Microsoft List - A new Forms experience to collect information 

Microsoft List and SharePoint is just rolling out with a new forms experience.
You can create Multiple forms that you can share with the Whole Company each one will have difference Columns and Themes.
The User that fills out the form will not see the list that was entered in the List (only the forms) unless he has right to edit the List.
