I just wanted to thank you for this. I'm not a nursing student but I had preeclampsia with my pregnancy 8 years ago and I didn't understand it in depth like I wanted to. In a way, this deeper understanding helped me to feel better about some of the lingering fear around pregnancy that I still have.

A wonderful overview. Well done.! 20 yrs OB/GYN and then a highrisk pregnancy Multiple with PIH and PCS. Well done. Biggest thing, that BASELINE BP an elevation can look to be WNL and due to a lower baseline BP or a low Pre Gravid BP you can miss that elevation. PIH labs are vital, and with a lower baseline BP all dependent edema with facial edema should be considered. These with the urine obtained must be addressed any changes in color, smell and amount, even slight spilling of protein . As Pre-eclampsia and Eclampsia can sneak right up on you. Be sure to bring all your information to the OB and push if needed. YOU see that pt more than them, and must advocate. My bp was 128/88 and I was Eclampsic and completely missed, by even me an OB nurse. Just a tid bit, good luck on your exam and careers!

Thank you for this video because I had EXTREMELY SEVERE preeclampsia with my first and (so far) only pregnancy 4 years ago.... And I've been wanting to understand what the hell happened to me and why... Especially when prior to onset, EVERYTHING had been monitored and checked out great both prior to pregnancy and during. Here's my story in full for those curious: Me: 5ft 4 inch 27yo Caucasian female, 1st pregnancy, pre-gestational weight 125-130lbs, zero hx of hypertension ever in life, no hx of cardiac, pulmo, hepato, gyno, or any other major conditions besides fibromyalgia, major depression and Anxiety and 33% S curved scoliosis, + recent knee surgery. otherwise being a healthy and well monitored 27yo female. Wouldn't have been that terrible except all scans and labs and OB appts being great and baby coming along perfectly on track through 20 weeks and 24 weeks scan at MFM appt and US/Sono is not good, baby showing absent end diastolic flow, IUGR and 2 weeks delay underdevelopment. My blood pressure was high for the first time in my life. Admitted immediately to women's center in the hospital (MFM office was thankfully in the hospital building already) straight from that doctors appt. Admitted at 24.0 weeks, given prognosis and put on bed rest and admitted into antepartum ward until it was time for delivery which they had hoped could be managed to put off from happening as long as they could. They hoped for at least 2 more weeks to give baby's lungs time to develop for stronger chances for baby's survival as micropremie neonate, as was the expected prognosis of conduction. We hoped for two weeks. Universe gave us 4 days before fetal blood flow was in full reversal and distress, and my blood pressure was 192/98. 7am, morning shift nurse gives 2 vials of lobetalol into IV immediately, no BP reduction at all, orders bolus of magnesium sulfate MEGASTAT, gets NICU notified, L/D +OR notified and prepped and puts me on liquids only for a few hrs and NPO cut off at 1pm to allow 6hr window minimum prior to emergency c section scheduled at 10pm. 6:45pm they push me up on em-c sxn surg time to NOW, roll me to OR feeling very definitely like every intense medical drama scene ever. Given epidural, thankfully BP was lowered enough with Mg Sulfate that they didn't have to put me under. Epi works splendid and at 7:08pm my 24.4 wks (6 mo, 4days) son was successfully delivered and intubated, weighing 410grams. He fit into the palm of his father's hand and was just barely bigger than whole hand. He even CRIED a beautiful tiny squeaky cry in the few seconds prior to being intubated....... He made it 33 days in NICU before horrifically passing in my hands from perforated bowel that resulted from him CODING 12 hours prior to that due to atelectasis, sudden full lung collapse and CODE. Was revived with adrenaline but in even more extremely delicate danger. Visited for touch time at 10pm. 2am woken by emergency call from NICU to come to hospital NOW because he was crashing. Made it barely in time to be there for his last few breaths and moments and got to hold him in both my hands for the first, last and only time outside of my womb or the isolette as he passed and was gone within just a minute or two at most..... And after everything, I am forever grateful for still having the chance to have every second I did with him... And I will never be able to get over the guilt I feel for the completely and utterly unexpected devastation of an outcome and trauma that his father had to experience without any ability to do anything at all and forced to endure what tragic fate that struck without any power or choices possible.... I've been and able to come to cope with my trauma and pain to have gone through all that as the mother but I feel guilty for having been blessed with having spent more time with our baby since I carried him and felt him 24/7 and everything. He didn't get to have any of the miraculous and wonderful time like that whatsoever... He never got to hold his son... I feel forever guilty to be the woman that tragically had to be the one that involuntarily brought this fate upon him. I know I'm absolved of any true fault as it's entirely not able to be controlled if preeclampsia will occur or even confidently screened for if it will happen or won't happen for sure, just that could be increased risk of occurrence if right stuff gets screened for. So yeah. That's my story of my experiences with my nulliparous pregnancy, preeclampsia, emergency premature c section, micropremie neonatal delivery, growth and horrifically traumatic loss.... Been 4 years but I'm far from being recovered entirely... Definitely so much better than I had been immediately at first after the loss so, progress yay. But I would not ever wish this genuine hellish tragic Greek play of a fate upon anyone...

Thank you so much for teaching others about these issues. I would be forever in your debt if you were to cover HELLP in more detail. I was diagnosed at 31 weeks and almost lost my life and my daughters. My kidneys and liver were shutting down and my placenta was already 30% infarcted. Without the care I got at the NICU hospital, my daughter and I would have died in a matter of hours. Unfortunately, the OB Drs and Nurses didn't know about it but they knew enough to transfer me to a better hospital.

My granddaughter lost her baby 1 week before due date and now diagnosed with preeclampsia and who knows what else. I really feel they missed a lot of signs in diagnosing or caring for her during her pregnancy. That baby was perfect and should have lived. I wish I had read more about complications much earlier. I knew something was not right but was not with her so didn't sit on her doctor's shoulders.

I was in nursing school last year and my boss at my job was pregnant! And she kept telling me she’d been having headaches that lasted a week or more and one day she decided to text me her BP readings for that day and it was like 139/89 and I told her “ you know 140 is preeclamptic range right”? She said that her doctor wasn’t worried unless it hit 140! Fast forward early April her blood pressure hit 156/100 and she was put out of work! When she had her son in June she was induced due to preeclampsia! I’m like I told you that back in March, when you were 27weeks and you didn’t listen! Long story short! They are both fine and her doctor is a idiot!

I had secondary HELLP syndrome with my first daughter and I was only 20 yrs old. I didn’t have any complications after birth but I remember staying in the hospital for about 2 weeks or so.
