Not exactly. He was struggling with opioid withdrawal and residual depression from ceasing opioid use. Dr Now should not have taken Robert off opioids so quickly. He should have been put on suboxone or methadone and slowly began tapering over a period of months or even years. Opioid addiction is not something that can be fixed overnight, a week, a month or even a year for many people. People use opiates to help deal with their depression and anxiety. The opioids help Robert cope emotionally with life. The opioids were keeping him alive

That happens with addicts. It gets old dealing with addicts who don’t want to do anything to get better. I’m sure all those people, at one point or another, were there for him, but got sick of his crap!

His mom was on the way to visit him to lift his spirit… she arrived just a few hours after he died…

Because he needed to help himself. People don’t have to surround themselves with this mess if they don’t want to. They come to say goodbye where their words fell upon deaf ears while he was alive. It is not anyones responsibility for his demise. Once you grow up you get a chance to alter your programming. He chose not to. And even if I’m he HAD changed he would still die. You’re gonna die. Everyone is gonna die.

Why should people show up to help you if you won’t help yourself.

Someone from my school lost weight so he could start doing yoga so he was flexible enough to self polish himself

But Dr nows diet is too extreme. 1200 calories is dangerously close to having the body go into starvation mode and the body taking away muscles and other problems and nit burning fat. Also it's so extreme that most would fail because it's losing fat too fast and in turn will screw up metabolism and will gain it back and then some. Losing weight slower will give the body time to adjust to changes.

@@marthacrust334 well thats the only way to drop 40 pounds in a month.. thats why. I dropped 40 in 2 months.. I was 300+ now I'm 269.. its possible. not extreme. if they workout out daily. they wouldn't have that issue.

@@marthacrust334 ok,I'm no doctor but I believe the reason he does it is either because a) the ones that are set on it are 1 step away from death and need to lose it asap or 2) maybe because most foods tend to have more calories than said in their labels? I know restaurants have specific calories on each food but saw many videos where the foods are tested and there's like an extra 100-200. Maybe the doc knows about such stuff in normal foods too? Not sure.

@@heatherwatson9564I think Dr Now had said with how many meds he was on when he came in, he was lucky he wasn’t dead already and between that and his weight and being almost completely bed-bound, his body could give out at any point so they found it imperative to get the meds out of his system. And it was but then he had a surgery that requires pain meds but they gave him a lower dose so he wouldn’t get back into the addiction stuff again

I went through the same thing in 2016 after neck surgery. I had never used drugs of any sort, and wasn't remotely prepared for what I went throught. It felt like my brain was determined to convince me that suicide wasn't just the best option, but the only option.

Opiates are incredible liars. They kill so many people.

Same I was in a bad car accident they gave me tons of painkillers and then cut off not help nothing I'm glad I did get off but most people don't

I never forget his words: "all addictions are pain control", meaning more than physical pain, but emotional, mental, and psychological ... ... and it need not be a 'chemical' based addiction either, meaning either alcohol, or illicit drugs. Gambling causes similar effects, due to the heady rush of dopamine, and other neurotransmitters, especially if you win big ... but, of course, if you lose more than you can afford, then you wind up trying to win back what you've lost, which becomes a positive feedback loop: the more you lose, the more desperate you'll become, and you wind up in a downward spiral ... However ... Physical pain is complex enough, that to say that a patient is 'addicted' to pain killers, may be true, but only in specific circumstances. Having lived with a chronic, acute pain condition (fibromyalgia) for many years now, I can't imagine _not_ having the array of painkillers that my GP, here in England, has prescribed, as he understood, before sudden retirement, that the pain I was explaining to him was very real, and that to even get out of bed in the morning, required specific doseages, even of both codeine and morphine ... So, as much as I respect Dr. Nowzaradan, as Bariatric Surgeon of many decades of experience, really it takes an Anaesthetist/Anaesthesiologist, to understand that not all patients with chronic, acute pain, are addicted to pain medication. Very often, as in my case, before the pain was begun to be stabilised, just rolling over in my sleep, would cause such a large pain response, it would jolt me awake. Just moving to reach for the pain meds I had at the time, was abject agony ... At the same time, however, is how Drs treat chronic, acute, pain. I don't know if there is a difference in training, but here, GPs, and especially Anaesthetists, treating patients with long-term, chronic, acute pain, the aim is to start with the minimum, then work up until _just enough_ ... In America, going to a similar clinic with my fiancé, starting with high-strength opioids, such as oxycodone, at 220mg, sometimes ×4/day is, if your medical insurance can afford it, the go-to, thus creating unintentional addictions, as anything less, feels, to the patient, insufficient pain coverage. In whatever this poor guys case, simply taipering off his pain medication, but then refusing anything stronger than paracetamol/tyanol, ibuprofen, or, if lucky, prescription strength naproxen and/or acetaminophen, is ridiculous. His weight _will_ cause severe pain, as the human skeleton, and supporting muscles, are not supposed to be that weight-bearing, especially that amount of fat as mass ... From speaking with other people, with far worse chronic, acute, pain, than myself, what worked was being admitted into hospital, have their all their pain meds taiper down to the absolute bare minimum, the taipered back up to where there is sufficient pain coverage, without over medicating. At the same time, they told me there is a psychological aspect as well: being in that much pain, without sufficient 'global'² (as in the entire body) coverage, means just the thought of trying to get out of bed, knowing that the second they stand up, the pain will feel like being 'struck by lightning', figuratively speaking, is depressing enough to the point of creating an anxiety situation whereby increased anxiety may actually cause an increase in pain sensitivity. Again, what worked for them is having nurse practitioner support, plus psychological help to overcome the anxiety - including anti-anxiety medication, if required - equates to a decrease in felt pain ... I get that America is in the grip of an opioid crisis, especially more likely, in the same vein as antibiotics, overprescription of the wrong drugs, at to high a doseages, leading to patients saying they require prescription only painkillers, to be viewed as potential addicts ... ... which in many cases, could be true, but, more often than not, again, from what I've been told, is a dismissive attitude of "you _can't_ be in _that_ much pain ...", or, worse "you must be addicted to pain medications ..." ... ... even though the patient may very well be in _that_ much pain ... I hope the Dr. Nowzaradan³ we _don't_ see, understands the triad between weight, pain, and the psychological issues arising from both, in excess; in order to get the patient moving, without a wheelchair, say, but standing on their own two feet, may very well require overcoming that fear, and anxiety - to name but two - plus _just_ enough global pain coverage, so standing upright _isn't_ absolute agony ... [¹: I regret to say that, many years ago, he fell off the wagon, hard, just before, and after, splitting up with a partner he was dating for many years, as she couldn't stand cleaning up the bodily fluids he left behind after a serious bender/drunk. She loved him to the point of not able to live without him, but couldn't live with him, either. Unfortunately, acute liver disease caught up with him, leading to multi-organ failure. He was too ill to be placed on the transplant list. At his funeral, after the service at the crematorium, his partner, and friend of mine, said she wished she could "pull him out of that damn coffin, slap him across the face, and ask what the damn hell he was thinking ..." ... I have to admit, years later, then, as now, I felt, and feel, the same; I lost someone whom was vastly more parental than my dad and mum, if put together, as a singular person, ever was, nor ever could be ... ²: I have no idea if the concept of 'global', as in terms of pain across the entire human body, is an actual medical term, but is one that, in talking with people, online, with chronic, acute, pain, especially with chronic Clinical Depression, and Severe Anxiety Disorder, that is one they used frequently ... ³: naturally, I am, by no means, trying to cast dispersions on his expertise, and experience. The person whom we see on camera may well a slight exaggeration of whom we don't see ... That saying, I have seen my previous GP get pretty damn upset at the treatment I received from one particular Pain Clinic, at a famous London teaching hospital, as they didn't bother to carry out routine physical tests to examine why I was experiencing gradual, sometimes sharp, increases in pain levels, for no apparent reason. They just took an attitude of throw one medication (pregabaline (sp?)), at me, increase the dose, and, as that didn't stick, gabapentine, then also increase the dose. Fortunately, my GP understood that muscular ≠ neuropathic ≠ 'bone' pain, and, each not being the same thing, requires its own treatment, as there is no single magic bullet for the kind of chronic, acute, pain ... Granted, the combination of the medications, plus the combined side-effects, are a mixed blessing: pain-relief, but excessive fatigue, verging on exhaustion, whereby simple tasks can just floor me in fatigue ... ]

Yes, completely agree. The whole anti opiate movement doctors are pressing is giving such a cruel life to many who need it. It’s wonderful to not need them, but when you’re given excruciating daily pain, meditation just doesn’t cut it does it sometimes. And so many with chronic pain are suffering.

@@nigelft I too endure fibromyalgia since 2008 & use strong pain meds for that. Since then, I've developed "arthrosis" bone on bone in my backside. They cut the nerves with medications. I'd be incapacitated without the meds...as bone on bone, is the kind of pain you can't distract yourself from. Then, I developed "restless leg syndrome" again, more meds. My legs are like sticks (I've lost alot of weight) & I can't describe what I endure daily & how I must manage - adapt as best as I am able . My legs won't hold me much of the time. Chronic pain & mobility issues at 65 has reduced my life to just the basics. I don't socialize. I spend my energy caring for feral/,homeless cats... & they keep me up later at night than what I'd like. Sorry this person didn't make it. Being morbidly obese is the opposite of my worries. I've never been underweight in my life- the opposite. Seems we all have a cross to bear. Some more, some less. Condolences to the family

100%. That was a cruel thing to do And I believe ultimately what killed him. If he had a grip on his opioid addiction and pain control he would’ve been able to get up and move much better. Then once he started losing the weight it would be time to taper him down off of the drugs

So my question is though cuz I haven't seen the episode was he prescribed the pain meds? Even if he wasn't I agree I don't think they should have tackled two at once and his weight was far more important at the time as long as they can manage the pain pills. If he was in a facility they could oversee what he was getting. Anybody that's ever been addicted to an opioid tell you that rebound pain when you come off of them is really bad. Everything hurts. Depression sets in. This man was morbidly obese so the pain he was experiencing was going to be a lot worse. Compounded by the fact that he was immobile. This was a bad call. Another thing if he was prescribed those maybe he's in legitimate f****** pain. I know there are a lot of doctors out there and a lot of people who think the zero pain meds. But I believe you have to look at the quality of life. Someone who would lay in bed with pain after exhausting all other options and would be on the brink of suicide because of pain it's kind of cruel to leave them in pain like that. If they are responsible and not abusing them and taking them as prescribed maybe there is a legitimate pain issue that should be treated.

I'm always shocked when american doctors prescribe very addictive and very strong pain killers for their patients. In my country you only get the strongest pain killers if you are dying from some painful illness 🤔

Bingo. He went downhill because his opioid problem wasn't properly addressed. That's why he suddenly fell into a depression and lost motivation and ambition.

I was thinking the exact same thing when the doctor was explaining his opioid addiction. I've been through that and it's not easy at all. It takes more time than you'd think. Your emotions are all over the place. I don't watch this show but I hope he had a therapist also. When you're withdrawing, you cant sleep, you cant think straight, you're emotional like crazy. Just the addiction part wouldve been so tough to get through, poor guy had to deal with that on top of everything else.

I feel he was mistreated for addiction at the wrong time....detox thru surgery was wrong. How would his bypass be treated for pain? I think that's what killed him. Very sad and unjustified

Methadone is another narcotic. It is not effective with many many psyche meds used for depression or anxiety. The side effects are also difficult and affect many other systemic issues like heart problems. I am pretty sure Dr. Now made accurate decisions based on his entire medical history and withdrawal symptoms. So, please don’t be so quick to judge or blame doctors or nurses. Our profession is very challenging and we give 1,000,000% to our patients. God has the ultimate decision power!

I'm a chronic pain patient and have been on various narcotics for ten years. I'm slightly dependant on them but not addicted, which is a big difference. I can drop them cold turkey with no side effects other than unbearable pain and few days of minor flu symptoms. With all that said, I absolutely agree with you. His dose should have been tapered and monitored, but this was not the time to cut him off completely. Get him to lose some weight as you're tapering then wean him off once he's more healthy. Far too late now, of course.

Yep! That’s exactly what I went through. Opioids become your antidepressant and once they’re taken away, it feels like your world is painted grey. I’ve been clean for 2 years but the depression is still there

The depression sucks but so does the rebound pain. He Already had a knee injury. I don't doubt that he was in massive amount of pain and they did not do right by him. Because you detox on one cold turkey doesn't mean that they're going to magically get up and dance around and be happy and grateful. That pain the first few weeks was unbearable for me. I also know people who are in such a tremendous amount of pain that the quality of life is horrible. On The brink of suicide. so if you can treat it with a minimal opioid I do not see an issue if it is responsibly taken. Especially when all other options have failed.

@@theL0VERS I agree with you completely. Opioids should be given to people with chronic pain, quality of life should not be compromised in any way if there are medications out there than can help. It should not matter if it’s an opioid or Vitamin C , if helps to make your life more tolerable then so be it. When the USA clamped down on opioid prescriptions (suddenly, without tapering the prescriptions gradually) the rest of the world seemed to follow. It left people addicted without help, and addicted people, and I know from personal experience, will do anything to stop the withdrawal and pain. There were no significant increases in rehab centres, no increases in funding for methadone clinics and the like, there were was no increases of support workers and mental health services, and there were no significant increases in funding to support people free of charge. It’s just a cluster fuck.

Dr. Now has a habit of doing that to his patients. He's done it to other patients on the show. I don't think he cares about his patients withdrawal systems. These patients are extremely depressed to begin with and going through withdrawal from being abruptly cut off from pain killers makes their depression even worse. I know I've experienced this myself many times. He only seems to be obsessed with his patients losing weight and nothing else. I realize he's trying to save their lives but dealing with pain killers can have fatal consequences.
