Thank you soo much for this well explained demonstration ❤ For me I had difficulty to go with laryngoscope from right side of tongue - since I decided to insert it directly from the midline , it was easy for me to reach the vocal cord view

I am a pediatrician from Union City Tennessee. I cover a level 1 nursery in a rural hospital. Thank you so much. This is so helpful. I have to stabilize and intubate babies while waiting for the NICU to arrive. It is stressful at times trying to intubate.

Intubation in newborns is used in several critical situations to manage and secure the airway. These include: Respiratory Distress Syndrome (RDS): Common in premature infants whose lungs are not fully developed and lack sufficient surfactant, leading to difficulty breathing. Meconium Aspiration Syndrome: When a newborn inhales a mixture of meconium and amniotic fluid into the lungs, causing blockage and respiratory distress. Congenital Anomalies: Such as diaphragmatic hernia, tracheoesophageal fistula, or choanal atresia, which obstruct the airway or impair normal breathing. Severe Infections: Including neonatal pneumonia or sepsis, which can compromise respiratory function. Perinatal Asphyxia: When a newborn experiences a lack of oxygen before, during, or just after birth, leading to the need for immediate respiratory support. Premature Birth: Very premature infants often require intubation due to underdeveloped lungs and weak respiratory effort. Surgical Procedures: Infants needing surgery, especially those involving the airway or chest, may be intubated to ensure a secure airway during the procedure. Neuromuscular Disorders: Conditions like spinal muscular atrophy or congenital myopathies that can impair the muscles needed for breathing. In these cases, intubation helps to secure the airway and provide adequate ventilation and oxygenation to the newborn.

miller blade intended to lift up epiglottis, mac blade are meant for the vallecula

Honestly- I know some institutions use Mac blades- but all we've ever used are the Miller blades. And then depending on how tiny the baby is- we'll use it for the epiglottis or the vallecula. But you're right- in older kids and adults that's how they're used!

Thank u but how did u decide the depth of tube. If u explain it it will be good .but the demonstration video is too good .liked it , i m frm nepal jst started working in so it was curiosity that how did peditrician intubate .thank u alot

The depth of the tube is about 6cm + weight in kgs. For tiny babies it’s smaller than this. Eg 500g baby normally but ~ 5.5 cms. We should still get a CXR to make sure the position is correct. So glad you’re watching from Nepal!

Love the video, I was wondering if you have any tips for intubating a big kid term that is fighting you? At my NICU, we don't use sedative.

Hello! My big tip would be to use sedation!!! If no IV then intranasal versed works well. Esp if a baby fighting you! No other real replacement for it. We’re planning a separate video on sedation and set up etc- haven’t gotten round to it yet!

What is a tiny baby? Under 2k? 1k? At what point should you switch from trying to lift the tip of epiglottitis to lifting indirectly via the vallecula?

Oooh that’s a great question. I’m trying to think about my cut off (I tried researching this with little success!). I’d say 25-26 weeks and under- so less than a kilo.

Hi, why do do you insert the blade on the right side vs straight down the middle? I read people who insert from the right sweep the tongue out the way in adults, but that is not typically done for neos I believe?

Great question! In bigger babies- especially if they’re more active, then we do need to sweep the tongue out of the way. Their tongues are after the biggest battle towards extubation. In smaller babies- you’re right- the tongue isn’t usually an issue- and you probably could just put the ETT down the middle- but I always think it’s good to get used to the same process. I generally try to do the same thing with all babies!
