Im not sure how to ask this question. but this my attempt. when you see the word "management" failures. Do they refer to organisations managers or are they referring to the management of health n safety systems by all different categories of workers or a combination of the 2?
A bit of all of that. Think “underlying” and “root” causes. Could be the failure of managers to do things or issues around systems/procedures/culture etc. Also think “organisational human factors”
