Hello!!! So glad you're still here :) As I was going through that (and the next video) I kept trying not to delve too deep into the different aspects of acidosis because I really wanted to stick more to the respiratory aspects (i.e. didn't want to lose the forest for the trees). But absolutely! None of this makes proper sense if we don't cover that too! Thanks so much!!

Hello Gaitree- thanks for watching and commenting. We're so glad these videos are helping you! We are running a series on high frequency ventilation soon, and then will get back to vent parameters. We have been planning an ECMO lecture for some time, but it's so rarely used now in NICUs (babies mostly transferred to PICU) that we've been dragging our feet with it! Thank you so much for your suggestions!

I have been following your videos for awhile now, and I absolutely love your channel ! Will you be coming out with more patient based scenarios with blood gases and how you would react to them ? Same with PPHN types and interventions you would do for each of those scenarios ?

Hi Dr. Tala! Thoughts on a 46 weeker on nasal CPAP with the following numbers: capillary gas 7.35/68 (co2)/ 43 (o2)/ 37.7 (hco3); when taking an arterial gas the next day the numbers were 7.36/61/107/34.3

The gases are acceptable given we accept permissive hypercarbia in BPD (in this hypothetical situation!) It takes about 6 weeks to get a baby home after a trach- so if a baby is improving and active etc- then allowing for slow improvement is completely acceptable.

@@TalaTalksNICU one last question, would it be possible for a baby with the above blood gases to try a HFNC? Baby sound’s mucusy and produces a lot of bubbles on nasal cpap that seems to be causing choking.

Hello! This is a good topic- the only issue is that different countries have slightly different vaccination schedules. In the US, we give infants their vaccines as if they were born full-term. IE we give the two month shots (but not rotavirus) when babies turn 2 months old. Where are you based?

Hello Consuelo! No- we don 't really use diazepam in neonates- it knocks them out really quickly and it may be bad for their development. For anti-epileptics we generally use phenobarbital and Keppra and for sedatives we use versed (midazolam) and Ativan (lorazepam). Honestly though, none of them are probably good for babies- so like everything else- use extreme caution when giving any baby drugs!
