I have severe pain EVERY DAY. I was put on pain medicine years ago and it helped so much. Then, last year all of a sudden the head doctor at my doctors office decided to cut me off. Then and there. I was always on time with refills, I never called for an early refill and passed every urine test. And I was cut off. Ever since I haven't lived. I just exist and go through the horrible pain. This is inhumane.

The only way a GP or any doc or health and social care worker will truly understand what chronic pain is like to deal with 24/7 is step inside the human body of the other person and be able to truly relate. It is a true test of your soul and being to do things each day that other people take for granted Movement , sitting , standing chores errands..all huge milestones when your in chronic pain xx

I went to a pain management clinic (after a knee surgeon botched my surgery) and both drs tried to humiliate me. One wrote in his notes that I had psychological problems and the other stated I was lying bc I didn't make eye contact with him. After 2 1/2 yrs seeking help (saw 7 knee surgeons) a knee surgeon stated that I might have a pinched nerve, and he was right. The bad thing was the long term chronic pain, left me dealing with mental issues and depression. All bc not one of the 6 surgeons would help me. I can relate to why ppl in endless pain, take their own lives.

Someone who can work, make dinner, clean up, shower daily, + go to a wedding does not have 9:10 chronic pain. Nine out of ten chronic pain is when you have painsomnia for successive days + nights in a row. You have to exhausted to sleep +, when you wake up, you're sobbing in excruciatingly unbearable pain. You can't move in bed, get in/out of bed, walk to the toilet, transfer on/off the toilet, shower for weeks on end, or even attend local diagnostic labs draws, x-rays, diagnostic testing such as an echocardiogram +/or a medication induced heart stress test bc unmanaged chronic pain causes heart failure. You no longer can attend the specialist appts you've waited up to 20 years to attend, or to return to see a 1st time local GP visit, make it back to see an established local asthma specialist after almost 5 years. You have to be rolled from bedroom to bathroom on an old follator walker even though the manufacturer clearly reports this is unsafe + dangerous, you need to be helped w/ stability + transfers; you can no longer be transported out of area 4 - 5 hours each way to receive needed specialist care unavailable locally.. EMS often has to "unpretzel" + move you onto the gurney going to the ER where you will most likely be verbally abused + physicallly tortured especially if you're a woman +/or POC due to medical ignorance +/or sadism, you beg your God - or the Universe - to remove your soul out of the body which is so broken it can no longer sustain you or other's souls as your significant other of 7 yrs tells you, "I'd blow my head off if I was in the pain you're in. Then the longer your pain goes unmanaged the worse it gets and the more sensitive to it you become. Chronic infections such as Ecoli outside the digestive tract, Staph Type B, MRSA occur as a result of discrimination, harassment, a failure to provide or refer you to a specialist - such as infection specialist. Your dumped off needed Palliative Care Patient medications bc most drs don't understand that this designation is an exemption to the "CDC Guidelines" plus permits + legally protects Drs who can legally prescribe the medications you were being provided. Almost all drs don't realize the Palliative Care Patient designation provides additional billing codes which Medicare + other insurance providers also must honor + permits drs to actually be equitably reimbursed for the add'l time it takes to treat a Chronic Pain Palliative Care Patient for the 1 in 5 adults who have Chronic Pain which won't get better +/or will continue to get worse. Also, as many drs also don't know or understand, the Palliative Care Patient designation doesn't have to mean you're projected to die within the next 6 months as many drs misconstrue just like they misunderstood then grossly + dangerously misapplied the CDC Guidelines which the CDC now admits were based on falsified data. Additionally, the FDA does not support the CDC Guidelines. For more information, please see Dr Thomas Kline's articles online. Best regards, C.

Sadly alot of people take pain medication for sedative effects and how it relaxes you turning it into psychological dependence so they'll lie about their pain to obtain more even if they don't need it the situation in this country about depression and anxiety etc etc rely too much on meds ssris benzodiazepines and pain medication all that just makes you numb to your problems that don't seem worth facing without reinforcement so now people with real pain can't get a decent supply of their medicines cause of these people

I had a bad injury 10 years ago. When I couldn't get out of bed my Dr. prescribed opioid pain medicine. I knew the risks and for the first year I kept a dosing log. I've never been put through the ringer. I openly discuss my concerns about dependence and my other methods to reduce pain and keep my activity up. I've managed my pain without increasing the dosage and I've never lost or had meds stolen. My prescription is refilled at each visit after a good discussion. Depression is a common comorbidity with pain and some patients try to dose themselves out of depression- it doesn't work.

The acting in this video is delightful. This was made right about the time the pendulum on pain treatment had swung afield to overcompensate for the so-called "war on opioids." I had an open bladder surgery a few years ago which was the most severe pain I have ever experieced (70 years old). The surgeon gave me a prescription for Tylenol because "opioids can be addictive." I'm not too sure how my suffering helped in this war.

I call bullshit .Even this clip is swaying the audience to believe the woman is guilty of medicine abuse..and who defines abuse when you are undermedicated to begin with. I guess we are all guilty for wanting to live a life without overwhelming pain. We are never pain free. For example -My rescue dose should be written every 4 to 6 hours according to the P.D.R. but it is written for every 8 hours, so if I have to take an extra one on a bad day an attack of acute pancreatitis .instead of daily chronic. Does that make me a monster ? I would rather die than go to the ER!! Last ER visit I was treated like absolute crap I pulled out my IVs and walked out. We are adults I am becoming so tired of watching people being abused along with myself. This has to stop for the chronically ill of this country.We are going to witness a nationwide epidemic of suicide or postal pain patients.

Kind of suspicious to me. Well that was a thorough review and evaluation of the pain meds. He asked what her pain was from one to ten. It was nine. Case closed. No review of the medication, side effects, type of pain, duration and onset of pain, consideration of adjunct medication such as NSAIDs, though she said it was abdominal pain, so maybe not necessary, no physical examination but plenty of questions about depression, that is probably caused by uncontrolled pain but hey, she is going to do yoga, so job well done.
@Dominothespotcat
@Dominothespotcat 9 лет назад
She has pancreatitis (which i've had before, actually brought on by NSAID's) pancreatitis pain has been described in medical literature as like getting end stage cancer pain in an instant... and pain so bad it can cause a cardiac arrest. NSAID's actually caused my pancreatitis, the doctor kept asking me if I drank, and I kept telling him I didn't, I don't even like the taste of alcohol, and once he got my labs he was shocked to find the NSAIDs had caused it. Because she has pancreatitis, the NSAIDs would make her condition worse/possibly cause a life threatening event.

this woman is not an addict... she is suffering from pseudo-addiction. Pseudo-addiction is when a person who has severely under treated pain, acts like an addict, takes risks trying to get medication to treat her pain... But if she had appropriate pain medication *standard of care for pancreatitis is long acting morphine* all her addict like symptoms would go away.
@Dominothespotcat
@Dominothespotcat 9 лет назад
The Tylenol/acetaminophen in hydrocodone (Vicodin, Lortab, Norco... they are all the same, hydrocodone mixed with tylenol) can and does aggravate pancreatitis... hence why the doc doesn't want to increase her dose. Recently, a medication called Zohydro ER has been released. Zohydro ER is the first medication on the US market that is just Hydrocodone, without any tylenol, and is made to release slowly over 12hrs. Zohydro ER or MS Contin (slow release morphine) would work best for this woman... because the pain of pancreatitis, is like pancreatic cancer pain (widely known as one of the worst pains in man kind) but not dying. When I had pancreatitis, the pain was so bad it shot my blood pressure through the roof, and sent me into cardiac arrest... I was put into a medical coma, for 48hrs, and then kept on an IV drip of morphine for four weeks to stabilize me.

This is actually a very compassionate and effective exchange between doctor and patient. Many people misunderstand addiction and drug dependence. It seems that a staggering number of pain patients (both acute and chronic) are paying the price. One danger that occurs to me is the disconnect patients may have because of personal bias. Nobody wants to hear their trusted physician use the word “addiction” when discussing their medication history. It can feel inappropriately judgmental and invalidating. A great deal of shame and moral failing is attached to that word. Education about the physiology of addiction should be required before opioids are prescribed. It is not enough to tell a patient that they need to be careful because the prescription opioid is addictive. I believe that most people think it “won’t happen” to them. People hear the word “addiction” and think of the worst anti-social drug user they know about; a person who they perceive as socially and morally inferior who has nothing in common with them. The reality is that a staggering number of people who become “addicts” began their journey with an injury or disease and well-intentioned (if sometimes misinformed) prescription by a doctor.

Why don’t you talk more about the horrible side effects antidepressants can cause? 🤭 Because we ( physicians ) are paranoid about opiates, so we are constantly eluding to accusing a patient of something, but we don’t actually want to say it, because we know the consequences. We are so damned concerned about mental health, yet we won’t bat an eye leaving a patient in severe pain for long periods of time. Because, after all, that’s better! 🤬

I now no longer go and I feel better off of those drugs. I weaned myself off very slowly and I am a new person. I had a tumor in my spine and after they took it out they had to wrap my spine in a metal cage all around my spine all the way up and then they screwed a metal plate into my back to attach it to the cage around my spine to keep it from collapsing and I've had nothing but chronic pain since most days it's a 9 and a 10 everyday. My doctor of 20 years had me on opioids and I did great with them no issues I had a life back then. Then my doctor retired and the first doctor I saw that I was sent to afterwards took away my pain meds I'm tried to give me antidepressant meds in their place which of course did not work. I have been in hell since I have no life I no longer see my grandchildren are my kids or my friend I live in complete isolation and I have nothing and no-one to live for anymore. I feel like I'm already in hell so I might as well in the pain since I have nothing else to lose all they left me with the pain and my whole life is about pain I live in pain 24 hours a day I get no sleep now my heart stops every three months almost its stop four times now. First time 19 days after he took away my meds I was in such bad pain and my heart stopped at night and I just got lucky enough to call the ambulance before I died I died on the way to the hospital and they got my heart going again and that's happened three times where they got my heart started again in the fourth time they couldn't get it started again so I was on life support for a few weeks. There are no doctors anymore around here who will prescribe pain meds I had to go 3 hours to Seattle then they started arresting the doctors in Seattle and now there are no doctors that have any want it all to prescribe anymore because they keep arresting all the damn doctors. In the meantime I think about getting a pistol and blowing my head off because the pain is so intense and I have no life anyway so why I live for pain and that's all I live with her staying 24 hours a day no sleep heart-stopping what's the point I should just end it all except I made a promise to my kids that I wouldn't because my mom did and they saw how much that destroyed me so now I'm stuck between hell and he'll have to live with the pain and I can't hurt myself because of a promise I made to my babies and I have no life nothing to live for anymore I can't go anywhere I can't get out I can't see anyone in this shape and I can't enjoy anything anymore. I'm lucky if I get a shower once a week because it's so painful to do anything and of course that makes it harder for me to involve myself in anyone or talk to anyone because I don't want them to see how bad I've gotten and how miserable I am and how bad I look and of course they wait game because I can't go anywhere do anything I've gained a bunch of weight and that makes it worse my self-esteem and my confidence is completely wiped out and all I live for is pain and more pain 24 hours a day and I can't find a doctor at all who is willing to help me. I never had any issues with my meds when I went to my doctor I always took the same amount always just the correct amount I took them for 15 years and never had an issue then the first thing that doctor did was tell a 65-year old woman me a foster parent a grandma a volunteer at the hospital a volunteer at the nursing home in a Bollinger at my church adopted two children and live for my babies and my grandchildren everyday I took them on an adventure and had a life and love life and I had a husband and now I'm getting divorced and I can't play with my babies anymore I don't see them who wants to come over and stare at some old person who can't do anything and that's all I can do that I can't play with him or anything so I blocked them and I lost my husband and I lost my family and I lost my life all because of a stupid Doctor Who treated me like a drug addict the first day before he even knew who I was. He said he hated the new 2016 ruling by CDC and I thought he hated it because he couldn't read his patients but no he corrected me and said no because now I have to start seeing all the drug addicts drug Seekers and liars and criminals insinuating that I agree, and a volunteer at my church and nursing home and a parent foster parents who adopted two children and I've been through every criminal background check you could ever imagine this doctor was implying that I was a drug seeker or drug addict or criminal or liar. When I left the appointment that day I thawed out in the parking lot for at least 30 to 40 minutes lot crying my eyes out because I had never been treated so cruelly and so badly and then so judge by someone in my life. You did not even know me and yet he crushed me I couldn't drive because I was crying so hard so I had to sit out there until I could stop crying and drive. I cried all the way home and I cried for a couple hours after I got home I am one bullet away from ending it all but I can't because the promise I made to my children so I have to live in this existence of nothing but pain 24 hours a day excruciating pain and I can't get any help but they are willing to dish out the psychiatric meds because they think the pain is in your head which is a bunch of nonsense and cruel if you ask me so yeah it's a joke when they try to give you a psychiatric meds for a metal spine and excruciating pain and a heart that no longer works because the pain is too much for in the stress is wearing it out so it keeps stopping. My daughter made me rip up my DNR that I had because of a situation with her grandma where to get and get to the hospital in time to see her and it destroyed her and broke her heart so she made me rip that up so I keep getting brought back to life even though I want them to leave me alone. This is how bad my life is imagine that the saddest day of my life was when I woke up from being on life support and realized I was alive I woke up and it felt like I was hit by a freight train coming at me and hitting me full force that's the first thing that happened to me when I woke up with the pain came at me like a freight train and then the realization that I was still alive and I started crying and I couldn't stop crying I cried for about 2 or 3 weeks because I was live till it was the happiest day when I died But the saddest day when I woke up alive so the happiest day of my life was when I died. How bad do your life have to be to be so sad about being alive? Bad does your life have to be to have the saddest day of your life to wake up and realize that you are alive. That is chronic pain

Now that they are crunching down on narcotics for pain management they seem to be focusing any snake oil they can suggest. Yoga is great but it is just stretching balance and breathing. It is not going to take away chronic pain. When you feel like you have no control in your life, it is something to give you a little bit of control back.

The missing piece of the puzzle is the way the drug is/was prescribed. For argument's sake, let's assume the half life ( the time from when you take a drug to when the amount in your blood is 1/2 that of when you took it) of Hydrocodone is 4 hours. The way it is prescribed is 1-2 every 4 hours "as needed". If you wake up hurting and take 2 pills, then 4 hours later you take another 2 pills, because of the half-life of the drug, it is the same as taking 3 pills initially. Four hours later, the patient takes another 2 pills when there is 1-1/2 in the blood. This is the same as taking 3-1/2 pills initially. This key is the unmentioned contributor to addiction. The correct Rx for Hydrocodone should be1-2 for the initial dose, then 1 and only 1 every four hours as "needed". This would drop the maximum daily dose from 8 to 6. I recently worked out a regimen to get off Hydrocodone with my doctor. Because I have degenerative disc disease, such the doctors wont operate, I know, like my mother, (thanks Mom) I will be back on it eventually. Getting off it for now will help insure it will be effective later when I needed it. I am afraid of regressing back to Hydrocodone as my mother's life was over when she started Oxycontin. The key with Hydrocodone is to get down from 80mg/day to 40mg/day and bite the bullet. Take a week off. By day 2, you will have diarrhea - use Imodium to control. In my case, I had severe restless leg syndrome - carbidopa / levodopra will control this. If you are a physician, the mathematically correct prescription would be: for Hydrocodone 10/325 1-2 as needed first thing then 1 every 3.6 hours as needed - maximum daily dose of 6 instead of the 8 currently prescribed. The misprescribing is not taking the drug half-life into account.
@graygonestray
@graygonestray 5 лет назад
Technically the dosing interval for any combo product with acetaminophen is based off the acetaminophen component, because >3-4gm/day can cause liver damage. Opioids can actually be dosed once every Cmax (time to their peak concentration and thus peak effect) However this isn't really seen outside of the hospital setting because it's extremely inconvenient for anyone to be taking a pill every 2 hours. And yes, the half life of oral instant release hydrocodone is 4 hours, but the half life is used to determine if the dosing regimen (if it's taken around the clock, instead of as needed) is adequate. The drug level reaches steady state in 3-5 half lives (so ~1 day for instant release hydrocodone tablet), so after the drug reaches this steady state you will have a better idea of whether the dose is adequate.
@tedphillips2501
@tedphillips2501 5 лет назад
@@graygonestray Thank you for your thoughtful reply. Who determines if the dose is adequate and under what conditions? A patient may need a higher dosage when working than on the weekends. With doctors afraid of the DEA, a lot of patients aren't getting the dosage they need, so they turn to other things - like alcohol or street drugs. In my mother's case, she was prescribed a steady dosage for the pain caused by calcium loss from her bones. A steady dosage over time results in the drug losing its' effectiveness. Once she "graduated" to Oxycontin, her life was over. That's why I wrote my comment. With the increasing number of single seniors coming down the road, there needs to be developed a different regimen that minimizes the drug level but maintains effectiveness over time. After I did the math, I became convinced the initial prescription is the most important one. Remember, the pain epidemic, caused by the conditions people work under and the insurance companies wanting to max their bottom line quarterly is what created and drives opioid use. With the legalization of marijuana, I fear a lot of DEA agents will be looking for low hanging fruit - the elderly with chronic pain and their doctors.
@graygonestray
@graygonestray 5 лет назад
@@tedphillips2501 That decision is between the patient and doctor. It's a difficult decision because the meds really do work so well, but as you've mentioned, tolerance and addiction do develop especially if the dose is in excess of the need for pain relief. Typically tolerance develops in about 2 weeks, and if there is a change in the pain experience it is because the pain is progressing or the patient is seeking the euphoric effect. When the dose of the opioid is titrated to the pain level, the euphoric effect does not occur which is why doctors are very hesitant to continuously increase opioid doses. Pain is a physical and emotional experience, which is why doctors recommend non-drug therapy such as yoga and meditation. A good way to think about it is this: imagine 1 day you wake up, you are looking sharp (perfect hair day, clear skin), you go to the grocery store and the person in front of you at the check out line does one of those "pay it forward" things where they pay for the person thats in line after them. Then you go to work and your boss tells you how well you've been doing really great in your job and gives you an envelope with a big bonus check. As you open the envelope, you get a paper cut. How much does this papercut hurt? Is it really bothering you? Ok, now compare that to a different day: you wake up exhausted, your skin is all dry and pealing and your hair looks weird. You go out to your car and you got a parking ticket (or say your bike and it was stolen, etc.). On your way to work your girlfriend/boyfriend calls you and breaks up with you. And when you get to work, your boss gives you a letter as he tells you that there have been some layoffs and he is sorry. You open the envelope to read the letter and get a paper cut. Does that papercut feel the same as the one you got on a good day? The point I'm trying to make is that mood plays a big part, and so doctors are trying their best to motivate patients to improve pain in alternative ways. It's not always easy or convenient but with the opioid epidemic it is necessary. In my personal opinion, what drove the opioid epidemic was drug companies marketing these drugs as miracles drugs when they were first brought to the market, while downplaying there potential for addiction and abuse. I do not blame the patients with pain for this, because we are talking about peoples' quality of life and ability to function. Obviously these drugs have the ability to do that, so they are going to want them. And realistically, most of the older opioids are extremely cheap and generic now. It is not the insurance agencies that are creating problems for access as much as it is the laws that have been created that are trying to deal with regulating these meds. The number of overdose deaths is seriously appalling. It is completely disgusting considering the fact that it is preventable. At the right dose opioids will NOT cause respiratory depression which is why there are such strict protocols in prescribing them. And marijuana is still federally a schedule 1 drug. It is 100% under the jurisdiction of the DEA. Federal law supersedes state law if it is more stringent than state law, and marijuana dispensaries are literally being busted all the time, even in California. I don't know what the solution for chronic pain is, but I do know some options. "Opioid rotation" is one. There is incomplete cross-tolerance between opioids so switching between them cause produce more analgesia at an equipotent dose. But it's unlikely that having one medication at one dosage will be a long term option, and that's another reason doctor's like to see their pain patients so regularly: so that they can monitor them. There is also of course the fact that most opioid prescriptions cannot be "refilled."

Thank you for your thoughtful and excellent reply. Regarding the emotional component of addiction, you may find this blurb from Jordan Peterson of interest : ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-fjs2gPa5sD0T.htmlhere are several towns near me where the jobs have been sent to overseas sweatshops. The problem of pain numbing drugs and alcoholism have skyrocketed. It would be of great benefit if had a drug that can effectively address physical pain but not emotional pain. For that to happen ,we need research on how emotional pain gets interpreted as physical pain. Thank you and Best Wishes.

This seems kinda fake, but it is what doctors are doing with all their patients. My pain doctor that I had before this crazy war against opioids would start you off with a dose that was more than adequate but then encouraged you to work your dose down over time. It actually worked because you are able to get yourself physically and mentally fit and build up your natural endorphins over a period of time, therefore reducing the amount of your dose over time. These days though they give you a dose that is too small for a lot of people especially when you are active and metabolizing the meds quicker.

Oh wow just watching this made me cringe. I can't stand when a Dr. Tells me to do yoga and stretch and whatever else. It's my body and I no what I can and can't do and for as antidepressants, really come on that doesn't help your pain and alone are very very additive themselves. They just put my father on them about 4 or 5 months bk and he has always taking his meds like it said and done fairly well considering and now he's going dwn hill so bad all because they're taking him dwn off his meds. It angers me to have to see him this way.

I've been seeing the same pattern recently. Why even bother substituting opioids with ineffective antidepressants in the very elderly? Reduce the dose if necessary (falls, confusion ...). Educate about ways to stay active and why it is so important for health. These Drs must be in the pockets of someone making a lot of cash through antidepressant drugs. Drugs have their place and purpose. The US-led war on drugs now gone global with its replacement PUSH ANTIDEPRESSANTS. Or even better, I hearing about people being prescribed anti-psychotic meds for their non-compliance in the DRs PUSHING inappropriate drugs on their patients without support.

My pain clinic doc not once counted my pills. If they had, I wouldn't have developed a problem. I also showed up so stoned I didn't even know where I was. My husband told me the whole thing and I didn't believe him. They drug tested me and I had vicodin and percocet in my system. I had a knee injury and the pain was so bad I'd have taken ten bottles of whatever to make the pain stop. Still, they sent me home with more percocets. I could have died and their dosing me up more! I became addicted and got tired of withdrawal every month days prior to my next visit. I had to take things into my own hands because this drug was killing me and the pain doc was willing to sign my death certificate if it meant extra cash for him. I used kratom and went off the percs. It's been five months now and I'm finally feeling free of this horrible drug. I'm still in ungodly pain but honestly those pills made me a zombie. An addict knows how to work the system, I did and take full responsibility. Now I'm scared to death another surgery will creep up and I'll have to use the drugs again. Guess I'll cross that bridge if it happens. This chick is addicted for sure, she just doesn't understand that her body is addicted. You can even hear it in her speech, something only another addict might be able to hear as well as a trained physician.

Mike Mendillo I never said it wasn't scripted but probably pretty close to what an actual appt looks like. You're pretty mean, must have struck a cord with you. Let me guess, you're on pain pills too. Sounds like you need an antidepressant and some Valium too. Your comment made me laugh, only someone so miserable attacks a person they don't even know. You sound pretty misogynistic, let me guess, a trump voter too.

just because they don’t have a solution, they want to jump into yoga meditation antidepressant etc.... I usually suffer alone .... and don’t go to doctors ... because I know they don’t help and they look at a patient as a customer who should leave their office ASAP ... which is pretty understandable..... it comes back to what I said. Suffer alone

Lots. Too many. Sometime death is a cure not a defeat. Some things like chronic pain is truly worse than death. I read an article about a man who after having his opioids stopped, he went into his doctors surgery and ended the doctors life. After this he ended his. In another tragic case, a man sits on a park bench with his wife of 40 years. They spend time taking over their memories and life. The wife knows what he is going to do and respects the man's wishes out of love. When the couple and finished talking the man takes his gun and points it to his chest. Later the police arrest her for faliure to act to prevent him from doing this. She done it out of love for him and their beliefs they would be together again in a healed body. Sometimes chronic pain after decades can become too much to bare.

I'm a provider in a pain clinic. It's so tiresome trying to balance DEA regulations and rules with what most of the people "want." They come to the clinic and get angry if you try to talk to them about managing pain in ways other than strong narcs. Then, when they demand their opana and oxycodone and get addicted they get mad at you for prescribing what they insisted they needed. They don't have a job and even if they do they come and spend roughly $1,000 a month on the office visits and meds....most of these people can't afford it, unless they are selling their drugs. These dealers don't realize that the piss tests are a lot smarter than they are and can tell when they have spiked their urine to pass the drug tests but don't take their meds the rest of the month. And they all "work out of town" so they are not available for pill counts. I've been doing this about 6 months and started out with the best of intentions but if you let them they will walk all over you, take advantage of you, lie to you, and suck the life out of you. They get mad because you will not authorize early refills. They get furious when you approach them after they have failed the last several drug tests and need to be dismissed. They can't remember what dose of lisinopril they are supposed to be on but they sure know the doses of their narcs. And they want the max a pain patient is allowed to have, no matter what their imaging studies say.They are a sad lot, really. Their only motivation is to get more pills. I finally realized you can't treat these people like responsible adults. We do have a small number of honest and responsible patients and they are greatly appreciated, but the majority are addicts and will go to any lengths to get their fix. And evaluations and exams are a joke, because they will only give answers they think will get them the most medication. I'm trapped, I have a big family and a lot of responsibilities and can't afford to leave, but I am doing my damndest to weed out the junkies. That's about all I can do.

mountainaire Is it worth dismissing a patient if he/she goes off narcotics from some sort of violation then finds their way to heroin and overdoses even dies? At least you could do before you dismiss someone is offer them help with their problem. I understand its a double edged sword and you try to make everyone happy plus you cant let them walk all over you, I can def see that. so before you release a patient to a miserable hell the least you can do is offer them Suboxone or refer them to some where they can get it. You gotta do what you gotta do but don't toss people aside with no hope when they have a serious drug problem.

mountainaire Oh my God...poor you. Poor you with your big family and responsibilities. Do you not think chronic pain patients have those also? Apparently you think the whole thing is a big fun adventure that is only embarked on in order to take advantage of you. Could it possibly be, in any universe, that lisinopril doesn't work for their pain? Or maybe they've tried everything else under the sun that doctors push on patients in pain and perhaps those things didn't work - which is how they ended up going to a PAIN CLINIC. For most, it is an absolute last resort, because people will only deal with people like you out of sheer agony. By the way, how do imaging studies tell you a pain level? I wasn't aware that there was a test for pain levels. I'll tell everyone I know, that's wonderful! No need for rating pain on a scale or with smiley or sad faces, or talk about what a patient is able to do or not do - 'imaging' is what explains a person's pain level. I'm sorry, but the best pain management doctors out there know that 'imaging' does not explain pain levels. I also don't see you mentioning varying degrees metabolism of pain medication. You just want to fit everyone into a little box...that's amazing. You have no business doing what you do - you need to quit, immediately, because even if you are catching addicts, you are harming other legitimate patients in the process. With these horrible comments, it's likely that karma will visit you someday in the form of a chronic, painful illness...and then you'll understand what 'trapped' really is. Real pain patients do not sell their medication. Unfortunately, opiate medication (and likely not lisinopril, a blood pressure medication) is often what works best for very severe pain, and people seem desperate because they want to function, and by function I mean get dressed or walk or not cry in bed all day, basic things like that. And my God...why are you charging $1000 a month for office visits and prescriptions? That's financial rape! I also imagine most of your patients can't work because they are officially disabled. Or is that not the case? You can't read anyone's mind - and what may seem like drug seeking, may be pseudo addition due to under-treatment of pain. If you work at a pain clinic you should already know this - it's abhorrent that you don't. You are the reason people hate doctors. Just, shame on you.
