I could be wrong but I think in the US they call it "freestyle" when you rap something written on a random beat, and "off the top" when you are improvising.
Theres a very very small group of rappers that can freestyle, just because you cant come up with something on the spot doesnt mean you cant thrive as a musician/rapper
