The question is basically asking which of the following is incorrect. A is the only one that is incorrect because it should be taken in the morning and not at bedtime. The rationale further clarifies why the other answers are correct and why A is incorrect. Why do you mean by they all say the same thing?

The questions says "need further teaching" meaning the patient didn't understand and needs more teaching. Whenever you see that, you're essentially looking for the wrong answer. I made sure to highlight the "need further teaching" in red, so students don't get tricked.
