Hi I have done my osce exam and had a patient with pneumonia. I had 3 in 1 parameter so I documented as 1 hour monitoring. My total news score was 4 where I had 3 in respiration of 26breaths / min and SPO2 of 94% but I failed only in the assessment station. I dont know why? Do you think i have to document like every 15mins and then escalate according to news 2 for score 3? I have done my 2nd resit also still failed in the assessment station really confused now
By now u probably have gotten your feedback and just curious if u mind sharing it what it was. If u have done most things u were supposed to and correctly charted the news score, could it be perhaps that patient was visibly having any symptoms of resp. difficulty If so, needed to mention "assessor at this point in real life scenario i would pause the assessment and focus to prioritize and stabilize patients breathing with a thorough investigation. I would be using methods such as sit up, raise the head of bed, deep breathing, administering O2. Throwing my wild guess out there. Hope to hear bk from u.
