I also do not understand why the stat consultation for endocarditis should be first. I just graduated PA school and remember clearly learning during didactics and also with a patient during my inpatient rotation with infective endocarditis (and some research done for that preceptor using UpToDate), that initially blood work is done, including most importantly blood cultures (x2 at least, if not x3, 15 minutes apart, from 2-3 different areas, depending on circumstances) should be taken before empiric antibiotics are administered. This would also be the standard approach in the ED, as far as I remember. Can you please explain?

I am not understanding a stat consultation for the endocarditis question. Why would a TEE not be first?

I agree. Why not do the TEE to confirm before consultation.

That’s how it would work in applied, real world medicine. First, labs and cultures, then TEE. Also knowing the context of WHERE you are hypothetically practicing for this question would be helpful as well. Working in cardiology myself, a work up would be much more helpful than a stat consultation without any information. But this is PANRE, so things don’t always make practical sense 😂 lol

Just curious for the question around 14:30, since the abdominal aorta runs slightly left of midline when the stem mentioned right flank pain I used that to kind of rule out AAA. Is it often that an AAA will present with right-sided pain or is that a rarity? Thank you!
