I'm currently using Zathura as my pdf reader; among other nice things it has vim keybindings by default (pretty convenient if you're on i3) and does the periodic auto-reload you mentioned.

Regarding images inside emacs: sometimes I just want to paste an image, and I don't care how this image it's called. When I paste screenshots from a game I am developing, I can take 10 screenshots in 5 minutes, it's extremely annoying to first save them in right place, and then name them again in emacs. Maybe you have a simpler solution to that?
