Pharmacy is a lucrative business for universities. As long as students will absorb the incredible debt burden, they will continue to be fleeced.
@briankfung
@briankfung 4 года назад
Absolutely. It’s unfortunate how much incentive there is for schools to open up. Though, it’s hard to blame them cause why would anyone turn down a lucrative opportunity? Not that it’s right....

As long as you are competent there is no saturation
@briankfung
@briankfung 3 года назад
Saturation isn't a matter of opinion, but rather a fact according to the BLS. The issue we're currently facing in the US is that even if you had 14,000 competent pharmacy students that graduate every year, but only 5,000 jobs, it doesn't matter how competent you are, there is simply no jobs for those competent individuals.

Pharmacist need to stand up for themselves and stop looking the other way when laws are being broken no matter how insignificant. When RpH allow this it allows employers to cut corners and use less employees to get certain tasks completed. So many pharmacist work off the clock . So many pharmacist allow techs to perform tasks which they are not allowed to just because they are over whelmed by the work load. I know that if you bring this to your supervisors attention you most probably will be fired, if so you must act on it and take action against greedy pharmacy owners. Its sad that this is the truth but it is happening everywhere and the government is so under staffed that they are unable to catch all these violations. This is why pharmacist must not forget their ethics and must follow the law no matter how small of a violation like allowing your tech to take a phone call. I ask you all to really be honest and see how many laws you and your pharmacy are breaking on a daily basis.

I empathize with your comments here. I imagine the setting that you're describing is primarily community/retail, which I don't have very much experience in. I really do hope the conditions improve as, anecdotally speaking, the working conditions don't seem great.

This discouraging news about the pharmacy profession really blows my mind. I remember being an undergrad in 2008 and hearing about what a great profession pharmacy is. Such great pay, good autonomy, good authority, well respected, and the like. Here in Canada, you need a very very competitive GPA in difficult courses to obtain admission into a pharmacy faculty. How things have changed! Disclaimer: I am not a pharmacist.
@briankfung
@briankfung 3 года назад
I was in undergrad from 2005 - 2009 and heard those exact same things: 6-figure salary, flexible work-life, $50k sign-on bonus, doctorate, etc. To be fair, pharmacy school USED to be competitive. I had a 3.3 and a 68% PCAT when I applied in 2008 and got rejected. Nowadays, it seems like anyone can get accepted. Things have certainly changed quite a bit. Very unfortunate

@@zohrant7399 To your point about it being up to the individual student, I absolutely agree! In fact, I think that an individual's ambition is single-handedly one of the major contributing factors into the opportunities they obtain down the road. However, there are two major barriers here: 1) job market saturation and 2) not everyone is ambitious.

I have been a pharmacist for 40 years. The thing you should do is look to the future, there is none especially in community pharmacy especially in urban areas, there maybe in rural areas. The PharmD title is a joke. It is just a way to pick your pocket for more tuition. Look at it this way in two more years you would hav an MD or VMD or DDS. A PharmD is just a title for insecure people.

The mistake started with the introduction of clinical pharmacy as the face of pharmacy practice. In Nigeria, Pharmacy school offers a mix of course, ranging from pharmaceutics, pharmacognosy, pharm micro, pharm. chem, pharmacology, and clinical pharmacy. Hence, there were lots of options open for us from beginning especially in pharmaceutical companies, where you can put those skills to work in the lab or as a medical rep. In my class, almost half of the the class got employment with Pfizer, Astrazeneca or Roche. They were given a car with a driver. Many had a house within 7yrs of graduation. Remember Nigeria does not have a mortgage system. You buy your house with cash. Pay and go system. It's sad that most developed world operate a capitalistic system which is killing the profession.
@briankfung
@briankfung 3 года назад
That's fantastic to hear of those opportunities in Nigeria. We certainly get a mix of training in the US as well, albeit, heavily focused on the inpatient clinical setting, The thing is, I think the oversaturation of pharmacy in the US stems from not only that one thing, but rather a mix of many other factors.
@chukwuebukaokafor3468
@chukwuebukaokafor3468 3 года назад
I'm a Nigerian too. A biochemist. A medical representative for a pharmaceutical company. Worked for 8 years now. Moving to US in weeks time. I wanted studying pharmacy but thank God for this info. In Nigeria, what you said is true but remember, sooner or later same fate may befall Nigeria where we have more pharmacist than what the pharmaceutical industry can take. Lot of Nigerians are doing everything to become graduates in pharmacy. Right now in Lagos, there's so much pharmacy retail stores opening. Streets is flooded with retail shops. I think we may likely experience what US is going through sooner.

I'm a Pharmacist based in the UK and I recently made the move into informatics for essentially the same reasons you have mentioned. The writing was on the wall for me since my university days. I feel sorry for the pharmacy students just starting out. I always tell them to expand their horizon and think of pharmacy as a gateway to alternative careers that don't necessarily explicitly state "Pharmacist." Underfunding by the NHS, increased hub dispensing and multiple other factors is causing chains and independents to consolidate stores. In addition theres a lot of parties that are interested in introducing remote supervision.. having a pharmacy open and running without a Pharmacist. With regards to hospital setting.. the NHS would rather provide funding for another nurse or doctor...
@clarity8296
@clarity8296 4 года назад
What careers would you recommend? I'm currently a new grad pharmacist and don't want to go into retail.
@briankfung
@briankfung 4 года назад
@Priscilla that's quite unfortunate to hear. I was not aware that this issue extended beyond the states (primarily because of the surplus of pharmacy schools here). However, now I'm a bit intrigued into the contributing factors to the increased surplus and/or decreased demand in the UK that you have presented here - namely the underfunding by the NHS. Given the funding structure in the UK vs. US, it would almost seem as though there would be a larger pressure on pharmacies in the UK to cut costs and increase efficiencies as compared to the US. Perhaps a fun side project I can look into next. Thanks for sharing your experiences!
@briankfung
@briankfung 4 года назад
@Clarity That's certainly a difficult question. Given the general demographics of BLS, the vast majority of positions are in retail. However, the largest growing opportunity is in hospital and health systems. In terms of job availability, that'd probably be the first place I'd shoot for. Keeping in mind that there is an insane amount of competition as well. Can't go wrong with finding ways to upskill yourself with transferrable skills and networking is always a wildcard that could potentially open up doors. Keep me posted - and good luck on the job hunt!

@briankfung
@briankfung Год назад
What's funny is that I've actually never attended HIMSS lol. I probably can't give any tips unique to HIMSS, but I think general conference tips could be useful. Some things I can think of: 1) network with everyone you encounter. Especially those in your vicinity. Usually leaders of that particular conference would also be volunteering and it's a great way to introduce yourself and expand your network. 2) Approach random people to introduce yourself. You can do this in a targeted fashion (individuals you know you want to meet) or random (e.g. strangers you don't know anything about). The latter is easily done during lunch by approaching individuals that eating alone. They usually appreciate the company. 3) Attend any sessions you're able to and be sure to say hi to the speakers and ask them a question about their presentation. It's a great way to network. Overall, just use the opportunity to meet NEW people. More importantly, don't expect anything in return. Having that mindset will help grow your network authentically and that could be beneficial later down the road. Have fun!

@briankfung
@briankfung 3 года назад
That's a great question. There likely are other areas of pharmacy that are "better" relatively speaking vs. retail. However, from an absolute count of jobs, those areas of pharmacy likely have significantly less jobs from an absolute count - which is the more important measure. E.g. You might see something like 200% growth in XYZ pharmacy position, but then you realize that the statistic came from an absolute number of 5 total jobs currently that have increased to 10 total jobs. Overall, I think the job market in general, is extremely saturated. Thanks for watching and supporting the channel! I really appreciate it!

It’s interesting how alluring pharmacy was for me as I accepted a position the first year the program I chose dropped the PCAT requirement. This was probably one of the last barriers for students not attending pharmacy school, like I probably wouldn’t have started if I had to take the test. Now I’m in year two, and feel so overwhelmed with school. It’s so stressful, and it makes me wonder if I should’ve taken the time to prepare for my dream of dentistry by just taking a year postgrad. Working in a pharmacy is very draining and school is just as hard as any other doctoral program. I don’t understand how a health profession like pharmacy could get this unregulated.
@briankfung
@briankfung 3 года назад
Noah, that's a pretty deep reflection. The sentence that caught my eye was that you were wondering if you should have pursued your DREAM of dentistry. I'm a huge advocate for pursuing things that you are truly passionate about. Coincidentally, I had similar thoughts as I entered my 2nd year of pharmacy school about switching back into computer engineering. Ultimately, my classmates convinced me otherwise at the time. Thing is, the ROI was much easier to justify back then vs. now. It is quite unfortunate how unregulated it has gotten.

@briankfung
@briankfung 3 года назад
Hi Kade! That is a very intriguing question, but one that I'm not confident I have the answer to. However, I would think the ROI, in general, is quite low given the fact the number of available jobs and the average salaries are expected to decline. Thus, even if you were to receive free training, the lack of available jobs and the existing saturation of pharmacists would make it not ideal. That'd be my guess without doing more research.

@timkirk12 4 года назад
Steven. Pharmacists have been trying for decades, hence the "provider status". There has been a shortage in primary care that pharmacists "tried" to fill and claim jobs in that space. Unfortunately, physician assistants or a nurse practioners are much better fit that role. We found out that MTM doesn't pay enough and has no very little to no demand, and creates almost no additional jobs. There have been some expansion in pharmacy benefits management/managed care area and other unique areas of practice, however that did not come even close to being enough to keep up with the expansion of pharmacy schools.
@stevenkim4363
@stevenkim4363 4 года назад
@@timkirk12 Which is the entire reason why we are seeing this negative job growth. Tbh, at my independent pharmacy, I've seen very good result and impact from quality MTMs, expanded vaccination, and there is definite demand for both. It is not immediately in "demand" because almost no patients know about the MTM services, unless we offer them, and many expanded vaccination opportunities are missed out. I agree with what you are saying and would be best to get better reimbursement for our services. Once we prove our value and increase our own demand of the service, money surely will follow. I think current MTM reimbursement $55-75 is doable with right system set-up. Good feedback Tim
@briankfung
@briankfung 3 года назад
@Steven & Tim - thanks for sharing insight from the frontlines. It's unfortunate that we didn't end up getting a bigger piece of the pie with the shortage in primary care. Might sound like I'm beating a dead horse, but I feel as though the advocacy in our profession, relative to other healthcare professionals, is extremely poor and likely a factor in the equation there. As for MTM, I've heard a mix of things. I really do hope it's a sustainable business model and additional revenue streams come to light for our community/retail folks.

I agree to an extent. Do we need more representation? Certainly. However, I disagree that representation alone or in itself a solution. I actually think we need more representation in non-pharmacy, legislative efforts and ones that are more collaborative. I feel like it's an echo chamber how often we discuss how split pharmacy organizations are - especially compared to other healthcare professionals - when it comes to organizing for our interests and efforts. I do agree that corporate manipulation is a contributing factor. Though, I wouldn't say that is something unique to the pharmacy industry. Greed and personal interests are probably always going to be there.

I've spent a lot of time thinking about that and how our parents mean the best for us too. Unfortunately, I think I spent the earlier years of my life, especially my early 20s, resenting the pressures I felt when growing up to do something in medicine. However, as I get older, it's also very difficult to place any blame on them because that was all they knew. My parents were immigrants that never went to college and I feel very grateful that they sacrificed what they did for me to have the opportunities that I have today. As a stoic, there's not much to stress about when it comes to the past, but there is A LOT we can do to change the present and the future. I'm confident we will break the cycle for those future generations.

Hey Brian , Thank you so much for putting so much effort to makes these videos 🌸 I'm a 4th year Pharmacy Student in Italy 🇮🇹 . I got scholarship every year so I am in no debt, infact I've a bit savings cause of my job . I'm interested in clinical aspects , dermatology and direct link with patients independently . Thinking of switching to MD after my school .. I'm confused as heck . We are always supposed to work under doctors in clinical pharmacy . Even after being an Independent Prescriber , I'd still have to work under em somehow and that distracts me the most -- also idk if there's any speciality in dermatology after MPharm/Dpharm. What would you suggest?
@briankfung
@briankfung 3 года назад
Hi Amnes! I'm glad you enjoyed the video and I sincerely appreciate the comment as I did spent a considerable amount of time putting this video together. As to your question, I have to admit that it is quite out of my area of expertise. I'm unfortunately a bit uninformed about the job prospects in Italy, the working relations with the physicians there, and the dermatology specialization. With that said, and please take this with a grain of salt, my first thought is something someone told me when I told them I wanted to go to med school after pharmacy school: Practice as a pharmacist first for a few months and/or years before deciding if you want to switch. Maybe you'll end up liking it. Further, if the cost of MD isn't significantly higher - and I vaguely recall medical schools being extremely affordable in Europe - then perhaps it's not a bad choice. However, life is short and time is everything. So definitely try practicing as a pharmacist for a bit before making your decision. As for dermatology, I honestly have no idea lol. At the end of the day, I'm sure you'll make the right decision. Best of luck!

Thanks. Great video. I’m a P1 student and thinking about leaving the pharmacy school before it’s getting too late of all student’s debts and can’t find job... but wondering after leaving the school, what would be the best option to start?! I’m thinking about IT as a software developer? Any suggestions?
@user-lu6yg3vk9z
@user-lu6yg3vk9z 3 года назад
Yes that is a good, but don’t go back to college to get a software engineer degree, computer science degree ect it is a waste of money and time. IT field don’t care about degrees. Coding is more of trade. Look into coding bootcamps.
@briankfung
@briankfung 3 года назад
Hi Zohreh! I don't usually like to tell students to leave or not leave pharmacy school, but I will make the obvious comment that leaving as a P1 student this early on is probably a good ROI relative to staying. To your question about options after leaving, that is an extremely difficult question as it depends on what you enjoy. Personally, I think people should pursue things that they are passionate about or enjoy. So if you enjoy IT or software development, then by all means pursue that. I may be a bit biased, but I think that anything in the STEM fields right now, especially software engineering, is a solid choice from a monetary perspective. Further, just like what Filmon noted, a degree isn't necessary and coding bootcamps is typically all you need to land a job. Best of luck!

What if your goal is to go to pharmacy school, and then pursue nuclear pharmacy or working in a pharmaceutical company/lab? Nucelar pharmacy is a small sector of jobs and those jobs require years of experience to get those jobs they won’t hire a new grad. If you want to work in Pharma industry you don’t need a Pharmd a Pharmaceutical science degree is what is needed.

@Paul, I'd echo what Filmon said above. Nuclear pharmacy is a very niche job and going to pharmacy school, in light of the existing job market, just to get to Nuclear Pharmacy - especially if a residency is involved, likely has a very, very low ROI. I'd highly discourage it.

Not really. It’s a supply and demand. there are more graduates then jobs. jobs are on a decline from retail/hospitals closing throughout the country. Soon A.I. will cause more job losses. usually jobs correct itself over time, but this time it is different theses jobs will never return back to it was. this is a different economy u need to accept it and move on to another profession. don’t waste money and time on a dying profession. P.S. Also out in the real world not like your protected bubble of college . No one cares about what you like or are passionate about. The real world only cares about one thing and that is money. Your employer only cares if you can make or save money for them. If you can’t do that then your employer sees no value of your services therefore won’t hire you.

Great video Brian! What is your opinion on the effects of automation, A.I, machine-learning, etc. on the Informatics space? Would introduction and expansion of these tools within the field create more or less opportunities for PI? Thanks in advance.
@briankfung
@briankfung 4 года назад
Hey John! This is a great question and one that I have mixed thoughts on. From a supply & demand perspective, I don't imagine the field of pharmacy informatics to have significant net growth, perhaps a little, at the moment because the AI technology is so different than what I personally consider pharmacy informatics in terms of job responsibility. Many of my informatics colleagues, including myself, are upskilling themselves currently to better adapt to AI. With that said, this is from a completely uneducated perspective lol as I haven't explored the opportunities myself. I'm planning on tackling this area next though as I'm curious myself. What do you think?
@johnho8264
@johnho8264 4 года назад
@@briankfung I have mixed feelings as well. From an optimistic point of view, if any pieces of these processes require medication expertise then presumably PI would play a role in management and maintaining these tools at some level. I could be widely off the target with this high-level example, but the key elements of AI are essentially complex algorithms, which require large amounts of data. Data in healthcare is not all equal, so maybe a niche could be cleaning unstructured data or creating records related to medication management to be inputted into these algorithms. On the flip side, if the scope of these tools are narrow enough yet impactful, maybe we wouldn't have much involvement. Look forward to your next video about this topic.
@briankfung
@briankfung 3 года назад
Hey John - I completely agree with your comments here. Going back to your original question, I think I was responding to your question from a general pharmacy perspective vs. a pharmacy informatics perspective. The former is a bit more contentious as saying things like "AI will bring more opportunities" makes others think there are enough jobs in this area for ALL pharmacists. Absolutely disagree as these are very specialized roles. The latter, which was likely the question you were asking and I misinterpreted, I do think it will bring a NET growth of jobs for pharmacy informatics. Take the National Academy of Medicine's publication on AI in 2020 - they talk about this exact thing. It's a huge area of opportunity for all the reasons you mentioned - especially given the large role medications play within our society. I currently work on projects that involve population health informatics and artificial intelligence and cannot stress enough how important a pharmacist is in this capacity. However.....I think the caveat is whether pharmacists are able to pivot into these areas of opportunity. The window is open, but it'll be up to those of us in pharmacy informatics to make the jump.

Why not make a video on how to survive in saturated market Rather than demotivated people who. Are so passionate about there career in pharmacy
@briankfung
@briankfung 3 года назад
I didn't make the video to demotivate individuals, I made the video to look at objective facts and subsequently provide an opinion on the return on investment based on those facts. The facts lean towards the market being saturated. Thus, if I encourage individuals to enter pharmacy, I will be contributing to the problem, rather than helping it. Moreover, I provide one exception: if you're passionate about it, pursue it. Thus, the video is in alignment with your comment.

I almost wonder if there is going to be an increase in residency lengths, PGY3+ over the years, similar to medicine. I also almost wonder if attending physicians will suffer the same fate in the long term, particularly specialists that aren't cheap (I could probably look this up) . Anyway, residency length might prolong the job market further (bandaid solution), not to mention decreased salaries across the board.

@Kris - I wouldn't be surprised if that was the trend. I've discussed this issue with a few of my physician colleagues and they certainly think it's the same game: extending training to prolong the issue and ultimately leading to a terrible outcome. Overall, my thought is that the upstream issues (e.g. pharm schools, number of pharm students) needs to be addressed as a priority before others. Historically speaking, our chances of that happening doesn't seem very likely...

I really wanted to be a healthcare professional. But, it's better to do this than be unemployed with 150k in debt. I tried convincing my ex to also consider leaving pharm school in her first year. Didnt work sadly. I hope the best for anyone struggling.

You can always pair up that tech-related degree with another healthcare degree and/or experience. If you ever decide to work in healthcare in the tech area, I would argue that ANY healthcare experience + tech background is better than someone with pure tech experience. It's very important to understand the clinical workflows and issues that occur on the ground to implement the technical solutions to address them. I wish you the best of luck in your tech related degree!!

@briankfung 3 года назад
The current statistics would suggest that it is not worth it. I can't predict the future and that could always change by 2024. However, my thought is that the chance of that occurring is slim to none for quite awhile. All industries fluctuate in their demand and supply curves. Thus, the assumption would be that the demand for pharmacy will increase again in the future. Granted, new technology could make that demand questionable in the advent of artificial intelligence.

Hello Brian, thank you for your video! Do you think the pharmacy admission will be easier year by year based on this trend (Sorry, kinda silly question) I might be a really really weird candidate of the future pharmacy school admission. I came to the U.S since 2015 to pursue my dream of being in medical field. I decided to apply pharmacy school since 2017. The reason is that the pharmacy degree especially doctorate one from the U.S is still amazingly valuable and easy to find a job even in Beijing or Shanghai(Medical treatment in the U.S still much more advanced than in China). I hope to change my country's medical condition by what I learned in USA. I would say that I am willing to see the difficulty of admission become less competitive every year (Sorry for saying that). Since the Covid-19 pandemic, it is hard to find some volunteer work. What can I do for my interview? (if I can be invited😂) I appreciate your help :)
