When casting for equinus we leave casts on for 1 week. During that week we have the following protocol for mangement: 24h hours non-weight bearing. Then when POP is set hard weight bearing and activities for active extension of knees. Practise sit to stand and walking and stair climbing. They are seen by the therapist ideally on day two, five and seven when the cast is removed. This may be repeated as serial plastering or after a couple of months free of casts depending on child's response and age. We always do a week's casting like this before sending for AFO casting so that the best position is then achieved for ADFO casting. Botox is not an option in our region as the cost is prohibitive. This is a viable alternative and has had good results.
