It's just what I feel listening this masterpiece.

I hear you! --- ON "FEELING UNDERSTOOD": "I often think lots of people know how it feels." => Yes, and no. Sometimes there are situations people genuinely don't understand your struggle, and then you can take that sentence in the lyrics at face value. But, it's also inner speech, "no one knows how it feels, no one knows how I feel" conveying an inner dialogue and a spiraling effect of making yourself believe no one will be able to help or understand you, as indeed society likes to push it away. A thing you indeed pointed out too. --- ON "HELPING THE OTHER": "We do care we hurt as well. But we feel we are powerless to help others when we can't help oursleves." => Help lies in the small things. Listening can be a big help, making time and space. Whilst you might not be able to solve the issue, it may mean a lot if you take the time to genuinely listen. So I feel that goes a long way already. It makes an invisible person visible again. I'd also say that in the words of Levinas a lot can be found on what he calls "the small goodness" aka "a little kindness". I'll leave you with this: "the "small goodness" from one person to his fellowman is lost and deformed as soon as it seeks organization and universality and system, as soon as it opts for doctrine, a treatise of politics and theology, a party, a state, and even a church. Yet it remains the sole refuge of the good in being. Unbeaten, it undergoes the violence of evil, which, as small goodness, it can neither vanquish nor drive out. A little kindness going only from man to man, not crossing distances to get to the places where events and forces unfold!
