When Nagato was a kid, the real Madara gave him both his sharingan. Since the Uzumaki are relatives to the senju, and Nagato had Uchiha eyes and a senju body, his eyes devolved into the Rinnegan.

yesi do in the fight versus konhan he has to use his other eye with is working perfectly and neither the eye nor that side of his face was scarred and with a big ass boulder crushing you like that it would leave scars/marks etc
In the show they point out that Hanzou was allied with Danzo to take down the old akatsuki. He was there leading all the leaf anbu when they tried to kill off akatsuki.

if it was hidan at the battle hanzou vs nagato he would have been there and would have started his crazy things also akatsuki would have acomplished more with him because of his imortality.
Madara gave his eyes to Obito to have them implanted in Nagato, then Nagato would use one of his jutsu to bring the dead back to life, specifically it would be Madara would be brought back and in turn give him life once more. When Pain turned on Tobi the plans changed, Tobi took back one of the eyes and used Impure World Resurrection to bring Madara back which would mean Madara still had the Rinnegan in this revived form without need of his natural eyes (or the evolved form of his brothers eyes)
Nah. He pretends to be Madara because the name itself strikes fear. Madara actually died but was brought back to life by the reanimation jutsu for the Forth Great Ninja War.
