Appearance:
bradykinesia, ↓ spontaneous movements,
Hypophonia and palilalia (repetition of the end of a word)
MMSE :dementia
Vitals:
Postural hypotension
Gait:
Rigidity of movement - difficulty initiating movement, en bloc turning.
Shuffling with narrow-based gait
. Diminished arm swing.
Festination. Retropulsion - reveals poor postural control
Head and Neck: Mask-like and or surprised facies.
Decreased blinking frequency. Drooling. Upward & downward gaze paresis in progressive supranuclear palsy. Glabellar tap reflex.
Extremities: Micrographia, pill-rolling tremor.
Tremor at rest . Bradykinesia.
Tone: Cog-wheeling & led-pipe rigidity
, ↑ Tone with sudden release.
No spasticity
Bradykinesia: ↓ Speed & amplitude of complex movements
(e.g. Tapping of fingers on table
or
tapping foot on floor)
