Welcome to another livestream from intensivecarehotline.com. My name is Patrik Hutzel. I’m a critical care nurse consultant, also the founder of intensivecarehotline.com. And I want to welcome you to another livestream today.
Today’s livestream is about, “My mom is in ICU after cardiac arrest and hypoxic brain injury. She’s not waking up and what should we do?” This is quite a common scenario, unfortunately, in ICU and I will talk more about that in a minute. So, before I get into the topic, I want to welcome you and thank you for joining me. And I also want to quickly talk about what makes me qualified talking about this topic that so many families in intensive care want to know about. What makes me qualified in talking about this topic, is simply that I worked in intensive care for over 20 years as a critical care nurse. Out of over 5 of those years, I have worked as a nurse unit manager in intensive care.
I have been consulting and advocating for families in intensive care since 2013. I’m talking to families that have loved ones in intensive care all day, every day, all over the world. And I’m also running and operating an organization called Intensive Care at Home where we provide intensive home care for long-term intensive care patients at home.
So unfortunately, I haven’t done these live streams for quite some time, and I want to welcome you back. I have been very busy in the last few months, and I’ve been very hands-on with consulting with clients, with helping our team, and looking after our Intensive Care at Home patients. I’ve been very busy. So, I do apologize because I very much enjoy doing those live streams. I just haven’t had any time.
Continuation...

My husband suffered a choking incident which led to a cardiac arrest two years in June. He was on a vent. Went through the whole cooling procedure they could not lower the meds for him to wake up because he was having "seizures." The doctor in the ICU basically told me the same thing you said. If he wakes up he may not know who you are and the quality of life may not be good. We ended up transfering him to another hospital that had a continuos EEG because they did not offer that where he was. He was transported put on a continuos EEG. No seizure activity found. They took him off the vent and put in a trach. Started lowering the meds for him to wake up unfortunely everytime they started lowering the meds he would start have severe myloclonus jerks. After messing around with meds they were able to get the myloclonus jerks under control and were able to take him off the meds for him to start waking up. After a few days of him being off of those meds he was not waking up. I would be there every day playing music for him. When I would play certain songs that meant something to us, he would start crying. I told the doctors about this and they told me it was not intentional it was part of the pysudo bulba reaction. I refused to beleive this. When I played songs that meant absolutly nothing there would be no reaction. Then it was meeting time. They pulled me into a room and basically told me that he's not waking up and I should start thinking of how we are going to move forward. Samething if he wakes up his quality of life may not be good. Again I refused to beleive this. I asked the doctors are his kidneys good? Is his heart good? They said oh yes all of his organs are good. I then said good we can continue to wait!!!! I will not give up on him. Fast foward almost two years of continuing to tell doctors my husband is in there and them refusing to beleive me. He is now in a facility still has the trach but he is awake. Completely aware of everything that is being said to him and can respond. He will shake his head yes and no to respond. Still has all of his memories. I'm so glad I did not let the doctors pressure me into giving up on him. If I can give any advise to someone going through this horrible experience, you know your loved one better than any of they doctors caring for them. They can have their medical opinion but you know that person. Never give up hope!!!

A years ago, the Intensive Care Team did their best to pressurize me. I am glad I did not give in. After 2 months, with tracheotomy, my husband was slowly waking up. 12 month after, his vitals are stabilized , no jerks and no tremor. currently, we are undergo intensive physio/speech therapies. Please never ever give up HOPE. ♥

Quality of life is very subjective. But families and patients should also understand that there is a BURDEN of treatment, when we are living but dependent on machines and other people for ongoing care--to feed us, to clean us, to move us. I do believe that life is sacred. But is it natural to prolong our life beyond natural limits with such extreme technology and machines? I also worry about the financial, emotional impact on my son, having to care for me when I am so debilitated. I worry about my dignity. I want a full life, but to let go gracefully when God calls me, and not use machines to go against God
