The global sanctions on trading (including tourism) prevents much cash ending up in the hands of "the people" i.e. government and Kim himself. Something has to pay for those high-end brands after all! They get most of their money from Russia and China, but that comes either from the "wages" of workers sent abroad to friendly/neutral countries. Eg there's a government run chain of NK restaurants where the waitresses are sent without consent to serve and later on, entertain with a little band of traditional NK instruments and players.
