True, that was really very mean of her and I really felt sorry for Rumpel in that scene. He was reluctant to accept the deal, perhaps even willing to die for a moment. But then he thought about his plan to return to Storybrook and surely there would be a chance to take revenge on Zelena. That's how I think...

quite physically too.his heart rate was sky high at least the first half of the vid while she ignore all his physical discomfort and keep touching him.that breathing tube was shoved down his windpipe so when she was playing with it in 00:29,she's literally choking him and moving the pipe inside the walls of his throat.whatever she did in 1:04 too.

Wouldn't it be funny if there was someone standing outside the room hearing everything. What would he think?

No. Maybe on some units but usually if a patient is a harm to themselves or others they would have a constant observer. If the hospital needs to watch them on a monitor in the room, the patient has to be conscious enough to sign a form to allow it. And that typically is to watch for changes in their condition. But they rarely do that. In this case, since rumple is intubated the nurse would have to check on them probably more often then every hour and usually intubated patients are in the icu and the entrance to the rooms are glass and the patient rooms are surrounding the nurses station. So the nurses can keep an eye on their patients from the nurses station. Though they do have curtains so the patient can have a little privacy. But they’re are a lot people taking care of one patient, not just the nurse. So rumple would have someone coming into his room almost every half hour to an hour. Sometimes more often than that. But there are cameras in the halls of the units.

Galaxy Eyes she gives him the elixer so he can leave the hospital and get her happy ending. She knows that rumple doesn't break deals so she's confident that she will get what she wants

Rumpel had a heart attack in Baelfire's apartment and Hood took him to the hospital. Without magic, he can't protect his heart (which is getting blacker and darker) and is forced to accept Zelena's deal to get the cure from her.
