When it comes to fighting hair loss, we're all looking for a low-cost, low-risk, high-reward treatment. At first glance, Nizoral® shampoo (ketoconazole) appears to fit this option...
But the truth is that Nizoral® works way better for some than for others. And fascinatingly, there seems to be a set of scalp conditions that correlate with hair loss... and if you have these conditions, you can often tell if you'll be a great (or terrible) responder to the shampoo.
This video dives into the science! Here are the time stamps:
0:00 - What is Nizoral?
1:37 - Microbes: how they hurt our hair
4:06 - How does Nizoral work?
5:30 - Clinical studies
7:20 - Nizoral: the bottom line?
7:54 - Takeaway #1: use a 2% formulation
8:32 - Takeaway #2: apply as directed
8:55 - Takeaway #3: don't quit too early
9:18 - How to know if you're a good candidate
10:05 - Before-after photos: Rabih
10:59 - What if Nizoral makes things worse?

The time I faced hair fall, I followed all these to effectively control it: Incorporate hot oil scalp massages into your routine. Do not use hot water when rinsing your hair. Protect your hair from too much sweat. Use a wide tooth neem wood comb to detangle wet hair. Decrease your sugar and unhealthy food consumption. Use natural anti hair fall shampoo and hair oil like Littleeextra coccconion. Maintain a positive mindset for general well being.

Good video, I came up to the same limited studies upon my search. Also the same conclusion as its cheap and falls under the why not category, plus even if you don´t have excessive or visual signs that you need it, in some studies the related the MPB to some micros. The question tho remains, how to use it ! The frequency suggested on the bottle is for treatment of fungus and other infections as mentioned. It also says it should be used at that frequency in the beginning and for a limited time only. (the frequency should drop to once a week after the treatment if my memory serves) Thus if you have any input on how often to use it, that would be really helpful! .

Reason being possible hair loss as a side affect.😮 I realize now that hair loss is the beginning of entering the growth phase of using hair growth products. I am a 61 year old woman. I have received great results from using 50mg of Spironolactone along with 6mg (1/2 tablet) of oral minoxidil once a day. I did not want to lose the growth required during this last year. Hey Rob, your videos just keep getting better and better! The production quality on this one is outstanding. I'm glad that you are finally on RU-vid. I've been compounding my own ketoconazole 2% shampoo with certain permeation enhancers for a while now, and for me personally, it works great. I use it daily and leave it on for 7mins. Nizoral has a plethora of sub-optimal surfactnats, stabilizers and preservatives and the dryness people get from it is not due to the ketoconazole itself but the surfactants. Also, ketoconazole is a very strong androgen antagonist and works in a dose-dependent manner. Just a 400mg oral dose administered over the span of a few days, drops both T and Free T considerably. You started alluding to ketoconazole's anti-androgen properties but you said that's gonna be another video. I personally believe ketoconazole doesn't go systemic when applied topically in a shampoo format because of its larger MW, 531 Da. That coupled with its anti-fungal and anti-androgen properties, make it a good, sustainable treatment. The only issue might be its serious hepatotoxicity if it enters the serum. Luckily, by applying it topically we skip 1st pass liver metabolism, so in theory, it shouldn't be so problematic even if it leaks into the serum of some individuals. Just my 2 cents. Thanks!

Thanks for the kind words! And thank you for your perspective - it's great that you're able to compound your own 2% ketoconazole. Are you doing this through a compounding pharmacy? And do you find that your formulation doesn't dry out the scalp as much?
@skillzset1066
@skillzset1066 3 года назад
@@PerfectHairHealth No, I don't use a compounding pharmacy at all. I compound everything myself - shampoos, conditioners and my topical serums. Ketoconazole is a BCS Class II drug and is very difficult to solubilise at the 2% efficacious dose whilst still maintaining a relatively non-irritating surfactant profile. That's why you don't see many alternatives to Nizoral. And the few 2% alternatives that are out there also use SLS, DMES, etc. My version is definitely less irritating but the biggest key is to use a good conditioner right after the keto shampoo. I've used 2% Nizoral in the past every single day without any problems because I compound very good conditioners that negate to a large extent the oil-stripping effects of the harsh nizoral surfactants.

@PerfectHairHealth
@PerfectHairHealth 3 года назад
It's a great question. As long as the headset is adjusted regularly and not pressing too hard against the scalp, I can't imagine this being too problematic. However, you will see pressure alopecia occur in people who wear high socks (like you mentioned) or in people who wear glasses constantly (along the sides of the scalp). So if you're gaming hard, it is technically possible.

I've been using Nizoral 1% for about 10 years now as directed. In order to make sure the shampoo is soaking in my scalp for 10 minutes, I apply the Nizoral outside the shower on my head with a little water and massage it in. I can't say whether Nizoral has resulted in hair regrowth alone but I can say it has helped keeping what I currently have and thickens up my hair for more volume. I've never tried the 2% as I just don't want to go through the hassle of continually getting a prescription. But this needs to be paired with the 5% minoxidil foam. The foam IMO works the best and is most effective. It does cost a lot more than the non-foam version but I find the best deal to be at Costco. I feel like this has been the only solution that works for me for the last 10 plus years but I am aware once I stop both, I may have accelerated hair loss.

In my country you can get 4% KTZ hair washing cream. No prescription needed. DO you think more KTZ, like 4% is better? Is there any study for more %. Thanks

Excellent advice. Unfortunately, I overdid the treatment, using 5% Nizoral instead of 2% and using it daily instead 1 to 4 times per week. After a couple months of this regimen I suffered significant hair LOSS not gain.. I discontinued the Nizoral and slowly recovered.

I'm surprised they even sell 5%. I guess that's for people with severe skin infections.

I have the typical symptoms you mention: seborrheic dermatitis, inflammation, burning sensation of the scalp if I don't shampoo for even 1 day and accompanied by alopecia. I've been using zinc pyrithione shampoos with no improvement. Using a hard water filter for my shower did help in reducing hair loss. I'm yet to try ketoconazole shampoos basically because of side effect concerns. Does it increase hypertension and reduce male hormones?

Hey dude, trying to help you a little bit. First thank you for the hard water filter idea, I think I‘ll gonna try this, because I have similiar symptoms. So yes, for me the keto shampoo helped a lot, if you first got it you have to take it the first 5 days (everyday) afterwards you go with 2-3 per week and you are good. Maybe it will dry out your hair a little bit, but you can use conditioner or something. But it will cure your flare ups and your scalp will be less itchy. And dude, I shaved my head and if your head shape isn‘t that bad, I will recommend it to you, the inflammation is basically gone, I think I noticed a little regrowth on my temples since I shaved my had to 1cm but can say that much, because it‘s only 4 months ago. Also thinking about to take topical finasterid, but not sure right now if the risk is worth it, but the risk is lower as with oral finasterid. And by the way, I had no sides with keto, only if you use it to often your scalp can get to dry and itchy, but normally it‘s not happening. And unfortunately I stopped using keto shampoo in the summer, because apple cider vinegar helped me a lot, didn‘t take it for 4 months and boy wow my hair loss got so worse, like I‘ve lost so much hair wow, I regret that I stopped taking it without any good reason, that‘s why I now have to consider taking something else, so dumb, such a shame.

I would try to follow the usage frequencies presented in the studies (2-4 times weekly for the first couple of months, with reductions thereafter).

I am jealous of your hair man , its so dense. Can i ask , by saying to use it 1 to 4 times per week , you mean that those are the only times you can wash your hair or wash the other days with an ither shampoo? My hair is too oily and i wash it daily.
@lolidangelmeier
@lolidangelmeier 3 месяца назад
i am jealous of you brother, i'd wish my hair would oil up faster but after taking nizoral 2% it really dries it out, not extremely but i still notice. It does help against the fungus and my dandruff stopped so thats that at least. I use Nizoral once a week and a hydration shampoo also once a week to rehydrate my hair and scalp, i think 2 times a week is a bit extreme but it also depends on how oily your hair gets. In your case i'd say twice a week.
@MarkWatch87

i dont waste Nizoral durring the hair wash but waterdown few grams in water and use it as topical for 30 min and the i wash with some other shampoo . This way i save money from every drop of nizoral and get the maximum of it in terms of time on the scalp.
@Bman67
@Bman67 2 года назад
I just bought two bottles of Nizoral a week ago but, I had no idea about the 1% and 2% variances and of course I bought the 1%. But at least I'll save money in the future because if the 1% doesn't work I won't be paying $15 a bottle for dandruff shampoo.

If it potentially reduces DHT, does it do it through systemic suppression or just at the hair follicle? Great vid
@PerfectHairHealth
@PerfectHairHealth 2 года назад
Thanks! While there aren’t any hard data, it is believed the shampoo only reduces DHT levels within the scalp skin itself. If any of it absorbs systemically, it metabolizes into byproducts that do not have an effect on circulating hormones.

hey man, about the hair loss: I was wondering if you ever heard of 2 things, 1 pilos anti- schilfershampoo it contains 2% ketoconazole and 1% piroctone olamine (PTO) my second question: (2) Did you ever hear of PTO Apprently it has the same function as ketaconazole it blocks dht. Do you think the Pilos shampo is okay because the main function is flakes and I don't even have them. what I do have is seberotic eczema that also causes hair loss ( but no coves ) and a lot of itching but that has now stopped because of that shampoo

Just take taurine as a supplement. Taking things systemically will probably always have a better effect than topicals. Don’t take more than 1000mg a day though and consult your doctor before starting
