The wrist should always be locked in and secured, I do like to flare the rope at the end of the rep to help engage the triceps so I am not sure I would say that means to bed your wrist but more so flare your palms away from eachother to help separate the 2 ends of the ropes.
It certainly can be, and the answer to this question is, you should always change the starting point of the cable position to help shock the muscle and put the triceps into different positions to work different areas of the muscles. Maybe you start at this angle one week and then the next week you go lower, and the third week higher. Every angle is going to work the triceps muscles a little differently which will ultimately lead to new muscle growth. At the end of the day, its all about creating a structured routine and making sure you are staying consistent with changing up your program.
