This is merely speculation, but maybe one of the secondary powers that Iva's Devil Fruit provides is the ability to detect gender dysphoria in other people, or be able to sense when a person is otherwise unhappy with their body. Then it makes sense why Iva's so confident in making the decision to change them.

It's pretty clear that the Newkamas from the Kamabakka Kingdom could have a fully changed female body if they actually wanted it. But it seems that they prefer being drag queens and looking like their natural selves, just like Ivankov himself.
You realize this makes the guard gay either way; 1.) If he wanted to be a woman and liked men.... he is gay until he was actually transformed via Ivan. 2.) If he wanted to be a woman but still liked women.... she is gay...... So either way we got ousted....

Ivankov's ability isn't a secret. I'm pretty sure a prison guard working for the World Government would already know he can do that beforehand, which means he wouldn't be afraid of him or refuse to tell him where Ace is kept in the first place if he actually wanted to become a woman rather than Ivankov making her like it and want to serve him.
Second Law of Gender Bending: In fiction, any character whose gender is changed - via any various methods available in the setting, often scientific or supernatural - will prefer the new gender to his/her original one, and adjust to it remarkably fast. (Although I'd say THIS is pushing it a little...)
{I can confirm that, I've seen enough of it trough several media[including wandering to the "weird side of YT"(which has many bad ones)], and yes, this really did pushed it a little} However, there have been cases where the character doesn't like the change, which can lead to eighter of the situations: *1.* Character gets stuck on that form and has to learn how to live with their new body(probably the saddest outcome tbh) *2.* Character gets stuck on that form and looks for a way to turn back to normal(succeeds, but might learn to adjust to their new body as well in the meantime) *3.* Character periodicly changes between both genders, panics first time it happens, upon realizing the change isn't permanent stops worrying as much, mainly tries to live as their original gender but eventually might get used to living as the other one as well(character might accept life as both, but still can't live as just one) *4.* Character obtains the ability to change its gender at will(might pass trough one of the previous two situations before or gains it initially), might panic at first until realizing they can change back; having that ability, the character can freely experience and have a perspective on how's it like to be of the oposite gender without having to fully comit to permanently being it, might find uses for that ability, such as esentially having a second identity. Character mainly continues normal life as their original gender, changing to the other at times if they want to; could grow to prefer living more as the other one, changing the its original one at times(might decide at some point to continue living as their original gender again); or, having access to both, the character might just stop caring about their gender, probably without fully living as just one of them, changing between them as so they decide (The most interesting one of them all, considering how complex it can get if you think of it) These situations(including the original coment, though not as commonly) can actually also apply to characters that are transformed into another being, not necesairly gender changed(but could be included if you wanted to), that being animals, mostly being antropomorfic human-animal hybrids(yes, call me a Furry) in a fictional world where they can or can't be a common thing, or any species that could exist in a fiction world with human-like inteligence, but can vastly differ from human apearance(otherwise a character being transformed to that wouldn't be a big deal(unless they include gender bending ofc)) Basicly just change the word(s) "(oposite) gender" in all these situations(incluiding original coment) for "(another/other) species", which can get preety interesting, as there can be more than just two forms a character can take, not to mention if whatever the character transforms into has abilities and weakneses that humans don't have, there would be more to get used to than just gender bending(unless it's a similar situation as the original coment where the character inmediately adjusts to it), and giving more reasons to the character to want to stay like that/to return to normal, and more uses to the shapeshifting abiltity in case 4, especially situation 4 gets even more complex than it already was: a character ranging between the ability to swap between its original form to another one to having the ability to transform into multiple species(that could probably include gender bending as well) at will could probably add an infinity of of layers to a story(maybe too much); but at the same time, the character will very likely just mainly continue their life as their original form, changing to others from time to time ... how... how the heck did I pass from (some sort of) a footnote about "The Second Law of Gender Bending"(not the most common thing to talk about) to all of this? even I am impressed. I just went on about transformation concepts (mostly genderbend because of the topic) revolving around if the character doesn't like the outcome, unlike cases where they do, like in this video, yeah, it could've been shorter, I'll give you that, but it's funny because unlike what you think, I was actually questioning myself maybe around that time because of the whole transformation and genderbend rabbithole, even identifying as non-binary and genderfluid for a time. Sometime after, a lot of self reflection and research did lead me to realize I'm actually male and thus, cisgender, but that time period did give me some insight and perspective on some things. I didn't claim to be an expert on gender at the time, nor do I claim to be now, I just posted my albeit long take on something that interests me, even then, I still don't see what's exactly wrong with it outside its lenght, unless you just tought "there's gender, he's probably cis, he mustn't know what he's talking about" which is not only kinda rude, is again, kindof funny considering at the time I tought I might've not been. One doesn't have to be trans to know about gender, and while I know I'll never fully understand some stuff, I do believe that regardless, you don't have to make everything about gender. Being cis doesn't automaticly make you unable to learn about gender, just as being trans doesn't oblige you to make it your whole life, I believe I would've known a thing or two before, and after questioning I do have a little bit more of insight, but I still don't claim to be a know-it-all on the topic, nor was I talking specificly about it. Given that most devil fruit users can just shrug off physicals attacks and don't care how dangerous the fight they pick are, being able to blackmail all non-Haki users could genuinely be one of the most powerfull power in all One Piece.
