Hi . . 1:I will identify the cause of the problem initially. . 2 : I will take into account personal safety and make sure a witness with me all the time 3 : I will explain the situation and offer an apology to patient as early as possible 4: I would escalate the incident to line manager and senior staff 5: I would raise an incident report with date, time and event 6: I will documents all the events into the case note
