But she doesn’t spill it on accident. She snapped because the moment she saw Shoto’s fire side, she saw Endeavor and threw the tea on purpose. She only realized afterwards that she was wrong and tried to heal him with her ice quirk and only caused Shoto more pain. That’s how he got his scar.
