Hello Regina, I have to thank you from the bottom of heart, I am beginning to pass the NCLEX practice test, all because of you. I just took test #2 and on question 10 for LPN's, I read on an ATI book that when we are assessing a client and there are visible bruises to reported immediately to the RN, is that accurate? Thanks again, you're the greatest!!

I think it's another fancy way of saying "airborne precautions". Nclex tries to use other like terms that mean the same thing. Something you have to watch for. It makes sense cause vericella zoster falls under airborne precautions. So you need a mask.

Are you sure about number 9? I was under the impression that incident reports were all about objective data and the patient's unhappiness with treatment seems like it would be subjective

Joshua Bowen Hello Joshua I understand where you are coming from but if a patient makes a statement about their unhappiness with the care they are receiving that would be a fact. You are not agreeing or disagreeing about the care you are just stating that they patient actually made the statement.

Are the questions similar to this level ? because when i solve other nclex books i find it very difficult .

Hello Deepanshi there are three levels of NCLEX questions that will vary depending on the depth of understanding the question requires and in general how many steps that you need to take to come to the correct understanding. Many NCLEX resource will give you VERY difficult questions that are beyond a new nurse's scope of practice in order to make you think you have learned something by getting it wrong so that they present an amazing rationale. Our method simply teaches you the content and THEN ask the questions based on the content that you've studied. This makes NCLEX so much easier. :) www.ReMarReview.com
