I was introduced to these diagrams recently in a Workplace Health and Safety course. The provided info left a lot to be desired in explaining how to use the diagram. This is the only video I have come across that spells out that the 'bones' are there to list items for further investigation, not a list of proven factors. Thanks very much.

What you start with is a symptom statement and not a problem statement. Ishikawa diagram helps you with a root cause analysis which leads to the identification of the problem. Ishkawa said often that a key skill to acquire is to be able to link a symptom to the cause. ... aka problem definition
