Lord, I know that I must die someday, and that sickness is but a preparation for death, for that liberation which will free us from all the miseries of this life and will lead us to eternal life, which we hope to share with the blessed in heaven.
Grant me the gift of perseverance to the end, so that when my summons comes, it may find me in the state of your friendship.
St. Peregrine, assist me at the hour of death. Intercede for me to the Divine Heart of Jesus and to the Sorrowful Mother whom you served so well in your life. I ask this through Christ, our Lord.

O great St. Peregrine, you have been called
"The Mighty" and "The Wonder Worker" because of the numerous miracles which you have obtained from God for those who have had recourse to you.
You who for so many years bore in your own flesh the dread disease of cancer, and when human power could do no more, you had recourse to Jesus crucified. He answered you by coming down from the cross and healing your affliction. Ask of God and Our Lady the cure of these sick persons whom we entrust to you (mention your request here).
Aided in this way by your powerful intercession, we shall sing to God, now and for all eternity, a song of gratitude for His great goodness and mercy.
