Question 1 and 2 Contradict seem to contradict each other. If the question says "what"action" should the nurse take" That usually indicates implementing something, not assessing. So in question 1, the "action" was loosen the restraint. I mean we skipped right by Assessing the situation! - but the question asked for an "action". So that seems.okay.. But in question 2 the phrase uses the word "action" and the answer is to re-assess?
Question 1 includes already assessing the patient in the question. Whereas Question 2 is about what to do prior to doing something with the patient. In Question 1 you already assessed and found the fingers to be blue. So the question is asking what you should do (implement) to mitigate what's happening to the patient. Question 2 is about you leaving a patient in restraints and the family took them off. Some time passed, so what should you do now that the family left. You always want to assess first then implement/do something. It may be confusing but that's how I formed my answer/rational to the questions.
To apply the strategies start off with APIE after reading the question then use the ABCD’s. You should have an answer after applying these. Also remember the answer is sometimes given in part of the question. So when you choose an answer ask yourself if it is answering the question being asked.
