Cataract surgery leads to dramatical improvement in vision, however cataract lens choice may lead to some vision problems during the night.
The problem explained in this video called nighttime dysphotopsia, and may happen with multifocal IOL, trifocal IOL such as Alcon Panoptix or Zeiss AT LISA, but also with modern EDOF IOLs such as Tecnis Symfony.
You may think that monofocal IOL is better option, but you have to know that even with monofocal IOL some patients experience halo or glare issues.
Moreover some eye conditions such as early cataract, myopia, hyperopia or astigmatism may cause nighttime dysphotopsia.

@@iol-adviser Does high myopia (e.g. -5 or more) affect the choice of lenses for the IOL operation? Are such lenses as vivity and eyhance possible to use in these cases?

@@isultan well, good question. Myopia pf -5 is not as high, however sometimes you may have some issues with contrast sensitivity reduction. Normally -5 does not have that problems, so you theoretically eligible for any IOL, but my personal opinion that CS (contrast sensitivity) is major factor in vision quality, thus Eyhance is preferable to me, and if you want more freedom of glassess while keeping high contrast, Symfony is better choice. Vivity has significant contrast reduction, and as any PC IOL might have PDs, at least there are some patient feedbacks. So, the key while answering about IOL type is to know your visual needs first.

@@BM-ms3gr Can I ask, where your eyes re-checked and measured before your op, not just in the consultation? I haven't had such a great result. I am experiencing loss in contrast and ghosting around pretty much everything. Frustrating. They also didn't get the prescription correct in my right eye or it isn't positioned correctly. That is difficult for me to find out as I am sure Optimax won't want to admit to that. I had the Alcon Panoptix lens. I didn't know how to research them. I wish I had done even more research and I thought I had done loads!

@@thegoodgriefconversation4515 I also had lens surgery 4 weeks ago, only on one eye (wasn't comfortable doing both). My night Halos/starburst are awful, so I have stopped driving at night. How long did your brain adapt to reduce the halos/starburst?

i know someone who got monofocal recently. Sees great in day time but disappointingly she sees halos at night such that she doesn't want to risk driving at night. But then again she had nighttime halos from cataracts before the surgery and didn't drive at night. Overall she's glad she got the surgery.

Everything you said is correct. If you have any sort of dystopia, just ignore it. I am in my second week with my new lens and initially I was focused on it. Now, it is so much better and continues to improve. My exchange was for glaucoma reasons. Im cured!

One of the main issues to get cataracts removed is to do away with nighttime halos & glare, not spend thousands on lenses that do the same effect!

I would warn people from IOL I have three or more large halos in each eye from light sources even daytime if it's LED. Now I can't drive at night and don't even feel confident walking around as the halos flash as the refraction changes. This isn't a side effect that my surgeon warned me about. I am now going to have them removed.

I just had a lens replacement a week ago. I was disappointed when I was told by the doctor that I could not have trifocals due to some issues with my right eye retina. She said that there is a chance in the future my vision will be distorted and I would not be happy with the results. She recommended mono-focal. My vision was so bad and I decided to go with it, to fix at least the distance. Little did I know that a mono focal was actually going to improve my close up vision too. Typing this without any glasses and only a week post surgery. The distance vision has never been any better, sharper, contrasty, vibrant, since I was 20. My close up vision is not perfect but I was not expecting anything at all. This has turned out to be one of the best decision I have done on the recent years. Will have a video diary of my full experience soon on my RU-vid channel.

@@musafir123 What I found out in my searching, was that the Multifocal gives you good near, mid and far distance, but has some limit on light. EDOF, gives you good mid and far distance, not the neat distance, but let's in more light than the Multifocal lens. I know someone who had both eyes done "Multifocal", and loves it. In my situation, I was just concerned, as a commercial driver about having as much as possible in my vision, and my eye doctor agreed. I do not have any glare, halos or starbursts in my vision. I saw different videos talking about this. Maybe I just had a great doc, but I have no problems al all. The combination of the two different types of implant lenses works great for me. I would say, that if you want to pick one or the other, and not a combination (go with both in the Multifocal) The Multifocal lenses are great! I hope this helps you out. Also, research as much as possible the doctor you are looking at. Customer reviews on any social media sites.

I suffer terrible Dysphotopsia especially at night and in day time I can see the edge of the IOL. I am not considering another surgery because I am terrified that more damage can be caused to my eyes. I have J&J ICB00 and monovision. I am now 6 months post refractive Lens Surgery. My take on Dysphotopisa is that the industry understanding is either limited or in denial for purely for commercial interest. The idea that Neuroadaptation is the magic cure or that the patient can simply ignore dysphotopsia is without evidence. Since there is no mean to report or record dissatisfaction, manufacturer can report whatever they please. However, it is a good video and informative on Dusphotopsia

Hello. Sorry to hear that… the situation you are describing might be referring to the dry eye syndrome. Poor tear film status could be a reason. Do you know anything about your ocular surface ?

Worse decision I’ve ever made was getting multi focal iol transplant for myopia just because I wanted to be “glasses-free”. Never mind the optic oedema, infection, night blindness, dry eyes, acquired presbyopia, halos, glares, lens rejection, etc., but regular risk of surgery too. Unless severe grade 4 cataract, I’ll never, ever recommend anyone to get them. Wish ophthalmologist are more honest to patients for other options. JUST STICK TO GLASSES OR CONTACT LENSES IF YOU CAN. It’s been 2 years. Not worth it at all…buyer’s regret. Don’t do it if you’re considering.

here I am looking into this because Ive heard it can be an issue, and as I dive further into what exactly it is - Im realizing Ive experienced these nighttime disturbances around light sources my entire life, with or without my glasses on, and without ever even knowing any different. Im 40 and my entire life I've just thought that this was how streetlights and headlights look at night...

Thank you for the information1. I have a question, I had cataract surgery yesterday with a monodical IOL. went well, they said. I was noticing yesterday and more today as I pay attention, that I see the OUTSIDE of the IOL, I think. It is like wearing eyeglasses with semi-clear-plastic rim, and I see the arc of the rim on the outside (left of center, about 60 degrees left of center) - when I have bright window off to the left side. (Left eye IOL.). This arc tracks with my left eye when I move eye horizontally. Is this a concern, or normal?? Is the IOL detached?? the eye doctors offices are closed until Monday morning (Friday at 3:30 pm now). Thanks for any help1

Hello doctor. Ι have hypermetropia 1,5 and presby 3,25. My doctor says go away from multi lens He suggests monofocal clareon. He is sure for the result Perfect vision and only 1,5 for reading and no issues for Ever. Im 45 What is your opinio??? . Thanks

Dear sir . I had monofocal IOL put i my right eye 1 1/2 weeks ago. my night vision was very good in both eyes , now my right eye is worse at night but my left eye is good . I don't know what to do . Left eye sx is on the 4th of October. If my left eye comes out the same I won't be able to drive at night , to dangerous. At day time right eye is still blurry , right eye is blurry. Please advise. Will it get better ? My night vision was great before. Don't know what to do.😢

6mths after double cateract surgery and I still have both negative and positive dysphotopsia in both eyes. Very annoying and distracting... Even during the day. Not getting better. Only thing that helps is sunglasses day and night..

Excellent content but “not focusing on the glare/starburst/halo effects of a cataract” is pretty impossible when driving at night as they are ALWAYS THERE.

If you go into the settings at the bottom of the video (the star icon), you can change the "pitch" which lets you speed up or slow down the speech rate. I find that using a custom setting of .85 adjusted his speech perfectly for me. You can experiment what works for you.

On a monofocal lens, what is usually the minimum focal distance?

As someone who has had the surgery, you will see clear for TV viewing and driving. If I had to guess, it might be from 6-8 feet and out. What will be annoying at first is for those with a desk job. For a computer you will need light reading glasses (+1.0 - +1.5?) but someone across the desk at say 4-6 feet will be not quite clear with or without the glasses. IOW, somewhere in between. I worked in an auto shop and my compromise was some +1.0 readers for general work under the hood where most things were 3-6 ft away. If I were in an office setting, I would go for a pair of progressive lenses... clear on top and say 2.0 for real close-up work. The last few years before retirement I worked outside and with the extra light, I found I didn't need any glasses unless I was trying to read a repair manual. I carried 1.5 readers for that. These days I am retired and very rarely use my readers unless I am grocery shopping and need to read the ingredients on a can of soup. BTW, I did have one eye done slightly stronger that the other. Took about 6 months for my brain to adjust but it works well in most cases... and helps mostly with that intermediate distance around 5-6 feet. (car dash as well)

monofocal calculated for infinity as a standard setup will give you a good sharp vision from about 2m and beyond. There are other setups available like -2 to give you reading ability but use of glasses for far vision. and vice versa - far setup for monofocal means near reading glasses

I had epi lasik it’s been a week still my vision is not clear. I can see starbursts at night but in the day it’s blurred. What should i do now? I’m scared.

Do nothing. If might be an issue for few days with blurred vision. However starburst might be for long period. It is normal effect for some patients, and your doctor had to inform you prior to surgery.

On April 7, I had the Vivity IOL implanted in my left eye. My reason was because I had cataracts, narrow angles and my doctor told me that the Vivity IOL would give me no halos and I would no longer need glasses, except maybe for fine print. I don't know if they measured my eyes in correctly, but I am having several problems now. Immediately following the surgery I was able to see and was pleased with the absence of tan. However by the time I returned home which was about an hour after the surgery. I was not able to see (except for peripheral vision) because of corneal edema. I also paid extra for the laser, which may have been a bad decision. After the surgery the doctor told me that I would have more swelling than usual because my cataract was thick. The doctor's office also forgot to give me eye drops to begin taking before surgery. I am able to see now but I cannot read except with my old prescrption sunglasses. At night instead of having small halos from the cararact, now I see huge lines like whiskers across my field of vision, which is much worse than before the surgery. Other extremely annoying problems are a strobe like effect which happens when I am trying to to read or work on my computer, and I can see the edge of the lens so it feels like I am looking through a monocle. I was planning to have cataract surgery on my right eye, but now I don't know what to do. My right eye is my dominant eye, but I am at risk for narrow angle closure.

bad story :( I would suggest try another surgeon, as here is everything looks like not ideal technique or some surgical issues. however - it is just my expectations based on symptoms, so either ask your surgeon to comment ("thick lens" is nothing to explain, as lens thickness is evident before the surgery and if there is any risks, you should have been informed), or ask for a second opinion at other clinic...

If you look at the Alcon web site, you will see that they explain the quirks of the Vivity lenses. Every Vivity complaint I have read is predicted in the Alcon literature.

I had cataracts removed in both eyes, and a Panoptix multifocal placed in my right eye, and a Vivity EDOF in my left eye, and I see great! I was worried as a commercial driver , driving during both day and night hours that these sample descriptions from people of Starbursts, Halos, ect, had me worried. After a couple days post surgery, I saw very well. Going with the two types of lenses worked great! Not even close to the pictures in the video...... I am very happy with my results and just wanted to pass my info on to anyone contemplating surgery.

it is absolutely subjective first and depends on eye status.. as an opposite example - just today I consulted the patient with Vivity, so he is not able to see as he said "anything that is not shining" - he can not drive with Vivity as he can see the light source, but can not see shadows, and in his case doctor have promised good vision based on "professional driver experience". This case was a doctor's mistake, who did not count in Vivity technology-specific light distribution.

I got the same plan - for panoptics in right eye two days ago and in less than 2 weeks, I get vivity in my left eye. My right eye was 20/20 in just 24 hours post surgery!

Mine is streaks That come from my Peripheral vision like from street lights or lights in my house that’s in my peripheral every single light in my peripheral vision has a streak going all the way across My vision.

Hello, I have recently underwent an RLE surgery and got tridiff multifocal iol implants in both of my eyes. I have very very high myopia, -17 left eye and -19 right eye. I am happy with my distant and intermediate vision but not really satisfied with the near vision. I see shadows around texts when I look at mobile screen and laptop which is making hard for me to focus. Is EDOF as a piggyback IOL to achieve a crisp near Vision possible to implant? What are the options to achieve a sharp near Vision? Thank you!

THANK YOU!! This is very informative. I had IOL surgery two months ago and I have been challenged by dysphotopsias, halos, focus depth (sometimes things appear in focus at different ranges and then other times they appear out of focus - I now understand this is my brain adapting! I will be patient) contrast and ghosting around objects. They are all pretty disturbing, especially the contrast and ghosting. I wasn't prepared for either of these. Working on my computer and looking at photos and the TV, is challenging with the ghosting. After watching your video, I will try to ignore this effect and hopefully, my brain will start to remove it. My surgeon used the Alcon Acrysoft Panoptix Tri focal lens. Are my dysphotopias to be expected from this lens? Thanks :)

Brain plus an important role, however proper refractive setting of the eye and cornea surface is important part of that. If something is not good , your brain may not fix it in full. So, the most important is to be sure in perfect optical outcomes first. If effects will persist try to talk to your doctor

@@iol-adviser Thank you. I had my first consultation in June and the operation on the 14th of August. I was very surprised not to have my eyes checked and remeasured before the surgery. I was literally taken into the operating room after having all the drops put in my eyes. Could this be a contributing factor to the problems I am having? How can I get accurate information from my optometrist? Shall I ask for my notes and treatment to be printed off?

@@thegoodgriefconversation4515 not, however the key question is how your eyes was measured and calculated pre-op, have you used contact lenses, do you suffer from dry eye, and number of other factors. regarding optometrist, as far as I know it is not easy to get records sometimes. it is number of points to check, far vision acuity, refraction, lens position.., however it does not make sense unless your surgeon is ready to discuss the problem and how to fix it.

How could I find out if the focussing in my right eye is down to not having the correct prescription in the lens or that it may be positioned in the wrong setting? Had implanted Alcon MA60MA IOL (-3 OD, -4 OS) My left eye has great night vision, without dysphotopsias, but in my right eye i experience starbursts at night, even one year after the surgery...

Hello doctor. I am 37 male active and fit and I have cataract surgry in only my left eye in 2 weeks. The cataract caused by injury 4 years ago and my vision is so bad in my left eye now. My right eye is healthy 20/20 with no cataract. I also have 2 d astigmatism. My surgeon recommended me penoptix toric or vivity toric and she said let me know your choice in 1 week before your surgry.(They only have these 2 options as toric lens) She is highly experienced but I haven't yet decide which one should I choose. I need my near intermidate and far vision but I also worry about night time driving issues regarding penoptix lense. Would you please let me know which one has higher satisfaction in your patients and generally which one you recommend the most ? Thank you so much🙏

I did have cataract surgery completed in both eyes in August 2022, I had Vivity lenses implanted. I am experiencing horrible halos, starbursts, almost a blurry vision around headlights at night and during the day (not quite as bad). What I want to know is if I choose to have them removed, what are the risks involved?

I believe I had the same one done, but exactly a year before you did. I have not been confident driving at night anymore and I get very anxious driving as it gets darker because of the glares. I try to limit it now or just get a ride from family or friends and if not, I just don’t go out. It’s been depressing since I started off my driving experience driving at night and I did enjoy going out at night with friends 😢 I’m still hoping it gets better overtime, since it’s only been almost 3 years. Also still trying to look for other helpful solutions like particular glasses that help with this specific kind of night glare for cataract patients like us. ❤

I never blink.. I am not a human ... LOL :) actually it is contact lenses + concentration on camera + video edition (some phrases should be repeated, thus some are cut off..) make this effect possible. Hope it explains :)

Thank you for your illuminating videos! Could you comment on seeing stars in the night sky with these different lenses? I would not want to lose the ability to observe the night sky. If I understand correctly, with multifocal and edof you will lose some (maybe a third?) of the brightness. Will stars also become blurred?

Hi, I have had multifocal lenses. The night sky is important to me too and it never occurred to me that I may have problems after surgery. They still shine brightly, sometimes with a soft glow around them. After watching this video, I am REALLY hoping my brain will adapt. Also, looking at the moon is weird. I now see glare around it :( Again, I am hoping my brain will remove this information so I will not give it my attention. There are so many other benefits though. I am not sure what my decision would have been had I known this. Fingers crossed it will go away. I am only two months into my recovery. I hope that helps.

Hi, i have had monofocal Acrysof, then exchanged that to trifocal Panoptix, the night time problems from light are almost equal to zero! I'am 27 years old with traumatic cataract with second eye healthy. The same for contrast it is not noticable. If any question fell free to ask!

Great video. I had a LAL lense replacement, because of defective lense. It was replaced with a traditional lense. Daytime vision is great, however last nite I experienced double vision. My left eye is for reading and the right for distance. What is going on?

😎After I get my multifocal IOL's I'm hoping I can wear my Serengeti photochromic driving glasses (lens by Corning) day and night like I used to 6 or 7 years ago. Since the lens adjusts with light you can see through them at night/dusk. Check out the Serengeti "Summit". Awesome glasses for glare!

I just had a cataract surgery and here it comes Christmas again. I see some kind of tinsel-type of stars around some light points. And they are massive. I just hope it will reduce as it’s only been 9 hours after operation. My eye is still dilated. Can’t see well at all - all blurry. I don’t know if this will ever improve. I just wish I never had this cataract. I was forced into this operation because of this. I wish I had my old eyes with glasses with high myopia. I will see how I’m tomorrow.

In the computer simulations of the nighttime dysphotopsias, the 2nd picture you stated as an new type of monofocal IOL with improved intermediate vision. Would this be an EDOF type of IOL? Thanks. Your lecture was full of thorough technical information and useful in helping me make a decision about choosing an IOL in my upcoming cataract surgery. After viewing several of your lectures and other RU-vid videos by ophthalmologists my choice is down to 2 monofocals set to distance & using readers (lower cost, no dysphotopsias) vs. an EDOF (better range of vision, but much higher cost and some loss of contrast).

Thank you for such a well presented video on the topic of dysphotopsias. I'm am scheduled for cataract surgery and I am concerned about the nighttime driving issues with some of the iol's being used. I currently experience the halo's and starbursts which make it extremely difficult to drive at night. My wife had her cataract surgery performed a few years ago with the insertion of monofocal lenses and she still experiences the nighttime glare and halo's. Her surgeon had warned her that the situation would probably not get better post surgery. I am hesitant to use this same surgeon to perform the surgery for me since safe night driving is very important to me. Is it you experience that some monofocal lenses might be better at attenuating the effects of dysphotopsias over some others? Was the Tecnis lens demonstrated in the video a monofocal lens? Are you familiar with Bausch & Lomb's SofPort AO lens? I read that they were one of the best at diminishing the visual nighttime affects of dysphotopsias. I do have a low corneal epithelial cell count of 1700 so I may not be suited for some types of lenses. I went to your site and filled out your short multi-choice questionnaire. I believe it really hard for him to make replies or even monitor this video at this point as he lives in Ukraine and Russia (aka Putin's ego) has been systemically taking out their energy grids trying to force them to relinquish their independence. The country has had to impose limitations on energy usage in order to try and conserve what they have so they can make it through the winter.

Hi Kristina! I am sorry for your experience.. Unfortunately, "anti-glare" glasses will not help. The only idea try to ignore that issues and help your brain to adopt. However you may need a long time. Another question - what is your residual refraction and lens type.. if may affect that issue. Try to check your vision (far should be 20/20, and if you will have some glasses which improves far visual acuity - that optical correction may reduce halo effects)

I really appreciated the idea of not focusing on the unwanted negative effects of light at night to improve the conditions. I got the Panoptix in my left eye on 3 days ago and had giant halo rings of light the first night - and around lights inside too. Mostly went away the second night, but now I see a horizontal line through all lights. At a stop light the horizontal line from one stop light actually joins together with the other one. I've never heard of anyone else complaining of that. I'll try not to focus too much on it and see if it goes away. I'm curious to know if you have any other ideas or exercises to help improve vision with these kinds of lenses that would help the brain adapt better and faster. Or is it better to just let the brain do it naturally on it's own?

Hi Doctor . Your video is the clearest and most concise I have watched so far and I have watched a lot. I just had my left eye done yesterday ( 18 hours ago to be a act)with synergy multi focal. I am unpleasantly shocked to the point of almost fear over how bad my halos are . It’s literally a light show at night around any bright light . Full halos competing with other halos. My pupil is still somewhat enlarged . Should I be concerned ? Will it at least get better if I follow your instructions as per your video or are some people that get it as bad as I have it( halos) just destined to be stuck with them? I do have my follow up meeting today at noon as I am writing this at 6 am in Toronto Canada .

ask your doctor to check residual refraction by "maximin plus" technique, and look at corneal surface quality. but cornea diagnosis had to be done preop

Now that I am on my third day post surgery I am experiencing a feeling of ‘a little something in my eye’ like a tiny piece of sand or something . Also a litttle flicker from the left of my eye. The ‘ something in my eye feeling’ will it go away in your experience ? What about the flicker ??

@@stevec8836 Hi, I had surgery two months ago and I went through everything you are. Two months in and the halos are smaller, though driving they are still quite large at a distance but I know that as the vehicle gets closer they go away. That knowledge helps with driving. The flickers from the outside of my eyes are now a lot less, just occasional. I believe that is something with light bouncing off the lens and reduces with healing. I had feelings of grains of sand in my eyes. This completely disappeared once healed. I am told my eyes were slow to heal. In fact, they have probably only just healed. I would suggest keeping the shields on at night for as long as you can. I went back to wearing mine after 7 days and I am sure that helped. I felt more confident sleeping at night as I didn't want to knock the lenses. I hope that helps. Good luck with your recovery. This is a brilliant video. I kind of wish I had found this when I was doing my research. Can I ask, when you had your surgery, did you have your eyes checked and measured again on the day of surgery?

I know I know I’m probably not giving it enough time to fully understand what a fully healed eye will see like . Only did my left by the way. I am not too concerned about the sand in my eye feeling more than the feeling of seeing some glare off to the side . I guess right now I would characterize my quality of vision as good but not great . I am hoping that some of these side effects type issues will settle down. Yes , they did re measure my eye day of surgery and I did opt for the upgraded laser procedure that measures and dissects lens prior to surgery. This is at an a additional cost in Canada anyway

@@stevec8836 Similarly my operation was on 19th October. Only in my left eye (wasn't comfortable doing both). I also have night halos/starburst (hence stopped driving at night) plus the eye swelling similar to sand in the eye. Unfortunaetly I have to wear reading glasses for montior work though (brain hasn't neuro adapted). Trying my best to ignore the halos/starburst (difficult), as per @thegoodgriefconversation. So keen to keep a tab and share progress if you don't mind?

if you are talking about monofocals, JnJ has a better platform, and I suggest Eyhance if available. if there are specific visual needs and you are ready for premium surgery, it is advised to have a detailed discussion on lens options.

I have no idea about real world experience in English.. However I have a lot of feedback on my another channel in Russian, where I have about 18k subs, and tons of comments. I could say, that patient experience is really good. But there is no reason to compare Eyhance to P{anoptix, as there is different IOL types, and different performance in terms of VA and quality of vision

@@athulgeorge3978 With mono-focal you will loose the close up vision. You will need reading glasses then. Because the natural accommodation for seeing close will be lost with a mono-focal. With multifocals you will be able to read up close and also see well in the distance. However at night time the contrast will be lower. So some light is needed to read op close and at distance. If the containers are in an area where there are light-posts or lamps (so not just in the natural dark) you will be able to read them fine with trifocals. I have trifocals and can read the small streetnames from about 50 meters at night when there is street-lighting. With mono focals you can still read container numbers in dim light conditions like some one with normal eye's. But reading up close will require glasses. Does not matter if you could read fine before the operation. Spectacle independence requires something like trifocals.

Hi! If you need the best far vision, especially at night, the best IOL is Tecnis 1-piece. It is the best IOL in terms of far vision and contrast at dim light. However, as with any monofocal, your intermediate vision will be reduced. So, next option I suggest is Tecnis Eyhance, which will give slightly less quality for far (however still better than similar lenses), and will add better intermediate.

Thanks for the Video. Can you advise based on your experiences with patients getting one eye with lens surgery, can this approach work? The reason I ask I have only got one eye done as I wasn't comfortable doing both. I have issues with night halos/starburst (if I had done both eyes my life would have been hell). My vision is blurry/halo when reading text on the mobile and monitor, hence I gone back to my reading/monior glasses. Note it's only been about 4 weeks since the operation (Alcon Panoptix). Any tips, exercises to speed up the neuro adaption?

@@iol-adviser Thank u for your replied, btw before I did surgery I remember there’s one doctor told me my pupil in the darkness test is little bit bigger than other people, it’s 7.2 but I’m not sure is that the same data as your country or not

I have panoptix implanted in my both eyes for 3 months. All symptoms appeared to improve very slowly. The most annoying issue is my near vision. The glare, starburst affect my work very much. The neuroadaptataion is far below my expectation before the surgery. Should Inexchange back to monofocal IOL

have you checked for residual refraction? are glasses may improve the near vision? It may be a key find the reason of the problems..

@@iol-adviser I have checked it and became 175 degree presbyopia and 50 degree long sightedness in both eyes. Glasses improved a little bit but the glare and starburst never go away.

@@wingshingkwong4211 degree is wrong measure of residual refraction. if glasses helps (far vision) you defenitely have refractive error, next reason might be dry eye issue.. any way I wish you to adapt soon

nobody knows, it depends on your qualify of life for now. If you will exchange a lens it is risks of secondary intervention and you will need glasses for reading/computer or far, depending on your preferences (if you will go with monofocal)

Hullo your explanation s very helpful - I had cataract surgery 5 years ago and still have glare, halos star effects during night driving - I am very unhappy- I have seen the original surgeon and two others and have been given drops for dry eyes or Brimonifine drops - nothing changes - I have a toric lens in one eye and I'm not sure of the other eye but it is a touch clearer for distance - is it safe to have the lens changed to a monofocal close up lens? Or are there glasses that can prevent this night blindness from too much light?

I had a Synergy lens implanted 5 days ago and my vision improved each day since except for today. Today my vision is worse...almost back to before surgery. Is that normal?

@@iol-adviser yes thank you...I saw him 4 days ago and he said my eyesight should return in about a week. my sight has improved very slightly in the past 4 days, so I am hoping it will be fully restored in a few more days.

Alex, thanks for your illuminating video. I have cataracts on both of my eyes.Left was severer than right. I had eye surgery 7 days ago on my left eye.. Panoptix toric placed on my left eye. After 7 days near, mid and far vision is nearly perfect. BUt I see the halo effect at night especially car beams and disturbing..My right eye is still untouched because I am unable to decide. Surgeon advised me to use Vivity Toric on my right eye so that it balances the vision. But Vivity has 1.25 near glass usage. I don't wanna use glasses. So I extensively use near and mid range reading and working. Right now although only the left eye lens changed to Panoptix and right is untouched (still has a cataract) I see halos. If I choose vivity for the right eye how would halos disappear ? ( because the left eye Panoptix lens still sees Halos) . As a shortcut just choose the same lens of Panoptix on both my eyes. Pls advice. ( I would be grateful if other patients contribute with their experiences)

I am waiting for Synergy to be FDA approved and for more published results on this IOL. One of the biggest factors I worry about is dysphotopsias. I don't want an IOL where the dystphotopsias would be worse than my current MF IOL (left eye). Watching your simulations where the Synergy dysphotopsias is almost non-existant and less than the Symfony and PanOptics is Frankly hard to believe, expecially considering the Synergy having the best defocus curve I have seen. From what I have read the Synergy is similiar to the Symfony but add a close add to obtain that 33cm close vision. I just don't see how the dysphotopsias for this IOL could be this small. If that is the case why would your recommend or anyone get another premium IOL? I really which you would do an in depth video on just the Synergy. I am talking about a detailed video showing actual patients and their vision results at all ranges from close to far. And the dysphotpsias they are seeing.

Hi! One of the reasons of less Synergy dysphotopsia it OptiBlue filter, which cuts exactly at one wavelength (unlike Panoptix yellow, which cuts multiple wavelength and have significant color changes). The patient testimonial will give no idea at all, to be honest with you. As it is a) absolutely subjective and b) is unpredictable from patient to patient in terms of appearance, intensity and subjective perception. If I would sell any IOL of mentioned, I would collect 1-2 patients who will say "it is perfect" and 1-2 patients with competitive IOL with significant dysphotopsia and it is done :) The simulation I showed, is just an idea of what you may expect if anything will go in let's say not the best way for you. PD is multifacrotial issue, and does not have clear evidence. You may see a monofocal patient with dysphotopsia, you may find a patient with Panoptix with no complains etc.. As an example you may find an Alcon Vivity patient feedback with severe PD (I found already some feedback).. So, regarding detailed review - I am planning to do Panoptix, Vivity, Symfony, Synergy, Eyhance IOL reviews. And the best way to minimise PD risks are: - check for dry eye syndrome - calculate IOL power carefully - place IOL well centered and with no post-refractive impact - check and remove secondary cataract once (if) it will come

@@iol-adviser Thanks for replying :) . I'm trying to compare the premium lenses. I am an IT worker so I spend a lot of time looking at monitors, but I also like to do outdoor activities like hunting and fishing where the best times are typically during low light. And, I've heard the PanOptix has difficulty distinguishing grey on grey contrast in low light conditions, and it also can have double or shadowed contrast on letters and other images with monitor viewing. I'm in the US, and from what I've heard the Synergy and (I think) the FineVision Triumph will likely be approved by the end of 2021. However, there is zero information comparing and contrasting competitors to the PanOptix lens, at least not in English. With all that said, I'd be most interested in the Synergy IOL to start with. Hi! You are right, Panoptix, as well as other multifocal IOLs has reduced contrast at dim light conditions - it is design limitations. Synergy has more that 2 times better contrast vs Panoptix, however Symfony is the best presbyopia correcting IOL among others in terms of contrast and night vision, but you pay with less near visual acuity. I will publish a detailed review soon.

Great advice regarding neural adaptation to help diminish/remove halo, starburst and glare issues. I am day 5 post IOL surgery with multifocals and there are halos everwhere at night. Huge multiple ring halos. About 20-30 rings around each street light source. Car headlights have smaller halos. Some lights have starburst. In a way it is fascinating to watch from my balcony. But walking on the street it was disturbing me a bit. I will take Alex's advice to try to stop thinking about it so the brain decides I don't need to see these visual disturbances, as it tries to adapt to working out how to decipher the light signals in my artificial lenses. Amazing actually.

@@tonyEEagle hello. Night-time visual disturbances still exist, but I am less disturbed about them. There may be a small decrease in the intensity of halos, glare and starburst. I noticed that the lumen (strength of light source) has a big impact on halos and glare. I have made videos about my experience on my channel too.
