Psychiatric Interviews for Teaching: Depression

In this film, the GP is seeing a patient who has a depressive disorder. The patient describes symptoms including low mood, tearfulness, reduced energy, reduced motivation, early morning wakening, loss of appetite, weight loss, poor concentration, reduced enjoyment and reduced interest in self-care.
The GP explores the effect of the symptoms on other people in the patient's life, explores the past history of low mood, and makes an assessment of suicide risk. The GP then gives the patient an explanation of depression.
Please note that this video has been made by the University of Nottingham for teaching purposes. The psychiatrist is a real psychiatrist but the patient is played by an actor.

I've had depression since about age 13 and I'm 41 now. It's basically a curse that never ends. Antidepressants, exercise, prayer, etc., reduce it but it never fully lifts. Prior to going to rehab I at least had the pleasure of feeling good when under the influence. Now I have sobriety and depression -- I'm back where I started.

Damien H: I see what u mean. I have gathered much knowledge, tools, but am down from walking it alone, and want a healing-and coping comrade to work aimfully and practise disciplinedly with. A lone log can't burn, as a lone hag can't nag, and a lone soul can't heal. We might build and develop, alter and invent as we go along. Being a joyful and comforting, helpful input and critic to each other as well. Taking it very easy at the start, so as not to have neg pressure of any kind. Practising earnest communication.

I hate how depression is portrayed as just someone acting sad. A lot of people with depression put on a happy face. They will joke with you. And then one day if the pain gets too much, kill themselves. Some people with depression struggle with expressing it.
@ElisabethogHestene
@ElisabethogHestene 5 лет назад
Not true at all, I never have energy to anything else than lay in bed all day long. Maybe people with high functioning depression are able to fake, but often patients with major depressive disorder don’t even feel the need to fake, because they just really couldn’t be bothered about other people around and they are often very exhausted and uncomfortable in their skin.

I'm a licensed therapist and professor in the US and I struggle to find videos for students. Here, the actress is astonishingly believable and real--but the clinician is also really, really skillful. This social worker exemplifies caring, thorough, thoughtful assessment. Really skillfully done. Any student of therapy should hold her up as a role model.

Man, the Depression patient is seriously a wonderful actress! She isn't overdoing it and isn't stigmatizing it! She and the doctor both verbalized how my depression is. I really want to know why we patients with depression have bursts of sadness when there's nothing to be sad about! My heart breaks for this woman and I'm wondering if she may actually have depression.

There is some wonderful casting and great acting here. The industrial/educational videos in the states are filled with terrible indicating and overacting and sadly sometimes its not even the actors fault as they are asked to "act" that way at the request of the directors or producers. This is honest and truthful. Wonderful!

Honestly here because I’ve just been crying and so sad lately. Been depressed since 15 and have tried to take my life a few times. It got so bad in 2018-19 that my body felt literally numb, no emotions nothing, I just felt empty for months at a time. Yes I do have good days but the pain never really stops. I’d never wish it upon anyone and I pray that anyone going through a similar experience gets through it.

Brilliant videos. I've never actually watched something more informative than this. Congratulations! It'd be nice to have a second video, adjunct to the interview, where the doctor briefly discusses the case highlighting the major features (especially those which might have gone unnoticed) and then discuss the treatment. What drugs? What psychotherapy? What kind of follow-up should be done? Thank you!

Great video. If only real consultations were like this. Mine certainly wasn’t. I’ve been diagnosed with depression and anxiety for the past 18 months. All .done via a ten minute telephone consultation resulting in a prescription for anti depressants and a referral to online cognitive behavioural therapy material. Mental illness still feels beyond the scope of everyday GP practice. Mental health week this year passed with barely a mention in mainstream media. As stated, great video, great acting, but a very long way from my experience.

When I was diagnosed with severe episodes of reoccurring major depressive disorder I had gone to see my nurse practitioner, she was asking me stuff and I was smiling and making eye contact at her, I said everything in my life was going wonderful. I had a beautiful family, got good grades, everything was peachy but my brain and emotions made me wish I was dead and that I would wake up in the morning and wish it was bedtime again so I wouldn’t have to suffer throughout the day. 10mg of Lexapro once a day later and I feel great. These are such good learning videos. Depression can disguise itself and not everyone shows the same symptoms. You don’t always see sadness in every patient. Sometimes it’s hidden deep inside.

It saddens me that most GP surgeries now would never allow that length of time for an evaluation/assessment you only see them for about 10 minutes and even after that all they do is refer you to somebody or prescribe you pills and send you on your way. Just because a patient or client is diagnosed with depression does not mean that they will always walk into your office tearfully. They can be talkative and engaging, but when home, perhaps shit hits the fan. My sister is bipolar, leaning on the side of depression (bipolar people tend to lean one way more than the other), but it's not like she's gonna walk in the counselor's office ready to cry or yell. What if she happens to be happy? Christ. But she still has underlying problems.

Ashley White I used to think that as well, but when you start talking, the tears just keep on coming, uncontrollably. At least that was the case for me.

While I think this is brilliant, I do take some issue with it, namely: What if the you Are severely depressed but do not *appear to be at all depressed? What about the patients who are engaging and talkative While still being chronically depressed?

FloorBoardWorld I wasnt diagnosed with depression until I tried taking My life. I am a bubbly person who talks alot- Which is sad that i couldnt get help early.

That's why it's important to have LEGIT therapists who are more clairvoyant and can read people. Too many get into this field for the money, as evidenced by how many comments we have saying the exact same thing you did. A legit therapist will see through your bullshit and call you out on it. I found one, and I hope you do too.

Odd that she inspires this qualifier. Not competent, effective... but adorable.
