Thank you for your excellent lecture, very helpful.As a pediatrician, we rarely adopt the AC mode, since babies tend to be tachypnea, AC mode may cause big problems. AC mode allows patients to trigger a gas delivery at any time and give them a full assistance: fixed iT, fixed VT...which may cause total iT longer than eT leading to gas trapping even pneumothorax. SIMV mode does good, however, it can also make a conscious patient very uncomfortable, patient can trigger the ventilator, the ventilator can trigger patient’s spontaneous breathing either, that’s Hering reflex, once it happens, the patients will be struggling

Thank you, Dr. Prajapati. Essentially, when in NIV-PC the set I time is active on all breaths. During NIV S/T mode, the patient controls the I time during spontaneous breathing, and the set I time is only active on mandatory breaths.

Hello, Reanna! IMV allows for spontaneous breathing between mandatory breaths, but does not synchronize with spontaneous breaths. This may lead to breath stacking. SIMV does synchronize. If the patient initiates a breath close to a time triggered mandatory breath then the vent will synchronize the mandatory breath with the patient's spontaneous efforts with the intentions of avoiding breath stacking. Hello quick question, at 26:34 under the pressure waveform, if the patient initiates a breath at 3 seconds would the ventilator synchronize at that time too? or is it right before the set RR at 7seconds?

If a patient is put on VC-AC with set R/R of 10 /min n patient triggers breath after every 3 seconds n assissted breaths got delivered so my query is will the ventilator be delivering the preset manadatory breaths at all during the whole minute .

I'm really not sure. Some think that SIMV taxes the patient's respiratory muscles during a time when full mechanical ventilation is appropriate. I think it's more of a balancing act on a per patient basis that should be considered. Even ARDSNet protocol doesn't specify a mode, so I'm really not sure. I finally understand the difference between AC and SIMV after watching this video, even though I read a couple books on ventilators! Thank you sooooooo much! I was always confused what would happen if patient triggers a spon breath when the machine is about to deliver a breath. Still one question: if patient Just triggers a breath before machine breath, for AC you get set assisted volume on top of patients own breath voume, which is why it causes breath stacking, But for SIMV, it synchronize with patient own breath, then how much tidal volume patent gets eventually?cos I notice for SIMV, for example we set up TV at 500ml, if patient happens to breath Just before machine breath, they eventually get more than 500ml, it is not around 500ml, sometimes it could be 600ml quite a big difference, it makes me wonder that might be a risk of breath stacking As well, But my educator says SIMV Does not cause breath stacking.

CAn you go over mechanical ventilation in cases like DKA 'severe metaboluc acidosis with huge minute ventilation. These pt's may have 40 litters of minute ventilation ventilation. Lets say guy is 80 kg Can i put him on SIMV MODE IF SO WHAT KIND OF SETTING.

Hello. In AC-VC, you set volume and flow, therefore the only thing that will vary is your pressure. You want to watch for a dip in your pressure waveform. Your pressure volume loop will also have an S appearance. Hope this helps!!

HI and thanks for commenting with your question. Everyone wants to know which mode to use for which pathology. Modes of ventilation are not specific to pathologies. The answer is which mode is best for this patient, with this pathology, at this time!

In theory yes! SIMV will allow the patient to generate their desired tidal volume instead of being controlled by the vent.
