I've actually never thought of that, but I just tried it on my Analogue Pocket (which is the easiest method since you can load any save state into any game). And you might have suspected it, but you'll fight a black Mr Dream. His profile pic looks quite good, but the faces he makes during the fight are a bit scary. He's called Mike Tyson, but during the game intro, his name is kinda obscured by the text "©1990 Nintendo". And unlike this video, the copyright year doesn't change during the title screen. It still says "1987" Guess I'll have to make another video this weekend.

OK, I uploaded the video.

It's not a romhack, it's a glitch that happens when you load a savestate from the Mike Tyson version to the Mr Dream version. But I don't know if it will work on any emulator, at least it doesn't work in the Mesen emulator, it only works on the Mister FPGA and the Analogue Pocket.
