I recon you’ve displaced you AC joint in your shoulder. Have a look into AC rehab and stabilisation. I came off my bike in july and did the same injury, 6 weeks later I did an ironman so i think you’re looking at 6-8 weeks of rehab for this

20 minute FTP test first needs to be multiplied by 95% so 400w @ 90kg would be 4.22 w/kg as it's the theoretical power for an hour not 20 minutes.

Thats a lot of Bioscience right there. You can evolutionary not be so lean, that you would not have enough fat storage to sustain a hard training over several hours. Fully recovered you have glycogen storage (blood, liver, muscle) up to 800g - x4 kcal is 3200 kcal. That's a lot of work, considered, that you never run only on carbs, there is always both fat and carbs running.
