful implant done there. I did the extraction and bone graft at a general dentist in the states w/o membrane last time. I have been to 3 Maxillofacial Oral surgeons within the last 2 weeks. Of course 3 different approaches and quotes. I have no dental insurance. (1.) extraction $325 , (optional) autologous blood $475 , bone graft $615 = $940 or $1415. (2.) extraction $284 , membrane $962 , bone graft $791 = $2037 . (3.) extraction $440 = $440 with no bone graft at the time of extraction he thinks the bone will grow back and fill in fine. Plan to place graft with implant if needed. The reason I am confused is because the last time I wasn't even offered a membrane or blood platelets. And the graft price was much lower. (1) I was leaning toward option (1) $940 because I felt like I really trusted her. (2) I did not like the option (2) at all. Interesting though, they were the only ones to mention a membrane being placed (3) Then option (3) sold me because of price and the conservative's approach. This guy is a 30yr long bone specialist and I feel I trust him too. Its just a different approach I guess. I wish I could send you xrays for your opinion. I know you said in your video "you" would want grafts done on yourself and I'm thinking that's the final answer. I am just trying to educate myself more. And of course I want to do it without a graft because I don't want to spend the extra money if I don't have to. Ughh and thank you RW

A question I can't seem to find a proper answer to is -- if you have a tooth that needs to be pulled and you have absolutely no plan or desire whatsoever to ever put an implant there (let's say the tooth is further back in the mouth where the lack of tooth wouldn't be noticeable -- or let's assume you're older in age and just don't want to do the implant), how necessary would it be to have bone grafting done there when pulling the tooth? In other words, is the only reason for bone grafting to be able to later do an implant? Or are there other crucial benefits to it that are still important even if you never get an implant? The reason I ask is because my wife has a bad tooth in the back of her mouth (not a wisdom tooth, one tooth closer to the front) that she needs to get pulled. The dentist she went to for a consultation insists that she must do bone grafting there, despite that she has no plan to ever do an implant there. He's taken it as far as saying that he will not do the extraction unless we agree to the bone grafting (which, not to mention, is not covered by insurance). The money is not the primary concern, though it's a factor as well. The primary concern is that we don't want to be bullied into doing something that doesn't seem to be necessary, and so far, based on all the research I've done online, bone grafting seems to be always tied to implants. In other words, if you don't plan to do an implant then it seems that bone grafting is not necessary. The only reasoning this dentist has given us so far for insisting on the bone grafting, even with us telling him that she doesn't plan to ever do an implant, is -- "you might change your mind in a few years." That's not a valid reason to refuse service, in my opinion. I've tried calling other dental offices for a second opinion, but none are willing to give their opinion by phone. All insist that we'd need to come in for a consultation, redo all the x-rays, etc., just to get a simple straight-forward answer to this question.

I will find a way to smoke after this procedure. I'm already bitter about it because I take very good care of my teeth and yet, this happens. An infection at the end of my roots and cracked roots. Had a root canal done about 20 years ago with a crown and the crown redone with a new, better fitting one 5 years after the original. I'm not happy with this situation at all. And who the heck has that much money to just throw into a single tooth repair? I appreciate Dr. Choi's suggested questions to ask the dentist though because he didn't really explain the procedure thoroughly, however he did really push for the no smoking thing for about 5 minutes. I could swear that the dentist told me they will be using dissolvable stitches. Guess I'll call to get more clarification now that I know what to ask about.

Can graft at the time of implant take care of insufficient bone in the area of the implant? Also, I am very curious about the nature of the implant/gum/denture relationship. In a normal denture, the gums take the chewing load. I don't understand why everyone keeps saying the implant needs to be strong enough to take ALL of the chewing load. Why are the implants not placed as "locators and fixators" and allowing the gums to take much of the chewing load as a denture does ? Especially on premolar and rear where aesthetics is no big deal. I am looking at having a bunch of this done (graft/implant) the healing of 4 months here/ 5 months there etc, it's a year plus to get finished......

My problem is that you do not consider that an Inplant could just break just because of natural causes. Or perhaps a person chewed on something hard, I had all the cleanings one should have and 3D scans but is was never detected that implant was failing. My bottom teeth are all implants and are over 20 years. Over the years I have spent about 45 000. I would like to know would you ever recommend a denture for someone instead, under what circumstances would you tell a patient to forget about implants, Would you ever feel like the patient may have had enough. I believe I had a bad implant. What are your thoughts, would you do it all over again or would you just use a removable denture

Now I'm thinking I should go get implants and straighten my teeth while at so I'm guessing I would need bone grafting after so long. That's the first I heard about it. The left and right tooth from the missing tooth are already not good. Ie one is caped and another needs a cap. I had a congenital problem with the color of my teeth, and at the age of 18, for aesthetic reasons, I made my mouth complete with porcelain metal bridges, (I emphasize on both jaws) now after 10 years, I am 28 and my teeth almost all of mine are infected and all to be removed.. I have significant bone loss in both my jaws which cannot support implants. a doctor gave me the possibility of zygomatic implants but it didn't seem like a good idea.. now I'm in Germany and here they suggest the method of bone grafting by taking it from my body or from my jaw. I wanted to ask if this is a good idea for such a large bone loss and for the rehabilitation of all my teeth.? and considering my age, is there a good success rate.? or finally any other solution besides traditional prostheses. hate to tell you this but you wasted your money. if you have a deep hole there then basically all the bone graft came out. at this point you probably have to wait till you're totally healed 4 months from now and wait to see how it heals. you can get more bone added at time of implant. maybe a good idea to go to somebody with more experience? not sure, I dont want to throw anybody under the bus without looking at your case, how bad the tooth was, what they did at the time of surgery, what it looks like now etc. I'd like to give this dentist the benefit of the doubt so hard to say

Hai, I'm from India.I have a doubt.My mother is 54 years old,a psychiatric patient,has lost her teeth over many years due to poor oral hygiene.Hypothyroidism patient.Lost weight of around 5-10 kgs recently due to not eating, probably due to pain from few remaining loose teeth.A periodontist suggested implant immediately after tooth extraction. My doubts - Is the implant necessary or dentures are okay? Should we wait for few months after extraction ,and then go for implants as her oral hygiene is poor and she has lost weight recently? Periodontist suggested implants as her bone loss is more,he also suggested cantilever for right lower teeth as bone present is very thin. Sorry for the long question.

Hi Dr Choi, my Mom had a tooth implant extracted about 4 months ago and her perio also extracted an adjacent "good" tooth at the time telling her it was b/c it was unstable. At that time the perio also did a bone graft(human/bovine mix). But after checkups her perio said the gum wasn't healing right and needed to do FGG. She lost confidence in this perio for that and other reasons so sought out 2nd opinions: 1st perio: Took x-rays. Also said gum was not suitable for implant. Described the gum as "mushy". He couldn't tell if the bone grafting was integrated or not and recommended trying to "scoop" out the mushy gum and see if the bone graft is good. He said if the bone graft material integrated ok, let the gum heal again and see if heals properly next time. He also thought CTG would be ok for it, if it's even needed at all. He thought CTG is better over FGG because it's more comfortable in the healing process. 2nd perio: Took 3D CT scan. Said gum was not suitable. He said that from the scan, the bone graft looked ok but didn't look fully integrated yet. He said there was some detached layer of floating material which probably causing the mushy gum..like inflammation. And that body will expel the foreign material over time. He said FGG was needed though because of not enough gum. He also never mentioned using a palatal stent to help with healing comfort or mentioned CTG as an option. When asked why FGG over CTG, he didn't really describe why but said for this situation FGG was better. After also viewing your gum grafting video you said you perform CTG 95% of the time and that it's a lot less painful and less bleeding compared to FGG. I'm wondering what situations FGG material is better than CTG material. Seems CTG would be preferable if possible. For me, I preferred the first perio, he took more time to answer questions but I wonder why he didn't do a 3D CT scan. Can the 3D CT scan really show definitively if the bone graft integrated ok? I think my mom preferred the 2nd because he seemed to be very booked and have more patients. But don't think that's necessarily a good thing. Anyways we're going to visit the 2nd perio again to ask more questions and get a copy of the CT scan....and also visit the 1st one and have him look at the CT. Appreciate any advice/recommendations. Thanks for videos....learning a lot!

Hello Dr, I am in a situation. I got my teeth Removed on 1/5/23 and 4 implants placed on the bottom. The two front implants already failed. There is a big indention where the implant was placed and the implant has traveled down and is situated in the valley i would call it between my jaw and lip. This dentist had a woman with no experience actually being voice coached taking my impressions for my beginner dentures and she had me bite down open up and then used her hands to smush them up wards with so much force her hands were shaking and I guess that is why my dentures have never fit at all. My mouth will not close with them in and i cannot talk and i wont even try to eat. Everything is all wrong with this whole situation and I am really worried thAt this dentist does not care at all. He didn’t even have me wearing my dentures when I left, my wife said they acted as if they didnt care or want to mess with it as it was toward the end of the day so once they discovered the dentures didnt fit at all they blamed me not wearing them out on the fact that I was acting slightly loopty from the oral sedAtive. I am supposed to go in on the 27th for them to see whats going on with the implants and i am very skeptical about anything else with these people. It seems like he barely put the implant in like right behind my bone not even down evenly in the thick part of it. I had confidence these people could do this with my reasoning being it was an affordable dentures and implants and I figured they must have performed 1000’s of these procedures. I am worried its going to be a complete shit show with these people and I live in the Area and feel like i would be better off if i came to talk to you or your establishment. I watch your videos and feel like I should have just came there. I paid 9000 for 4 bottom implants, 17 teeth removed, several still good but mostly cavities and breaks and then those rediculously wrong dentures. Idk but i am stuck out here in West Tx working and i am freaking stressed. Anyway i would like to speak to you or someone on your level and maybe get some work done the correct way. A week and a half ago i had to have tooth 13 removed. A root canal from maybe 4 years ago went bad and my face swelled up and it was abscessed very bad. The oral surgeon x ray and pulled it out. Showed me the stuff hanging on the bottom which he said was proof of abscess and infection. I told him i want dental impant. He said i am to come back in i think 12 weeks time and if it heals to his likes we will talk about dental implant options. He did not do a graft or offer one. He did a x ray with a machine that goes 360 degrees around my head before pulling the tooth. I went to my gen dentist yesterday to check in with them and he looked in my mouth and said the extraction site is healing very nice. The huge abscess lump has went down, i just used up all my amox i was given for the infection. My gen dentist said when i go back to the oral surgeon for the healing check if they like everything they will then set a implant surgery time and date. Then after that i must wait 3 to 4 months and then go back again. If it looks to the surgeons likes they then issue a formal letter to my gen dentist who then has the clearance to go forth and order the abut , crown and get ready for impression work. I can only think the reason no bone graft was done or talked of was either plenty of bone there? or maybe due to a bad root canal abscess infection during time of extraction? Its a waiting game now but i am putting money aside every friday so this can happen. Please let me know if all this sounds spot on or if i am off on it? Thank you for your time and your channel.

I think that the plan sounds reasonable. everybody does things a little different. some people may not want to graft if you have an infection. I dont agree with that personally. if you had a very infected root canal and wanted an implant in that site, I''d definitely want to graft because you could have a lot of bone loss otherwise. however like I said, many people like to do things differently. best of luck!

Can you have a bone graft without implants and bridges? Is bone graft permanent or does it dissolve over time?
@northtexasdentalsurgery
@northtexasdentalsurgery 2 года назад
yes you can bone graft without implants and bridges. bone graft will fade away after 12-18 months unless you put an implant there again because there will not be any stress on the bone in the area that you used to have with a tooth. if you want bone to persist then you will want to use bovine (cow) bone because it will not resorb like human bone does.
@rickcooper3180
@rickcooper3180 2 года назад
@@northtexasdentalsurgery Does bovine (cow) cost more? Main reason for my inquries on bone graft is because i have 3 tooth (last 3 upper left molars) that were root canal and crowned several years ago. Now the middle tooth&crowned broke just a couple of weeks ago. My dentist said this broken tooth is unrepairable and needed to be extracted and bone grafted. I am not planning on getting an implant knor bridges because i just couldnt afford the cost. If i dont have it bone graft. Will the area of the tooth cave in?will it jeoperdize the 2 other root canaled tooth next to it? Also if i get this root canaled broken tooth extracted. Will the hole closes back naturally? I told my dentist that i do not plan on getting implant/bridges but he’s adamant that i should still get bone graft. But what for? If this bone graft will Fade away in 12-18months if no implant and bridges will be done to it. And he’s’charging me $250 out of pocket share for extraction and $1400 for the bone graft. Bone Graft is not Covered by my insurance at all. Thank you

I go for a visit with my periodontist this coming monday. When stuff like that happens, is a implant still possible? Like the whole outer part of the socket is missing. The perio had troubles getting the tooth out bc it was damaged and broken. I dont understand why it failed. I didn’t have any signs of infection or dry socket and i dont have periodontal disease either. But is an implant still possible since half the socket (gum) is missing?

I payed in advance to get a discount for 11 teeth bone grafting. On that day they only pulled out 2 teeth since they needed to get approval from my insurance for partial dentures. Btw, i have medi-cal and they denied the partial dentures requested by my provider. I called insurance and asked why they denied it and cause my teeth are all bad. Full dentures for upper/lower is the best for me. Which I understood and agreed. My question is do I need bone grafting for each teeth they will pull out to get full dentures. I have no plans to get implants ever and I don’t have anymore money to pay for more bone grafting. I have 28 teeth in total and remember I already paid in advance for 11 bone grafting. Thank you so much for responding and for the advice. I had gone to my appointment and they had proposed a new procedure. Since I already had paid in full for 11 bone graft, which was almost $5k. Instead of pulling five teeth on the top, they will only pull one. On the bottom, they will pull out six teeth and replace it with a stayplate. If I agree, I will not have to pay anymore. When I looked up stayplate, it is a temporary denture to be worn while you wait for your partial denture. They did not tell me this. I feel like they are ripping me off and had enough of their business. My next appointment is next month on the 14th. I will request for my money back and just find another dentist.
