Although he didn't recognize her he felt the connection between them and therefore came back and wanted to introduce himself to her. With time he would start remembering her, at least her gestures, probably he would feel like in deja vu. Anyways, they dated and got married and had plenty of babies :)
he did not die... he proceeded with the operation and luckily he survived and lived but then he lost some of his memory including his memory with Rio that's why he couldn't recognize her when they met again...but then you know what they say which is true in love, the mind can forget but the heart always remembers...
plus as he said...maybe his personality will be changed and I think he's changed from one who not good at conversation now he seems so open to talk first

2022 This is stil my fav romantic movie of all time! I've watched it 4 times already but it always makes me smile, makes me cry and makes me fall in love with the characters each time!
This movie was in my recommendation in MDL for over a year now but I didn't really put any attention into it until last night. It was beautiful, funny, sad and very romantic. I fell in love with both of them! ❤️ I was happy the ending turned out like that. this is beautiful movie.. the ending song has a presumption of something playing naughty with the two characters and just when the girl can no longer bear she hug the guy same time the chorus was played.. only a typical Japanese plot can pull off.. and I did liked their choice of song.. and she's a lovely actress..
SPOILER!!! I think he was going to ask whether or not she knows him. He lost his memories due to the surgery and may not remember the people around him.
