As someone who struggled with moderate depression for years without getting adequate help, this is a great video! I luckily got professional help at the beginning of this year, first treating a vitamin d deficiency the blood test at my doctors showed, then getting an emergency referral (not in the er way, but it’s like a sped up process in my country where in serious cases you get assigned a spot at the nearest available doctor (both spatially and time wise) There i was super lucky that combined with the therapy i was going through, a slow starting course of antidepressants and now significant positive lifestyle changes i’m in a much better place now. Sending love to everyone else who might be struggling out there! 💚

I just want to say that antidepressants have completely changed my life for the better! I avoided trying them for so many years because of how much scepticism there is out there (and people touting the benefits of exercise and therapy), but after five years of therapy and still struggling to function, my friend dragged me to the doctor to get prescribed some ssris. Within months of starting the medication, I went from being a shy and awkward student paralysed by depression, anxiety and low self esteem to finding a full-time job, a big group of friends, a relationship, and having the courage and energy to do the things I'd always dreamed about but talked myself out of, like solo travelling and playing in a jazz band. SSRIs have also all but cured my insomnia. I'm still a little more anxious and negative than the average person, but it doesn't get in the way of doing the things I want to do. And now I'm in the right headspace to actually go out and exercise and do the things my therapist recommends instead of talking myself out of it. I expect to have to be on ssris indefinitely, and that's OK.

I want to hopefully be able to encourage others who are struggling or who have also struggled for many years like I have, it can get better and you can find a new lease of life like I did. Couldn’t sleep, lost way too much weight as I couldn’t make myself eat, and had no interest in anything. For me CBT was huge. I’ve been on meds for 13 years (half of my life). I have depression with bipolar spectrum, used to have anxiety, social phobia, currently struggle with compulsive eating. I’ve been in therapy for 10 years and I’ve made so much progress. I’m lucky enough to have amazing support from my parents. Even though I’ve had 3 suicidal attempts (last one when I was 16), slept 14h per day and getting no rest and many more, I’m a walking proof you can live with that. My mom and has been battaling depression for literally years now and honestly, it pains my heart to see her struggling in her time of need. Right now, I just want to say thank you to you Siobhan for uploading this video as now I feel like I have a better understanding of depression and how we as the human race can treat it. But to be honest I really don't know what to say here because i feel like while I defiantly know more about depression than when I started watching this video but now I honestly don't like know if I would be going through the same thing as my mom. Like I feel like I have some of the symptoms of depression but yet at the same time I'm also doing a lot of the treatments talked about in this video. Like I have a healthy diet, I do exercise and I have a vast friend network and furthermore I am working on catching myself saying certain things and being like "wait a second, is this actually accurate?" it's something i have been struggling with for a long time and I feel like it has something to do with how I was raised in my family. But idk for sure. what I do know i think is that depression is a complicated topic and I think this topic defiantly warrents discussion with my therapist when I see him next because this is serious and I really don't want to end up in a state like my mom when i would not be able to help her out in her struggle with depression. Anyway Siobhan, Thank you very much for uploading this video and keep up the absolitly amazing work!!!!! Exercise is still a tough one (I’ve had sciatica for 2 years) but my current job is very physically demanding and somewhat fills that gap. It’s a very slow climb up from depression, no sugarcoating that, but While I was healing I tried to just focus on the next step forwards. Whether that was therapy or medication or a bedtime routine or strengthening my support system or just getting out of bed. Just the next step. That’s all you gotta focus on 💚💚💚

I have struggled with depression for as long as I can remember, I was very young when I was diagnosed and I’m in my early 40s now. I didn’t try med for a very long time, I exercised and tried to eat healthy but that turned into disordered eating and exercising too much, especially with how little I was eating. I did eventually try SSRIs and SNRIs and Wellbutrin and have gone to therapy off and on. SSRIs and SNRIs, for some reason, give me serotonin syndrome which is really scary. Wellbutrin makes me super glittery so it wasn’t a great option either. Therapy with CBT, EMDR among other things has been helpful but not enough on it’s own. I have had to cut way back on my exercise in recent years because of my physical struggles and having 4 spine surgeries and coming up on my 5th. That has all contributed to my worsening depression. I started a medication call Spravato (esketamine) which has been so incredibly helpful along with therapy (including CBT and EMDR), and the exercise I can get. It’s a medication used for severe and treatment resistant depression with or with out suicidal ideation. It has helped me so much. It is a medication that in the US I know it has to be taken in a doctor’s office and you have to stay at the office two hours after taking it. It’s a nasal spray with two metered doses in each device. I use three devices each time. Some people do well with only 2. It’s taken 2 times a week for the first 4 weeks and then moves down to one time a week and then they try to spread it out even more but therapy with things like CBT and EMDR helps it to be so. Much more effective, or really Spravato helps that kind of therapy work even better in the long run. I also have ketamine lozenges I can use at home a couple of times a week. So the esketsmine in office and the ketamine lozenges at home have been so incredibly helpful to me along with therapy and exercise as much as I can. Spravato was approved for use in the US about 3 years ago I think, clinical studies being done before that in patients were showing a lot of success. Ketamine has been used off label to treat depression for a really Ming time though. IM injections or IV ketamine has been used a lot time to treat depression and chronic pain. It is not FDA approved though and the IV or IM ketamine therapy can be very costly and is almost never covered by insurance here. I am grateful for Spravato which is covered by my insurance. It has saved my life. The troches (pronounced tro-keys, kind of waxy lozenges) which are taken at home are $40 dollars a month’s supply at the compounding pharmacy where I get them made. I take two 10mg troches twice a week and have one Spravato session a week. This has been so wonderful for me as traditional meds either don’t work or actually make things worse for me. I have multiple mental health, and physical diagnoses. I wasn’t sure if there was anything that was ever going to help me. But Spravato and Ketamine under a physician’s care and while working with a therapist, have been so helpful with everything. My diagnoses: PTSD, Generalized Anxiety, treatment resistant depression, ADHD, Anorexia, PCOS, severe degenerative disc disease, chronic migraines, POTS, EDS, osteoporosis and rheumatoid arthritis and I’m sure I’m forgetting something but that is most of them.

Social situations could put me out of control and I was barely a step above a medical emergency from depression. I didn’t think I needed to go to a doctor. Something happened that cause a colleague to basically force me to call and make an appointment and by the time I was out of that appointment I realized it was way worse than I thought. Being in the depths of depression is literally the most awful thing I've ever been through and I wouldn't wish it on my worst enemy. Medication for 6 months with therapy it was so successful with me when I lost my 21 year old son in a car accident 17 years later I had the tools to survive and help my family with our nightmare. I totally agree therapy is absolutely necessary with medication never one or the other. It’s can be a game changer. Thanks for explaining the whole process because my doctor was a life changer because she knew exactly what I needed ❤️❤️❤️❤️

When I finally spoke to my doctor about my mental health a couple years ago it was SO life-changing. It took my mental health getting worse at the start of the pandemic for me to reach out, but I realized after finding the right medication and talking it through that I had been struggling for years, if not most of my life! If you're thinking about reaching about but don't want people to think you're overreacting just know that if you're thinking about how bad things are constantly and have been struggling for a long time...you're not overreacting and you deserve to get help!

Yes depression is so difficult and you have given us a wonderful blueprint to start and follow if we are suffering. And yes with depression, we tend to suffer in silence. I know I did b 4 I finally shook off my pride and asked for help and now I'm good, it took a while and I still have questionable days but I'm good now and I hope that people with this awful disease will see your vlog and reach out for help! Thank you so much for this video! I have suffered from major reoccurring depression with suicidal idealizations for decades. During COVID we realized my meds were not working at all anymore and my depression got so bad. My Dr. took immediate action and put me on a new med Viibryd and it was like no other med I have been on, I felt effects within 3 days when it usually takes over a month. I was able to start focusing on therapy CBT and EMDR along with other things because I had some positivity, energy and hope. I am very blessed to have a great Dr who is my best advocate. My depression still has break through moments but it’s been a whole lot better. 💚

Dr. Siobhan, Thank you for sharing this topic for the world to see. It is amazing to hear that 1 in 5 people suffer from depression. As someone who went through my own challenges - as well as those reading this - it is no easy process trying to cope with these emotions. Many times, we feel something is wrong with us. It takes much courage to seek help, watches these videos, and share knowledge on this subject. As an aspiring physician, thanks for sharing! Your videos keep me motivated to pursue medicine as a career. Let's do this, doctors! 🙏🏼

Thank you, unfortunately, I know all about it. CBT can really feel like: "Just stop thinking that, it's bad" which turns into "Stop feeling that." People with emotional trauma develop negative thinking as part of a complex survival strategy. It can be a way to hold someone responsible for an unhealthy family dynamic for example; if it's just you that is the unworthy freak, then at least you can justify having to live in the situation you're in. We might just try really hard to ignore it and move on with life but at some point, you have to dive deep into the underlying feelings. CBT is great for the person that never feels successful enough because their parents expected an A+ on every test, it's harmful for the person with a 747 full of cargo.

Theres a strange feeling of shame surrounding depression. I’ve had suicidal thoughts on and off for years but I’d never talk to someone about it. Going to the ED feels like a waste of healthcare resources and talking about what causes these thoughts would just be too embarrassing. Anyways thats where I’m at, thanks for coming to my TED Talk.

I'd come up with some coping Strategies as I do not like to talk about my depression or anxiety because people take it the wrong way or use it against me. I've been drawing my emotions. You make sense of the day that I am feeling emotional or physically drained I call them a pickle of A-day. As I used to beat myself up in my mind thinking why can't I snap out of this Or I struggle to find an answer why I'm feeling like this way right now. Saying a pickle of A-day it helps.

This is so important! I love your way of explaining and that you include medication without judgement! I have C-PTSD causing depressive symptoms and recurring severe depressive episodes. I tried 3 different antidepressants before finding one that worked - so don't give up if the first one doesn't work! In addition i want to say that one medication worked, but not enough. This is also a reason to talk to your doctor and try another medication. Also, while i had an effect after 4 weeks, the full effect came after two months - so be patient 💚 I want to say that if you are depressed exercise seems so hard and 3 times a week seems impossible, don't beat yourself up over not being able to do it even though you know it would (theoretically) help. When you are depressed it is so incredibly hard to change anything, maybe just getting out of bed is exhausting, so changing your eating and exercise habits can be incredibly hard. This is very normal and a sign you might need more help, maybe medication.

💚 Thank you for a nuanced video on how to treat depression and how there is no one size fits all answer, it's usually a combination of different strategies for the individual to have an impact over time. I think this is also why it is important if you don't have depression to be cognizant of your lifestyle and any lifestyle changes. Sometimes negative changes in lifestyle unchecked over time will compound and exacerbate or cause several mental health conditions that then may not be as easy/quick to treat once it reaches that point versus the build-up.

Great video. I have struggled with depression & anxiety on and off for 30 years. Living with a severe disability im sure is a factor, and the extra isolation due to COVID has definitely exacerbated my symptoms. I have been in and out of counseling and used to be heavily medicated. I have also had several different diagnoses as to what type of depression I actually have. I have found, for myself, one of the best ways to reduce my symptoms is lifestyle - diet & exercise especially. I try to eat healthy and exercise several times a week. I find I tend to binge more when depressed, especially high carb foods.

I have been in treatment for depression on and off for more than half of my life. I wish I had started medication much, much sooner. It didn't solve anything on its own, but it gave me the energy back to invest in taking care of myself. @Violin MD 💚💚💚💚💚💚 and more. Siobhan, thank you for providing a clear, concise and common sense approach to the treatment of depression, in simple terms for all of us to understand and from a doctor's approach. Many don't realise there can be a physical cause of depression and I am so glad you mentioned hormones, such a thyroid hormone levels. Also, I think female hormone levels can also affect one's moods. Your positivity in this video would have given so much hope to many people indeed. You are so right, the treatment for depression is not a one size fits all and it can involve much trial and error with a combination of treatments. I was so happy to hear the mention of non medicinal treatments before medication. You are so right, CBT can work, for people to unlearn certain behaviours and thoughts. I am a big believer in the power of exercise, the Mediterranean diet and providing nutrition via prebiotic/probiotic foods for one's gut microflora, as they do help with one's moods. As for medications, some people do expect a band aid effect as they think depression is due to a chemical balance, which can be easily and quickly fixed. In fact, I have read current research that the underlying mechanism of depression is not due to a chemical imbalance, as antidepressants normalise serotonin levels quickly a few hours after dosing, yet the antidepressant effect manifests 2 weeks later. Hence, there are certainly other complex brain processes involved in depression. Antidepressants do work indeed but still it is not known how they do so in the brain. Thank you again Siobhan💚💚💚💚💚💚

Great topic! I'm on 100mg Sertraline and 150mg Wellbutrin and while it has helped, it hasn't as much as I'd hoped it would. I'm in my 50's and never dealt with this before this year, it came on after having Covid which along with a bout of C-Diff also destroyed my gut bio which like you said, i believe that gut mess absolutely contributed to my depression. Also, in the US it's almost impossible to find a CBT therapist that takes insurance. Thankfully the meds give me enough will to go to work but thats all I do, go to work and come home and lay on the couch then go to bed. Thankfully I'm able to recognize suicidal thoughts as being fictional and just a result of the imbalance.
