Ports and IPs you mentioned for Teams optimization are they needed to be opened from base machine or from the AVDs/Citrix VDIs?

Base machine (not required if ur base machines are in open network )

No, teamsbootstrapper is not supported on server OS as of now. But instead you can use general command to add an app package which is add-appxprovisioningpackage.
