So, let’s talk about the recent scandal concerning the Nepal USMLE cheating ring…
For those of you who might not know, USMLE is a three-step licensing exam for physicians, residents, and aspirants alike who want to practice medicine in the United States. Here, applicants aspire to have the highest score possible, especially residents who want to attract the attention of residency program directors and get themselves onboard.
And just recently, the National Board of Medical Examiners announced that they’re invalidating the USMLE scores of a group of residency applicants and current residents due to validity concerns.
When I first saw it, I hate to say this but I wasn’t really shocked. I have always felt that the opportunity for an organized effort has always been there; however, I always assumed it would be difficult to bring it out.
But then this recent USMLE scandal happened, and not only were there people who had an advantage; now, there’s a systemic advantage with such a coordinated effort to make it happen.
This is such a big scandal that could and has already affected lots of people out there, and it’s something that we cannot afford not to discuss. That’s why in today’s episode, we’ll be learning from Dr. Bryan Carmody himself, a pediatric nephrologist and a medical school educator. He is known as the Sheriff of Sodium on social media, where he does commentaries on residency selection and medical education.

Im a vet and my husband is a psychometrician and this topic was fascinating for us. He believes that the NBME will have to pause the test until they create a new question pool, as this one is severely compromised. Also, as a foreign vet, the ECFVG has us pass the CPE, a 2 day practical exam covering 5 areas, small animal internal medicine, large animal IM, surgery (spay), anesthesia, and pathology (necropsy). How come the ECFMG doesn't have something like that for IMGs?

what interesting is we talked about what you could USMLE do about the test and the concern is how long it takes to create a test that getting a new question pool would leave multiple classes without the ability to take the test.

Unfortunately that is the reality of a compromised test. My husband worked on k-12 testing and if a student during COVID took 1 screenshot of a math test they went into code red and started redoing the whole test. I'm just so confused how the NBME is not taking this more seriously as it has been going on for at least 3 years.

I have a question. Was it 832 examinees who got their scores invalidated or 832 scores invalidated?

I heard 832 doctors .. But it doesn't matter because this is the tip of the iceberg and many invalidations will still happen!.

We need to think to revisit all Nepal's USMLE passed student record and think about to appear retaking the test as a opportunity otherwise it would be unfair with US and US citizen studying medicine in Caribbean.

For a document to have 95% of the test questions with answers it is probably due to more than recalls. Someone suggested that it is possible that these people hacked the equipment

We haven't heard of them getting that in deep as far as the cheating is concerned but I will keep a look out on it

So you are accusing them cheating because they study, recall or practise more than you and your fancy peoples ? wow what a psychopaths they are. what a shame.

you are emotionally charged, you know its very wrong! That was the most blatant cheating! You aren't suppose to share questions with anyone else thats what you sign for, and then creating 1000 page question bank from recall, I don't believe its just recall, there has to be screenshots etc. involved, and sharing between everyone without USMLE authorizing it is blatant copyright of their material on top of it all! Its all bad no matter how you look at it.

@@oma_gooner tell usmle to don't put repetative questions. If they don't have enough iq to put new questions each and every time they don't have rights to take exams. Handover to other body to take exams. The exam taking bodies should be sued for not generating new questions, putting same and repetative questions every year. And what meand by 99% cheating. Dude if theres cheating it has to be 100% or no cheating. What a psychos. Didn't know america was full of psychos. Love and support from Russia.

Some of these students come from poor and corrupt countries so it's not surprising for comments like these. Either they don't understand professional ethics or just don't care. Each country has its own medical licensing standards. The main reason for this is to protect the public. If you think a student memorizing the answers of an exam gives them the medical knowledge for life and death decisions then you need to rethink your standards. I personally would not want my surgeon held to the standards of what elementary school kids could accomplish 😂

I'd encourage you to look more into how people got 'caught'. It wasn't simply high scores, it was questions being answered in less than 30 seconds with 99% accuracy. Now, if everyone scored very high it's difficult to say a person cheated without evidence. But, these recall sheets also had answers. And when people who are scoring in the top 10% of test takers are all missing the EXACT same question with the EXACT same answer on a question that is 'easy', it makes it suspicious, and statistically improbable that it is simply due to chance (in fact, statistical models have found the likelihood that this is by chance as less than winning the lottery). It isn't about how high the scores were, it is how rapidly every person answered certain questions with extreme accuracy (the same questions), as well as how unlikely it is that these same elite scorers were missing the same easy questions with the same exact answer (indicates that whoever compiled the sheets also provided an answer key, but made a mistake on some easy question and no one bothered to fact check it). Also, the sheer amount of people finishing the exam incredibly early, uncovered chat groups, etc. My guess is that they also have gotten their hands on the recall information to more accurately assess the question/time/response criteria. I highly doubt this is limited to Nepal, and I even more highly doubt that every person that took the test in Nepal was part of it. If I had to guess, it is the story was getting out and someone was going to break it before USMLE had concluded their investigation (I'm assuming they were/are going to go through every country/test center/ school with a fine combed tooth looking for data to take action and make a very very strong deterrent against this type of thing happening again) , and USMLE didn't want to look inept and reactionary, so they decided to get ahead of it and release their findings related to Nepal before the entire investigation was done.
