Let's talk rib fractures!.
First, some anatomy.
We have 12 pairs of ribs that make up our rib cage. They originate on the bones of thoracic spine and attach to our sternum (10/12 do, the other 2 float, meaning they originate on the vertebrae and don't attach anywhere).
The primary role is to protect our organs (liver, heart, lungs, pancreas, spleen, stomach).
The main way to fracture your ribs is by blunt trauma (fall from a height or a motor vehicle collision) or by a direct blow.
The biggest concern is the possibility of a flail chest, which creates an unstable chest wall due to the type and amount of ribs fractures sustained and how the injury affects the integrity of the chest wall.
Other concerns can include: Damage to vital organs, bleeding and lung conditions such as pneumonia and pneumothoraces (collapsed lung(s))
How do simple rib fractures heal?
Time. No surgery performed.
How do you prevent secondary complications (as a result of lying down in bed as a result of pain), like pneumonias, blood clots, pressure sores and deconditioning?
1. Deep breathing and splinted coughing or huffing (Active Cycle Breathing Technique)
2. Mobility (Walking, exercising gently)
3. Positioning (Lying on your stomach, sitting or standing)
What shouldn't you do?
1.Tape/use tensor bandages to help alleviate pain.
2. Lie down in bed all day until symptoms resolve.
Update: When I discussed V/Q ratio (Ventilation/Perfusion), I didn't explain it clearly. V/Q ratio basically has to do with how much air is entering your respiratory tract, relative to how much litres of blood are going through your capillaries per minute (Normal is 0.8, 4L of air and 5 L of blood, 4:5). When you have something like a PE (pulmonary embolism), there is now a mismatch in the VQ ratio, where there is more air entering the lungs, but less blood entering the capillaries, thus a higher V/Q ratio, which is can lead to serious complications, such as death.

Broke 2 ribs last week Fishing in my waders at night ,slipped on rock. Immediately came to this video. Theirs nothing you can do about it, he said. HE IS CORRECT Day 7 is great compared to day 2-4 Sleeping on back and opposite side Getting out of bed is scary and takes mental prep. Any questions feel free to ask. 😢

Hello there! Hang in there! It seems like you’re on the right track. Look into getting a bed rail (you can put under your mattress), if the pain is really bad. Otherwise, use the log roll technique to help. Keep up the good work!

@@luminousa5090 I would chat with your family physician about that since that’s outside of my scope. There is some research about the use of lidocaine (a local anesthetic) to help with pain after a rib fracture but not sure about the use of anti inflammatory creams such as rub A535. Talk to GP. I fell off of my bike, broke two ribs on the right seven ribs on the left cracked my scapula and collapse both my lungs. It’s now been five weeks and I’m taking it one day at a time. Your recommendations were excellent did not know I should be laying on my stomach rather than my back. Looking forward to doing that tonight. Thank you again.

Hi Doug, thanks for sharing your story and for your kind work. It’s not necessarily that you should, rather it helps from a lung health perspective and can help decrease risk of lung complications compared to on your back. However, if you’re a back sleeper and sleep better on your back vs stomach and you’re getting up throughout the day and staying active; then this would generally be sufficient to limit these possible concerns. It’s been 6 days since I broke 2 ribs. Doc told me some basics but I wish I could’ve seen this video sooner!! Especially the trick with rolling onto your side and tucking your legs in!

I am a healthy 68 yr younger lad women and fell 15 stairs to the bottom hitting every step of the basement while 9:14 9:14 visiting out of state. I can’t get on a plane, 😎

I didn't go to the doctor because I've always been told they can't do anything. .. I don't know if I broke em or not .. I do know whatever I went through with swan diving ..tripping on a computer case onto tube TV.. the corner of it . Do not recommend.

More so to go to the doctor if you have any other symptoms other than pain to rule out any other injuries. Fractures can occur in a multitude of ways and how many fractures can play a role if something more may be needed to treat other than rest! Hang in there. -Aleks

Aleks physio AP it is very helpful, but, i hate when people ask to like their channe,, but, this time , i did liie it, because i fell my ribs hurt, i don't know if i might gave a fracture or what they compressed,just bruised or.... Do think swimming is not good.?

@@cinmac3 you should see your GP if you believe there may be a fracture. It would be hard for me to know without the appropriate imaging and testing being done. Swimming may provoke symptoms as well based on the arm movements but would depend based on types of strokes being performed and modifications possible with the swimming and how intense the symptoms are. Hope this helps! PS: I ask people to like if they found value because that’s how the RU-vid algorithm works unfortunately. So glad I found this. 60 yr old female. I slipped stepping into our cockpit after washing down the boat and landed (luckily) on my side on the fiberglass seating/bench- I had tried to catch myself with my right hand and it was enough to turn me so that I didn't land on my spine. I also missed our 10" winch with my head by inches so all in all a couple of broken ribs was very lucky. I have been doing all of the breathing exercises as prescribed and staying out of bed as getting out of our bunk is very awkward with 2.5' headspace but I've figured it out. I'm staying mobile but avoiding certain actions like reaching forward, lifting heavy objects and bending over - I leave that to hubby for now. I'm 3weeks in and tried yesterday to lengthen the time between my advil/tylenol combo to 12hrs, initially, it felt not too bad but by 10pm I was in a lot of pain. Should I just try and power through? Or should I continue with my 8 hr schedule and try and stretch it out in another week? Also...6weeks to 3 months?! Ugh! Our cruising summer is over with this, but I was hoping to start up my yoga by Sept. I will push myself harder and do more walking off the boat, though. When can I return to light weight training? Thanks for any advice you have. Oh...I broke the 10th, which was almost a complete break, and the 11th was a simple fracture.

As with all my videos, remember this is just educational advice. I cannot comment specifically on your condition as you are not one of my patients. Best bet is to go see your GP, especially regarding the medication questions as there are important aspects to consider about your general health before even giving general advice on medication. ▪️ Yea the icy stairs got me a few days back... Had to leave the job and am taking the following 3 days off. If I don't feel better, I will have to miss more work and go spend more cash on X-rays and probably more. All because there wasn't salt on the stairs... All it took was some salt or gravel to save all this time and money I will be losing. I'm going to get a massage later today. Hopefully it won't worsen it. I highly doubt the ribs are busted. Pretty sure they're just bruised. 🟥

Thanks for vid ,mon broke some ribs blunt force trauma,, doing exercises as suggested ,so limits future complications, went to a&e and left after sitting for 5 hrs waiting hurts ,stats a aok at mo ,will go see my GP asap to make sure ok ,found aloe Vera laying in a zen chair in the sun really helped ,

@Stay Tight Hmmmm never heard of that response. But generally, that increase in intraabdominal pressure has a tendency to reproduce symptoms. Use a pillow if needing to cough or generate more inteabdominal pressure. Aleks

Injury happened last Fri night...I havent gone into hospital as just self assessed how to treat. No improvement as yet but icing and no clear visible bruising.I probably give it few weeks and reassess if need xrays. No issues sleeping

Great advice. I'm in Day 5 from my injury. Sounds like tracking perfusion index with a pulse ox may be a good idea? Mine was 1.4 on day1, but now is up to 14.

Hi Nebby, generally it isn’t something that is commonly tracked. However, something that may be worthwhile to track is your SpO2 using a pulse oximeter to measure the percent oxygen in your blood. Getting in and out of bed is pure agony. The banks for the tips. Broke a rib last Sunday and didn’t hurt so badly at first, but after 6 days holy cow it hurts!

Yes getting out of bed can be quite challenging. I have a video where I talk about getting out of bed specifically. Hang in there it gets easier. Play around and see what works and what doesn’t in terms of positioning and mechanics. Thanks for sharing! -Aleks

Thank you. I coughed hard last night and it’s possible I did something…heard a pop and it felt like someone stabbed me in my left side. It really hurts, 5 hours later, to walk, but I can breathe deeply and move if I’m careful. I might go into the ER tomorrow; hoping maybe just a bruise or something minor.

It would be hard to know. Generally with a healthy individual a cough wouldn’t be sufficient to cause a fracture, unless you have an underlying condition that can make bones weaker such as osteoporosis or other diagnosed or undiagnosed condition. But yes, see how it feels and seek appropriate medical attention as required.

Hi Carmen! Good question. The premise is the same though regardless if it’s broken from the front or back. The ribs loop around and originate around your spine and attach to your sternum (first 7 directly onto it). Good point though!-Aleks

I broke two lower ribs snowboarding in Feb of this year. I am still in pain 7 months later. Not in near as much, but most of my pain stems from scar tissue on my intercostal muscles. Some days are fine, some days are achy and beyond uncomfortable. My doctor told me that he broke a couple ribs in a motorcycle accident five years ago, and that he still suffers from periodic pain. I have broken many bones in my life and ribs are by far the most uncomfortable and inconvenient fractures I’ve ever had.

Pain is so complex and so many factors playing a role that even though you may have healed physiologically, doesn’t mean that you automatically will have no pain. @@aleks_physio thanks for the personal response! That’s cool. I have my stretches and I do them daily. Unfortunately for me, my job title is in my display name and I was never able to adequately rest and recover. I never stopped working, and some of the favoring I was doing to alleviate the pain, ended up causing muscular compensations that have realistically prolonged and moved the area of pain to different muscle groups. I’m getting through it, and month over month, I’ve been steadily getting better. I can get out of bed easily, I can twist, and I can cough. Those right there were big victories.

Great information Aleks. Thank you! 3 days ago I fell on a hard ground landed on my left hip, side, and knees. Hurts to laugh, cough, move, walk etc. I don’t think I broke or badly bruised my false ribs. I’ve been icing my left side 3 times a day for 20 minutes, it’s black and blue around my hip bone. I have no fever, breathing okay, just feeling pressure. Yesterday I noticed my left side is swollen. Any recommendations would be greatly appreciated.

Hello, thanks for your message. Unfortunate to hear about the fall (it happens more often than you think, as I work on the trauma floor and we see it all too often). Do you know for sure if it’s broken? Did you seek medical attention? (CT or X-ray?). It’s hard for me to give medical advice as I of course haven’t done a full assessment on you just know what you’ve gone through based on what you told me. Best recommendations is to see your family physician or doctor and get imaging if you think it’s broken. Sometimes breaks vs contusions (bruises) can present similarly, especially in the first few days. Imaging is good to rule out anything else going on. From there, if all cleared, then it would be good to see a PT in the community to work on some mobility, exercises, etc. hope this helps and all the best!

It has been two weeks since I fell and bruised my ribs and cracked three ribs. I have broke bones before and this is the worst pain I have ever had and I have a herniated disc. I do not feel better. There are times when I feel 7 or 8 out of 10 but only if I stay in a sweet spot. The rest of the time it is 10 + pain. I had a CT scan and I am taking Tylenol and Advil and they don't work well. I can't work because the position causes a lot of pain. There are spasms. I had to push a rib back into place several times, but it is finally staying in place. I wouldn't wish this on my worst enemy. I can't sleep unless I crash and usually only able to sleep for 2 hours. Thanks for the tips.

Yes rib fractures are said to be one of the most painful musculoskeletal injuries you can have because of the fact that you can’t immobilize your ribs like you can other parts and as such are always using them to some extent. Don’t move much until after 2-3 weeks when calluses form to hold fractures together. I learned from experience.

It all just depends! But bed rest is definitely a no go. We get patients up at the hospital on the day of admission after rib fractures and everyone’s tolerance will be different!

More regurgitation....I just fractured 2 ribs and I used a compression bandage across my chest and 1) I found much better pain relief, and 2) I can breath as deep or deeper with it on than I could without it, probably because its helping the ribs from over extending causing the pain, and because I am active and weight train on a regular basis, I have no issues with my breathing. I also have an oxygen monitor (one that goes on your finger), and its around 93-95 so it seems to me I am not compromising my breathing. I think this was (again) done because the minority of people with this type of injury who probably had their breathing compromised in the first place, were told that and then recommended not to wear a compression bandage, however I disagree that it should apply to everyone!.

Hi John, thanks for sharing your story. Everything that I state here can be context dependent, I just try to state what is the general consensus and recommendations and try to use research to back the claims up. This is why I state not medical advice and to speak with your healthcare provider. For example, I don’t use tens (electrical stim) to help with pain after a musculoskeletal injury because the research is quite sparse and there are other more beneficial uses with your time with more of an active based approach vs passive (in my opinion). I agree. 7 fractures. Right side like mash potatoes. They send me home without any support to the area. My dr did house call brought me a cloth that form fitted to body and hardened like cast after 20 minutes. Held it on with velcro belly wrap. Loosened every hour to practice full breathing. After a few weeks I bought full velcro back brace. Still wearing it for car travel. 3 months out, still have 3 displaced one of those are non union. Today gojng to try knisiology tape as daily movements seens yo be hjndering callus forming on non union fracture according to recent xray and rib movement I can feel. @Aleks

I've been dealing with a popping pain in the back of my rib on the left since the end of Jan by completely over doing it. The pain spreads to my chest and upper belly. Very painful, any stretching makes it worse as does coughing or breathing in sometimes. X-rays nothing came up, waiting on ultrasound results. I've been to many practitioners since and have tried just light walks, haven't really rested it much and it's definitely gotten much worse. Wondering if I just need to let it rest and stop doing everything to let it heal? Haven't had much luck with practitioners so far with a diagnosis.

Hello there, it would be difficult to know without doing a formal assessment and knowing your history. Generally, an X-ray wouldn’t be able to pick up something similar to what you’re describing. I would be curious on what the ultrasound results state. Regardless, I would generally avoid the notion of bed rest or limiting exercise completely for the other detrimental effects that it can have on your health. Going for light walks shouldn’t be contraindicated based on how you are describing. However, of course this would be difficult for me to know. Pain is so complex and relies on many different factors and aspects and can be entirely person dependent. Wait for the ultrasound results and go from there. See a PT also to see if there’s some opportunity for improvement conservatively. Hope this helps! - Cheers, Aleks

Hey Alleks, in day 8 of linear fracture of two ribs and returned to work. Nothing stressful but driving is necessary. I did fine until late in the day and a focused pain below the right shoulder blade appeared. Ice seem to make it worse and heat made it feel better. could this be a muscle spasm and if so is that common in ribfracture healing. Everyone says use ice but that made it worse.

Hi Arthur, it could be. This area has lots of various tissues such as tendons, ligaments, joints, and of course the bone itself. Your body is going through a natural recovery cycle which includes mounting an inflammatory response at the beginning. Also, muscles can be generally stiffer in the region and it may be because you’re generally not using them as often due to the pain and healing taking place. Regarding ice or heat, if you find that heat helps and you’re following safe heat hygiene (no more than 15 min consecutively, good skin sensation, checking temperature before and putting a thing layer between your skin and the heat pack), then it may be worthwhile to continue as heat can increase blood flow and promote healing and may even provide some benefit for the muscle spasms. Thanks for this video. I'm on day 6 of a rib injury. I fell on a scaffold peg. I had x-rays and the doctor said there's no clear break but that there could be a crack or cracks but it's hard to see on the x-ray. I'm definitely sore but I can manage the pain okay. Is it safe to go back to light duty construction work with a possible cracked rib? I don't want to prolong the recovery time but I also have a lot of work to keep up with and I don't want to fall behind if I don't have to.

Hi Dallas, sorry to hear about that. It happens more often than you think unfortunately with the falls. It’s tough for me to say without performing a full assessment. Light duties definition can vary from person to person. 6 days is pretty early as well from the injury, but again a lot of it depends on how you’re feeling. I would say definitely no ladders or working from heights as well as lifting weights. Usually rib injuries can take up to a few weeks to 3 months to fully recover. Hi, Same thing to me, they did an xray and didnt see anything. When they did the ct scan, they found out that i have 9th and 10th rib fracture. I am in my 2nd and half week and it gets better everyday though the pain is still there. Just make sure to take pain reliever, eat healthy and do not lift heavy stuff. Hope you will get better soon as well.

Riding the bus to work, I bruised my ribs on the city bus on March 19. I was almost to the back of the bus and as soon as I turned to sit down, the driver accelerates and I was thrown hard, the back of my ribs slammed into the top edge of the seats. Knocked the wind out of me and it took a few seconds to start breathing. Massive pain. No breaks or fractures, but I had yellow bruising on my right side. I still couldn't move when I went to bed as of last night, had to lay on my back. I didn't go to a doctor, I'm just taking doses of Aleve and extra strength Tylenol. Today I was finally able to get out of bed without spending 10 minutes trying to position myself without huge pain. Pain is starting to subside today, finally. My ribs still feel like they grind and click on my right side. Is there a treatment for this or will it go away by itself? I'm 54 and this is the first rib injury I've ever had.

Hi Ricardo, thank you for sharing your story. Sorry to hear about this. Did you have X-rays completed to confirm no fractures or did you assume? (I’m wondering because you stated you didn’t go to a doctor). Bruising is quite common. (Can be yellow blue or purple or a combination). I would first confirm with your physician if there was a fracture and then chat with them about anything else that they can provide for pain control. I know some may prescribe a medication called lidocaine which can help after rib fractures (it’s a local anesthetic actually, for pain control). Otherwise, best to follow up with your family physician to rule out anything else if you are having any other symptoms other than pain. If everything is cleared, then following some of the education pieces and recommendations discussed in the video is a good start. Keep me updated. Cheers and hoping for a speedy recovery. 🙂

My best advice is to go to the hospital or see your family physician. The urgency will depend on any other symptoms that you are feeling associated with this (black stool, shortness of breath, pale, dizzy, nausea are more severe signs to be mindful of). They would order any appropriate imaging (x-ray, CT, etc to figure out what's going on. The amount of bruising does not correlate with how much injury there is, so don't assume that because there is no bruising that it's not broken or that there is no further injury. Hope this helps. Hallo Doctor, that’s amazing meaningful clip. I have 11th day broken ribs, 8-9. More at the behind- right side . Lucky I did from beginning as your recommend 1-2, but the 3rd sleeping position I slept more on the side, and now I tried with stomach position and feel so good. Great. I feel less pain, should I continue taking Ibuprofen 400 (1-0-1) or should I stop? I will see my doctor in 1 week so it’s nice if you can share your experience please. Hello, thank you for your kind words and glad to hear things are going well. Hard for me to say for the specific medication requirements. Ibuprofen is an anti inflammatory medication meant to decrease inflammation and if its working then thats great. I would say the best thing to do would be to chat with your family doctor and take the medication as prescribed so as to not confuse. I've watched a lot of videos on the subject since fracturing six ribs playing hockey. This was the best I've come across. My accident was 10 weeks ago. I was feeling pretty good so thought I'd hit golf balls. The golf swing and impact did not hurt but I've been really hurting at home since then. Wondering if I 'un-did' the healing with this and I'm back to second base or is there some kind of muscular association with the fractures and this is normal discomfort on the return to activity. One rib was displaced but I'm feeling pain in a couple areas of my chest and back. I'm 71 and don't have a lot of golf seasons left, so eager to return. Thanks for any input.

Hi Don, thank you for the kind words. It would be difficult for me to give you any medical advice as I don’t know anything of your story other than the basics. It would be best to chat with your health care provider and if you’re really concerned to have imaging performed. However, it is important to note that golfing invokes quite a bit of rotation around the thoracic spine. Where do the ribs originate? On the thoracic spine. There are muscles that connect to the thoracic spine as well that may have been irritated with that rotation, especially if you haven’t been doing much in the 10 weeks leading up to playing golf (from a return to sport perspective). Are you crazy. Of course, you should not have golfed yet! You are older than me. Your body takes longer to heal. You have less stem cells. I had no healing after one month. Second month, I used vit d vit k calcium bone and tissue supplement s comfrey in homeopathic form. Finally, second xray I had callouses forming. 3 months out I still have 3 displaced fractures. One is non union. I moved too much first month.

Hi Jojojeep1, thanks for the feedback and glad you found value. There are actually a bunch of nerves in that region where your ribs lie, which likely would’ve been irritated during the break, hence the burning sensation and radiating pain you may be experiencing, likely. Nerves do take a bit longer to heal than bones and muscle injuries, generally and sometimes the sensation does not fully return, depending on the level of nerve injury sustained. Just be patient and see your GP if it continues to be a concern. There are other interventions that can help with nerve pain (shooting, stabbing, burning sensations, generally). Hope this helps! 🙂. Grat video thanks. I had a big fall almost 4 weeks ago. After a radiography and I was told that there was no big fracture, but they could not discard micro fracture. I am much better now, but still having a bit of pain especially going in and out of bed. My question is how do I know I am recovered and I can resume sports such as running or climbing? Hi Rafael, thank you for the comment and questions. First of all, happy to hear about the recovery in 4 weeks. That’s a good sign that things are coming along well based on typical recovery schedules. Generally to know this question is to understand typical timelines which can range from a few weeks to up to 3 months (and sometimes longer depending on general health, age, etc). In addition, recovery is a loose term that is different depending on who it is. Are we talking about a 25 year old who wants to get back into pro climbing or a 50 year old who just wants to get out of bed without symptoms. I think seeing a Physio to go over a return to climbing and running program would be best so they can follow up with you if you have flare ups or are progressing well to change your program and at the same time give you things to work on in the interim. Generally, wouldn’t recommend just getting right back into it especially if you haven’t done much over the past 4 weeks. See a PT. Not medical advice. Cheers and good luck! Regards, Aleks

Hi Max, this is a great question and a hotly debated topic for all all injuries and post recovery. GENERALLY, ice appears to help immediately after due to its numbing effects for pain control, however, after the first several days, once pain has started to subside, heat generally assists with improving blood flow/tissue elasticity, which can help with recovery, mobility healing. But lots of caveats to this. Not medical advice of course. Some can’t tolerate heat or ice due to other medical concerns, so see a PT or GP just to make sure you’re ok. For example, if you have issues with sensation at baseline, applying either would be difficult as if it’s too cold or heat there is a risk of freeze burn (with ice) or burn (with heat). Hello Toastopia1, yeah that is fine (if you can get a good nights rest in that position, I envy you haha). I generally would tell people to sleep on a recliner chair as it's generally more comfortable and easier to get out of than a flat bed. Hi Nasir, thanks for the kind message. The broken collar bone would need to be treated separately. Of course this would affect your shoulder range of motion and your ability to put weight through the right arm (usually if your collar bone is broken you can’t put any weight on it for at least a few weeks). Chat with your doctor to see what restrictions you have and what range of motion you’re allowed and strength exercises as each collarbone injury is treated differently based on the type of fracture, where, surgery or no, etc. As for the rib fractures the same principles apply, except unfortunately you just got them all at once 😞. Punctured lungs as a result of the rib fracture puncturing them or a blunt object? Best thing is to talk with your doctor as of course I don’t have the specifics. I see improvement but I don’t think there is any silver bullets rather with time to heal. the sharpest pain at times ( ribs area) unbearable but with medication it’s manageable. Or with walk it triggers the pain at times ( not all the time) . Again much appreciated your time and the video. It’s very helpful.

I fractured my rib on the left side under my left breast seven days ago. It is so painful. I did have an x-ray to confirm the fracture. When I cough, deep breathe, move, it hurts. To hiccup is intensely painful. In the last 12 hours i have started feeling a catching or snapping feeling that happens when I hiccup. In watching videos I have not heard anyone mention this symptom. Do you have an explanation?

Generally a snapping sensation could be associated with the soft tissue structures in the area moving over the bony attachment (such as a tendon of the breathing muscles). It could also be the movement of your rib cage with the hiccup could create a crepitus of the rib fracture region. My best guess. See your GP if it continues to bother you. Hope this helps. I broke 3+ ribs 3 weeks ago. Front 456 & #2 back #4. been brutal. I caught a cold and have a violent caught. Which is causing me to hack up Blood in my phlegm. My question is I've been to urgent care rt after injury they sent me home with a xray saying everything looks fine. 10 days later i went to the ER and the Cat scans showed multiple broken ribs. I really don't feel like anyone is giving me the information i need. Is this blood a sign of healing or infection? It only comes up.when I'm violently coughing

Hi Joey, unfortunate to hear about this. Anytime someone is coughing up blood, we try to look into any additional questions that warrant further investigations. Shortness of breath? Chills? Night sweats? How much blood? What’s the trend (more, less or same?) Nausea/dizziness? Changes in skin colour or vital signs (blood pressure or heart rate changes, as well as SpO2). Obviously I’m not your healthcare provider but these would be the questions to ask yourself. A chest infection can be one of the differential diagnosis (possible causes), which may warrant different treatment versus if it was just a rib fracture. I dont know if i have a broken rib, but i just had symptoms overnight. I had chest pain while breathing in, chest pain with moving my body, and some shortness of breath. I didnt fall or do any physical damage, but i do sometimes accidentally hold in my sneeze. Are these signs of a broken rib?

Unlikely in the absence of any trauma or specific mechanism of injury associated with this. If you had severe osteoporosis then a sneeze can fracture a rib but it seems like this occurred insidiously. Chest pain and SOB can be due to a number of different reasons. The first days i felt pain when breathing, but now after a week since it happened it doesn’t hurt that much. Hello, it’s tough to say because there are so many factors that play a role. Great that you seem to be coming along quite well. Generally, use pain as a guide but at the same time you don’t want to come too early, have a fall and then have to restart the healing especially if you start on that side. See a PT. Build up your tolerance and slowly ease into it. There are levels to return to sport (graded exposure or load management principles). Generally not recommended to go 100% after an injury if you haven’t done anything before to slowly build up your strength, function, mobility, sport-related movements etc. Not medical advice. Hello, that’s unfortunate to hear. It would be hard for me to know as well until doing a thorough assessment. Have you done any imaging? Has it gotten any better? Where do you feel the protrusion? There is a video I have that I posted on the “slipping” rib phenomenon that may be worth checking out, however, I wouldn’t be able to know if that’s what the cause is. From a physiological perspective, most rib fractures heal on their own within 3 months. However, pain can sometimes continue to persist well beyond the actual healing. As pain depends on other factors and is more nuanced. If it’s not healing, then follow up imaging to ensure there’s no mal union or non union of the fracture site would be important. If it has healed but still pain, then exercises may help with pain that work on improving mobility, strength around the muscles of the region and breathing exercises. Heat is generally appropriate to help if there is stiffness to improve blood flow. Cold more so for pain. Sometimes it just depends on personal preference. Motion is lotion. Best is to see a PT to help you get on an appropriate rehab journey. Not medical advice. Hope this helps. -Aleks

@aleks_physio thank you kindly! They are healing, as indicated by my 3rd CT scan. I have this lingering small cough every few seconds. I think I aggravated a few nerves as well in my back region. Hi, I have 3-4 ribs fractured on my back (9-11), it happened 3 weeks ago. You said that don't lie on the back, but just what I did so far mostly with half way (or more) up (electric bed); only a few times I did on my stomach, as I had difficulty with getting into and out of bed. The most painful to lay at the side, which I just started a bit pushing myself to do that for a few min at times. What's your opinion? No one mentions the situation when the ribs are broken at the back, which is different when it's at the front. I was basically looking for more breathing exercises and tips for do/don't dos. Thanks

Hi MrsTeatojas, unfortunate to hear that you broke your ribs 😞. The advice I’m talking about is just for educational purposes. Not medical. If it’s too painful to lie on your side or on your stomach, you shouldn’t force yourself into those positions. All I mean with avoid lying on your back is if that’s all you do for most/all of the day. Even if you can sit up/change positions/walk around (if possible) for a few minutes every hour will be more beneficial for you in the long term, likely. The goal is to gradually do more as the fracture continues to heal and as time passes by. As for exercises, in the first several weeks, limiting to just basic mobility (sitting, standing and walking short distances) and deep breathing, huffing, coughing (if tolerated) are a relatively safe bet. Follow up with your GP or trauma physician for more assistance with the medication side or if there is something I could of course be missing as I don’t know the full story and your past medical history. @Aleks Physio Thank you. Yes, I do sit, and being up, and by now I can go outside for a half an hour walk couple of times a day. I just still feel pain if I lay on my side, and on my stomach it's just a bit uncomfortable for long & bit more difficult to get up from that position. Yes, I'm monitored, I simply was just looking for more types of breathing exercises, because fluid keeps building up in my lung.

@@MrsTeatojas if your feet are swelling it may be a good idea to elevate them above your heart when lying down, if possible. In addition, performing ankle pumps (pointing ankles towards and away from you), could help with the swelling as your calf muscle activity helps with circulation of fluid and blood back to the heart. You should discuss medication as well regarding the swelling in your feet. Cheers.

@@kingcarlos5925 Hi, generally rib fractures can take up to 3 months to heal physiologically, under normal conditions (relatively healthy individual with no adverse health conditions or habits that may affect healing such as smoking). In your case, 4 months and not healed? A comminuted fracture is a more complex fracture as the rib (or part of the rib) is broken in smaller pieces, it’s not just a regular clean fracture with one fracture line). That could explain why it’s taking longer. Also, if you have diabetes, significant alcohol intake, poor bone quality to start with, etc, that could also be prolonging healing. It might be healing but not healed. Again, I wouldn’t know without the report. Just keep following up with the family doctor and follow some of the basic principles discussed here and it should help. Hang in there. Unfortunately pain is quite complex and depends on multiple factors. Even though healing can take place physiologically, pain can still be present. Then it becomes more about nervous system sensitivity versus physiological damage. Changes in the nerve firing take place. See a PT. Hope this puts into more perspective. -Aleks

@piyushkumar-me1nb it would be difficult to know how much recovery there can be. Generally, nervous system is more sensitized to stimuli. With chronic pain it’s more about managing symptoms and still living life with the goal of pain being reduced (although difficult to fully eliminate) to allow you to still do all the activities you’ve enjoyed before with rest breaks etc as needed. Not sure if I fractured a rib or it is a intercostal strain? It doesn't hurt when I breathe and I can still run etc without issue. It does hurt when I sit up and twist though...this sucks!

Will do! It was in jiu-jitsu class where we were doing Judo throws. I got tossed and landed funny on my side. I felt it but it didn't bother me much and I finished the class...about 40 minutes. It really just depends. Hard to know. Some can have tenderness long after as the tissue in the region has become sensitized as a result to injury and based on what happened, scar tissue may build up in that region as the soft tissues don’t usually recover physiologically 100% exactly as it was before the injury. I think you missed a few Key Components Due to the Pain we often become "flightfull" of doing something, which you state is getting up etc. This will often impact muscle usage, and then when one does heavier Sports or Activities the Pain will be severe as these muscles have been dormant for a long time. Some Physios give good exercises for upper body rotation to assist during recovery to maintain Muscle flexibility and usage. The Lying down is questionable, if you have specific fractures you lying on your stomach is ludicrously crazy, as gravity pulls your whole Thorax weight onto the Fracture and overstrained Muscles. The amount of excruciating pain that places you in, is unbearable, unless your on Extreme Pain Medication (Modern Day Opioid overdosing). Rather sleep on the Back and Ensure mobility is kept up with less pain Medication and more Anti-Inflammatory (As the inflammation causes the most pain) Nature. A Question about strapping, would it not be beneficial if instead of strapping across the whole ribcage one would strap short to the next functional rib? Like you would do with Fingers? That would still be supportive as you showed the pillow would be in the Coughing Exercise ( Which I think is a waste of time, as your neural network will immediately filter conscious activities that would force pain. Just like your brain would filter a conscious action of sticking your finger in a live socket because you want to become more resistant to electrical currents, we are programmed for self preservation)

Lots of good questions and comments. I could talk about this stuff for hours. Unfortunately, I don’t think many would listen to it. So there can always be missing components depending on who is watching. And the lying on the stomach is to reinforce decreasing pneumonia risk. Someone with several displaced fractures lying on their stomach would be extreme and not recommended versus someone with a tiny fracture who doesn’t want to get out of bed but may still benefit from lying on their stomach (if they can tolerate). Remember, everyone perceives pain differently which will subsequently affect their experience with it. No I don’t think that taping will assist based on how you described because the ribs don’t function like the fingers do. You can’t immobilize your injured rib by putting tape over as you would with a buddy tape system for your fingers from just the anatomy and how the ribs work versus the fingers. Remember, this isn’t medical advice just education. Of course someone can say well I did that tape job you described and it felt better. That’s great! I just try to give general advice. @@aleks_physio hey thanks I got into a fight and I'm an alcoholic unfortunately that was not the reason of the fight but I messed up my knee when I fell down and I think he punched me in the liver it's right under my right breast it happened I think December 1th and I'm in unbearable pain I can't straighten my back and I just noticed I have a few line of bruising I've been icing it and taking Advil I'm gonna go to the hospital I've just been dreading the bad news I'm scared because I'm not able to fully take care of my self so everything is difficult for me it hurts Everytime i move I got a question though if I call an ambulance will I be put into a unit or room right away or are they going to make me wait in the waiting room? And my knee is all messed up what would you recommend I go get a cast? Because I can't straighten my knee thanks for helping people I hope ur channel goes very far

Hey Spencer, darn that sounds like a rough go around. Like I said before, see your GP to make sure they rule out any other associated injuries. Being punched around your abdominal region you always want to look for red flag signs such as blood when urinating or in stool, diarrhea, nausea (as you mentioned you already have), shortness of breath, changes in your vital signs (Heart rate, blood pressure, etc) etc to determine whether this is any damage or bleeding internally. Hi Mr. Alex. My boyfriend is having pain/trouble w his forearm/bicep(?). What I can tell you is when he sends me a pic of him at the gym the little elastic black brace he’s got on starts halfway down the upper arm, over the elbow, to halfway down the forearm. He’s LEAN as sh!t, 81, been exercising his entire life. He mentioned, I’m going to the gym and just doing the lower body (bc his arm still hurts🤷‍♀️). I was a nurse for 20 y. I know it could be arthritic or he hurt it. Got any special exercise he could do?

I am senior lady with four badly fractured ribs from hospital machines falling on me. I had covid and pneumonia which has now gone. Doing quite well with deep breathing now, (i do have COPD ) mobility coming along but pain is still quite prevalent. I Refused opioids, Tylenol only 4 times a day, 7 weeks have passed. Is there anything else I can do? I Need to speed recovery up Spring around the corner. I was still walking/running 3 miles a day before this happened. Thank you.

Hi Lady Michelle, sorry to hear of your unfortunate fall. At the hospital, we see falls all too often and a lot of fractures as a result. Try some of these exercises that I talked about in the video (deep breathing/supported coughing, huffing and general mobility, gradually building up as time passes. Pain medications would help, but of course you would need to speak to a physician regarding this. If you’re not progressing along the anticipated pathway, it might be worthwhile talking to your GP to see if there is something hindering the recovery. Hope this helps! Cheers 🙂

There is some research that supports the use of Lidocaine for post rib fractures and we see that in the hospital, which can help. But again, risks and benefits to all different treatments. Hey Aleks, I have a displaced rib fracture (only one fracture, 9th rib, lateral, slightly anterior). It's been 9 weeks and it's still not healed, I'm 24yrs old very healthy, Got another xray yesterday and the rib is still displaced and not healed. My symptoms are very similar to when I first broke it. I am HIGHLY considering KT taping the rib or using a rib belt to wrap and risking pneumonia/complications because otherwise I fear that it will not heal. Additional info: The displacement is rather minor but I feel that the intercostal muscles are relatively weak. I have been keeping a slightly kyphotic posture (flexion of ribcage/thoracic) because it mostly alleviates the pain. I thought this should promote healing, reduce likelihood of reinjury. I'm also considering getting the rib surgically stabilized. Thank you!

Hi Austin, thank you for reaching out and great questions and observations you noted. As always, I cannot provide specific medical advice as everyone’s situation is different and I don’t know the whole story, etc. Just education only. With that being said, have you had the discussion with your GP about a surgery? That would be best to discuss with them. 9 weeks is still within the timeframe of physiological healing (12 weeks). Has there been any signs of healing or improvement from the date you had the injury till now? Sometimes when a rib fractures or is displaced (which I assume is also a fracture, unless it dislocated from where it attaches, which generally is not common) there is a risk of nonunion (doesn’t heal, no callous formation to bridge the gap between two ends of the fracture) or malunion (heals but not in the appropriate alignment). The risk is higher with a history of diabetes, smoking, etc due to poor blood flow to the region. Talk to your doctor about this as again, I wouldn’t know without knowing everything. Rib taping wouldn’t decrease the risk of pneumonia. It may do the opposite as you’re limiting the ability for the chest wall and subsequent ribs to expand. See if PT is right for you to help build up the strength in these muscles around your chest wall which will also improve lung health and chest mobility, especially if you haven’t been doing much for fear of exacerbation. 100%! I'm a very active person. Dealing with 8 and 11 right rib fractures. I've broken several bones from extreme sports. Rib fractures take the cake! You can't isolate the bones like a cast. This guy sounds like he actually had broken rids! Please take care...of anyone that has broken ribs! It's EXTREMELY debilitating!!

@@aleks_physio It is up to 100%. I have noticed more than quite a few of your vids have a whisper-volume. I do not have this problem with any other sites.

@@ilonabaier6042 it is quite trickier than you think. I’ve used mics as well and sometimes it’s gets too staticky. So I’m using the microphone on the iPhone. Sometimes I talk in a lower voice but that’s just me. I should look into a professional microphone in the future, but I’m still early on in my journey.

@@aleks_physio by all means get other opinions. I know how much effort it takes to make a video and if people don't watch it because they have to strain to hear it can be a heartbreaking expernience for the producer. Good mics that you can pin on your shirt pocke for example are very reasonably priced.

Ma’am I work in the hospital. I’ve worked with hundreds of people with rib fractures. I don’t need to have a history of rib fractures to know what to do to help. It’s like saying you need to have/had cancer to be an oncologist. As I mentioned before, it all just depends, some people won’t be able to tolerate huffing/coughing and some will. Everybody is different. Pain is a subjective experience that depends on multiple factors and we try to tailor the exercises based on people’s tolerances.
