One of the most informative surgery sites on youtube, excellent content. Just a question: As a bald guy I want to address sagging of the jaw/chin/neck area and potentially catering for this type of client. Im aware that scaring can be hidden around the ear, from the top curving around the bottom and around the back of the ear as opposed to the hairline. Ive also been reading about the direct neck lift which addresses the globbing under the chin but this doesnt seem to address the contour of the jaw. Im wondering how flexible/creative surgeons can be in tailoring a method for bald guys. With more and more men seeking surgery and most men typically suffering hairloss by age 35, it would seem that this may be the next frontier of facelift procedures. Your thoughts would be very much appreciated. :-)
Male patients with advanced androgenetic alopecia can still have a facelift but the incision has to be exquisitely closed and while it can still produce a visible scar, it is usually well concealed in the temporal/occipital hair that persists even in more advanced hair loss patterns (ie Norwood VII). The main issue arises, of course, mostly in patients who wish to shave their head. This becomes a risk/benefit analysis that the patient must consider. The situation is actually much more favorable in younger patients with only mild to moderate aging; this is for two reasons: 1) the facelift incision may not need to be carried into the temporal or occipital regions, meaning that the incision is then perfectly hidden around the ear, and 2) many younger patients actually do not need a facelift or a direct neck lift to get a sharp neck & jawline; instead, a submentoplasty with an anterior platysmal plication may be all that is required, as long as there isn’t significant lower facial aging. Most people think that getting rid of a loose neck requires skin excision, but that is not always the case; it actually requires that the loose platysma muscle be tightened. That is essentially the basis for all lifting procedures in the face. If you think about it, in the loose neck, the excess skin you see is about the same amount of skin you need to “hug” and contour that same neck when tightened and sharp. To the disbelief of many of my patients, the most dramatic changes in the neck don’t necessarily require a large skin excision, because the skin can adapt to contour quite well. This means that a facelift incision may not need to be long or even done altogether. Skin excision becomes more important in more advanced aged pts, typically 60-65+. For patients aged 40-55, I would recommend that they consider a submentoplasty with anterior platysma plication, which only uses a 1-inch incision in the submental crease of the chin. If a facelift would s required, then avoiding the temporal and occipital incisions may be possible if aging is mild. Best of luck, and thank you for watching.

Excellent. Only question is where are the incisions located for each of these fixes, to remove the excess skin? The incisions shown here address the muscle laxity, lipo, bone, implants.
In the pictures at the end, I noticed that the majority of the "before" pictures had a lot of the patient's forehead showing, whereas after the surgery was done, the foreheads were mostly covered up with the patient's restyled hair. Would this indicate that by showing less face/forehead, it makes your neck look better, even without surgery?
Do dr danny soares do deep neck surgery with removal of subplatysma fat and submandibular gland reduction and reduction of the muscle in event of hypertrophy on younger patients who is no need of skin removal

That is a very good point. I did not mention the anterior digastric corset because myself and many others perform it routinely with our anterior platysmal plications, even in non-Dedo VI necks, to help improve the contour of the cervicomental angle but to also better "draw in" the submental tissues and better define the jawline. However, in reality, and in my opinion, the digastric corset plication against the mylohyoid does not in itself substantially alter the position of the hyoid bone but instead merely maximizes the contour of a low-hyoid cervicomental angle by maximally lifting the submental area and ptotic floor of mouth in conjunction with the eradication of subplatysmal supra-hyoid fat. It is not a panacea to Dedo VI necks, but it makes a facelift result much more impactful in such cases. Thank you for your comment.

Here in France there is Dr.Knipper who did plication of posterior digastric and he mesures hyoid going up 1.5cm and 0.5cm posteriorly. I want to ask 2 question: 1)Im gonna have neck lift in 6month, my surgeon is gonna do corset platysmaplasty + digastric corset. But i wanna ask him to add platysma suspension (which is smas lift to mastoid fascia, correct me if im wrong). do you think that the platysma suspensî will increase my results by 50%? -I want to add plication to posterior digastric, can this be done by the submental incision and not around the ears? [Because my surgeon dont really want to dissect my smas cause im only 21, but if posterior digastric can be lifting from submental incision i will ask him to do it
should I wait till my Jaw Surgeon finishes a BSSO to get a submentoplasty or should I get it from someone who specializes more in that surgery and then the Jaw Surgeon can do his work without needing to take it into consideration. I believe I have a type 2 but maybe 3 neck
Robert Yeahright definitely let the maxillofacial surgeon perform the BSSO because that in itself can improve/favor the cervicomental angle and therefore also aid the submentoplasty result. It might even obviate the need for the submentoplasty, though that might be a stretch and is not as likely. I’d recommend waiting 6-12 months following the jaw surgery and then seeking a facelift specialist that performs anterior platysmal plications routinely to do your submentoplasty. I say this because an isolated submentoplasty will truly test the skill of the surgeon’s plicating technique, and I’m speaking from experience! Also inquire on whether the surgeon is planning on removing any existing subplatysmal fat, which is sometimes critical in order to achieve a sharp result.
