This was like a dark souls difficulty cardiology paper though the medicine part was much easier. I also gave endocrinology where the medicine part was ridiculously difficult (even more than this cardio part) but endo part much less so. The examiners need to do better difficulty balancing.

Question 60: answer should be digoxin As patient is in failure Beta and ccb can be contraindicated Question is quite ambiguous

Q is clear.BB is contraindicated in acute HF when u have a myocardial pathology like acute MI or DCM where u have systolic HF.Here the cause of HF is AF with RVR with preserved systolic function and the best drug in that situation is a BB to control rate .Digoxin is seldom used in modern day cardio,mostly as an add on drug

There is a wrong concept that BB are contraindicated in all acute heart failure.It requires further conceptual understanding.

All the questions are ss exam is short stemmed and foolishly represented option , it is quite shorter stem questions than USMLEs
