I wished Id seen the Man Medicine videos months ago. I have driven myself insane from online misinformation about hematocrit, blood clots and all this stuff. I just got my bloodwork back 2 days ago and my hematocrit was on the high side. These videos have helped me and the anxiety ive been experiencing immensely. Thank you.

What I find interesting is that the American Association of Clinical Endocrinologists released a statement essentially saying they are looking at those with "sub-normal" and "low-normal" free test levels in young adult subjects as being candidates for a trial of testosterone (TRT). This is per Quest Diagnostics, and is a relatively new notation. It's perhaps becoming their way of defining "Hypogonadism." Have been monitoring total and free since my 20s, and the fact that my free continued to nose-dive is precisely the reason I'm on now at 35. I think a lot of guys are hyper-focused on total, but really should be taking a hard look at free T. Physical gains were never an issue, and are even better now, but the 180 in terms of mental health and energy is the real king with TRT.

I a little oused 49 years old 235lbs 6’ and work out 4-5 days a week. Gave lost muscle over the years. Work out harder and still can’t get gains. Edit is good, no processed foods in 2 years, keto/carnivore. My test is 360, free is 6.9. I have no depression, I feel alright but just not myself like 15 years ago. I have energy and can go forever all day. I have a moderate sex drive, 2-3 times a week wake up with morning wood. When I discuss TRT with Dr. he is like you have no mental problems so absolutely not. I just don’t know to trust TRT clinics in NY area. I am interested in Enclomiphene maybe several times a week with an oral Testosterone like KYZATREX oral maybe several days a week. Just looking to get up to 500-600 and get free up so I can recover in gym and make small gains.

Excellent discussion. I'll provide some information for your next discussion. Being an older, skinny and very fit guy my Total T. is close to 600 without any supplement help, but Free T. stays between 2 and 4. My Doc said use DHEA, zinc and boron to counteract an SHBG close to 150. And, yea, that made sense, but so far with this treatment Total T. went up to 800 and Free T. only moved up a bit closer to 4 which is still below the normal range. Any thoughts about using a standardized brand of Tongkat Ali instead of the supplements DHEA, zinc and boron for improvement of T. levels?

I’m 23, I know it sounds like a horrible idea, but my total test is 177 ng/dl, I’m not considering starting trt to get jacked, it’s genuinely taking a toll on my life. I’m anxious as hell all the time, irritable, sleep is shit, memory sucks, lowish libido, (not exactly struggle with ED) have no energy to do anything after work and it’s taking a toll on my relationships (friends and girlfriend) I do worry about future fertility issues but if this is how I’ll feel the rest of my life I don’t think I should have kids. I did blood work 3 weeks apart, 2nd time actively making a point of getting more sleep per night and it went down 100 ng/dl. Granted lower test was taken later in day. I’m not fat by any means, I was actually pretty shredded for a little bit but my gains have turned to shit lately as well. Any advice for a struggling young man

Those are concerning numbers and symptoms. Get a proper medical workup from a qualified doctor to make sure there isn't a serious underlying medical issue and then you'll have a better idea what the next steps are. I agree you shouldn't have to live like this for the rest of your life.

I have a little shoulder acne going on ,other than that my experience has been good .I was told it could have something to do with the carrier oil and to try grape seed or MCT instead of sesame seed oil .I was also dealing with high E2 levels when I took my profile pic which has now gone away .

Been on TRT for a year or so now. My levels were so low I am surprised I didn't grow a vagina. For me the biggest benefits were actually mental. I had got so used to that feeling of "meh". I figured this was just life as a 45yo with two kids and a tired, stressed wife.

I have to keep going off of it due to it high HCT and hemoglobin levels, high red blood cell count. It has happened twice.

then try cream or oral pills, virtually no Hematocrit issues

I would think endogenous testosterone would be secondary to HCG or some other therapy but most men probably don't want to go through the trial and error process. I know I didnt.

You would think so, but for a number of reasons HCG is not a great option.

Doc….have you seen therapy cause an increase in anxiety symptoms in any of your patients? If so, what do you do about it?

Yes I have, usually in cases when levels are too high. Backing the dose down and/or increasing dose frequency can help.
