PRP vs Peptide Therapy for Chronic Pain

A really great discussion on the differences between approaching chronic pain with peptides such as BPC-157 and PRP (platelet rich plasma). In our experience, we have seen better results with orthobiologic injections (which includes PRP) compared to peptides when only one approach is chosen. However, we frequently pair these together for improved effects.

If you’re new to my channel, my name is Dr. Drew Timmermans, ND, RMSK. I’m the founder of Regenerative Performance in Gilbert, AZ, specializing in interventional orthobiologics, which is just a fancy way of saying we help people reclaim their life from chronic pain without steroids or surgery. How I got here…
22 yrs old: Graduated with a Bachelor of Science in Kinesiology from the University of Western Ontario.
23 yrs old: Competed as a long sprinter, anchoring the 4x400m relay to a silver medal at the Canadian national championships, contributing to Western's first national title in men's track and field.
23 yrs old: Started working as a personal trainer for GoodLife Fitness in Canada, quickly becoming one of the Top 10 Personal Trainers nationwide.
23 yrs old: Herniated two discs in my lower back, ending my track and field career and sending me into a multi-year battle with chronic pain.
24 yrs old: Enrolled at the Southwest College of Naturopathic Medicine (now Sonoran University of Health Sciences) after discovering my passion for naturopathic medicine.
26 yrs old: Still battling chronic back pain, I finally became pain-free through a platelet-rich plasma injection.
28 yrs old: Graduated and become a licensed naturopathic doctor.
29 yrs old: Completed the first-ever residency in regenerative medicine and integrative pain management at the Neil Riordan Center for Regenerative Medicine.
29 yrs old: Co-founded Regenerative Performance to help people with chronic pain just as I had been helped.
Today: We help people who have chronic joint pain reclaim their life with cutting edge treatments like platelet rich plasma, and stem cell therapy.

prolotherapy and PRP saved my life, after a chiro nearly crippled me, snapping ribs, snapping hips, lower back pain made worse hyper mobile neck, it took a lot of treatments but it was worth it, of course at 51 i cannot power lift anymore but i can lift weights again and live a normal life, never ever go to a chiropractor they are dangerous. We also injected GH BPC157 TB500 and GHK cu.

No direct experience with hGH for healing as it's not approved for use outside GH deficiencies so we don't have access to it.

This is the best info I've heard on peptides. I'm in California, trying to find a specific peptide called Ara-290, aka, Cibinetide, for my husband. He had a hemorrhagic stroke and is suffering from neuropathy. I heard this peptide targets small nerve regeneration. What do you think? I wish I could find someone as informed as you are in the Los Angeles.

I'm sorry to hear that. We haven't been able to get ARA290 in over 2 years (might actually be 3 years, it's been a real long time). I wouldn't be able to provide any guidance for treatment in this area as that's not my area of specialty sorry. You might want to look into Dr. Amen's clinics. I'm not sure how much he uses peptides, but they are a fantastic group for neurological conditions

I had a steroid shot in my SI joint and it didnt help. I had PRP in my si joint after that. Around the SI area Took about 8 months to be pain free. It lasted fot 1.5 years. Part of me thinks bc i fell off a wagon and it slowly got worse. Now im in the same position. ive tried shockwave and it definitely helped but im not pain free. The difference today is now i know what the source of my si joint dysfunction is. It's the dorsal sacroiliac ligament. That is the smoking gun. It took years to figure it out but that is the cause of all my suffering. So what do i do now? How can i heal this dorsal SI ligament forever, permanently? Should I try a steroid injection in the ligament first or try peptide? If none of those two help the ligament, should i try prp again or try stem cell? I heard that stem cell generates more stem cells than prp, is that true? If im gonna get a heavt duty shot, i want the best, most powerful one so i dont ever have to deal wirh this again and be pain free forever.

Hey dr. Just wondering How many prp injections does it usually take to help pain in distal finger joints? And do you need to have ultra sound guidance for those injections?

Patient response is unique to each person, and depends on a multitude of factors. In our experience, 1-2 treatments. We also perform injections with imaging (xray or ultrasound depending on the situation)

Hi thank you for your content, it is super helpful. I've seen several places that say that oral peptides don't necessarily survive the digestive track and that they are less effective than peptide injections. You mention you have seen success in oral forms?

Yes but BPC-157 and MK677 are really the only two I can think of off the top of my head that survive the gut

I also have a PRP question : does the needle '' pierce '' the tendons that are targeted or is the PRP injection done around/in the area where the injured tendons are?

From what I've read and heard, it pierces the tissue meant to heal. Eg Achilles tendon, the needle would pierce the tendon and the injection would go into the tendon itself.

The injection needs to go within the injured tissue. So if we need to treat the tendon, we need to pierce the tendon to get inside the tendon in order to do the injection.

If a PRP injection into the shoulder (with front and rear injection sites) using the proper technique and guiding imaging equipment did absolutely nothing for a shoulder SLAP 1 tear, would you even consider BPC-157 and other peptides such as TB-500 as a potential alternative for repair? Worst case they do nothing, but it is odd that there was no reaction whatsoever to PRP.

Most of the time if we see PRP non-responders we evaluate for misdiagnosis. Your pain may not be coming from the SLAP tear, even though you have that on imaging. We have seen this countless times, and when the PRP is directed towards the structure that is actually causing pain, pain gets better. But, let's assume your pain is coming from the SLAP tear. Peptides could be a helpful treatment for you, and failure with PRP doesn't mean that you'll fail peptides.

Do either bpc or prp work for spinal issues? Namely stenosis, bulged disc's, etc..

They can both be helpful, but BPC-157 is often not enough for most people. Our approaches are extremely comprehensive and focus around PRP injections as those give the best chance for healing

When trying to heal an injury using peptides, is skipping workouts entirely a requirement? Or it's okay to workout as long as you're not doing exercises that directly irritate the area?

That completely depends on the situation, as some people will need to drastically reduce their exercise and others will just need to modify. Unfortunately there is no one answer

Coming off a meniscus and ligament repair surgery. I am two weeks post-op and have started PRP with my orthopedic doctor. Would you recommend BPC-157 this soon after surgery and while going through three rounds of PRP, or should I hold off until I am done with PRP. If you do recommend BPC-157 right now, would you recommend oral or injections into the knee. With how raw my knee is right now, injections multiple times a week concern me.

do NOT inject it inside your own knee joint. God I hate this bro science. If you choose to inject it, inject it subcutaneously into your abdomen. You don't need to inject it though because oral arginate salt works just as well as injectable (again, despite what the bro scientists say). So moral of the story, just use oral. The best results are seen when you are taking the BPC-157 nearest to the time of 'injury' (or in your case, surgery)

Dr Drew. Is it safe for my 16 year old daughter to use BPC-157? I have been using it for the past 6 weeks and it has changed my like ( knee, elbows,calf). She had surgery oct 7,2022 pes tendon release and knee scoped ( x gymnast). After therpy was doing great for about 4 months and now down again with leg pain in the muscles, knee, hips etc. mri and exray show nothing wrong. Cant find anything on bpc157 and teens. Please help

BPC and KPV can be effective at managing pain. But the only thing we have ever seen to truly help AA is adipose stem cell therapy. The results in most people are amazing

I'm going to be looking for PRP injections from a doctor closer to me. Is there anything I'd want to ask them to make sure they're doing the PRP correctly? I've also heard of PRF injections. Any input?

Not necessarily. As long as there are no medical contraindications in your case, we would just need to run our calculations to know how much more blood we need to take to get a clinically effective dose in you

Hi Doctor, i recently got an MRI for my knee that has had pain for the last 2 years RESULTS: { Impression 1.6 cm longitudial horizontal tear involving the body of the lateral meniscus extending from the capsular margm into the froe margin. Associated 1.1 x 0.9 x 0.7 cm multioculated paramenscal cyst within the literal joit gutter, 3 mum radial lear involving the free margin of the anterior hom of the lateral meniscus. } --------------------------------------------------------------------------------------------------------------------- I started looking heavily into peptides for the last 3 week and wanted your opinion on the effectivness of the BPC-157, TB-500 10mg (Blend) or should i look into buying seperate vials of each instead. any help or guidance on dosage amounts for the blend (if valid) would be greatly appreciated!
