This video covers the anatomy and function of the rotator cuff muscles, which serve to stabilise movement at the glenohumeral joint. The following muscles are reviewed with 3D models where we take a look at the origin, insertion, and innervation:

- relevant anatomy of the proximal humerus: greater and lesser tubercles

Try using a “muscle scraper” using massage cream (‘Free Up’ brand is the best!) prior to any stretching or exercises. I recommend using a stainless steel massage tool (SIDEKICK brand or similar)since it has the best weight for doing the “work” for you. Do not “hammer” any muscles or tendons and stay off joints or enthesis where the tendon inserts to the bone. This will lengthen tissue and remove scatter tissue below the epidermis; unlocking motion. Don’t forget to use the muscle scraper behind and side of neck under jaw. Recommend increasing Vitamin C, bone broth, and gelatin with vitamin C (the mixture makes a “jello”); increase water content. Check out other foods that help tendons to heal. Another consideration is PRP with HA; or depending severity, age et al…consider injection of bone marrow stem cells. Absolutely no steroids! Hope this helps.

The use of the massage cream with massage tool should be a gentle application with long strokes as much as possible. Keeping in mind any areas above the heart should be downward strokes (as much as possible in, and around the shoulder,) and upward when below the heart. The application of the tool should be very carefully performed around the supraspinatus tendon as the tendon “ties” into the top of the humerus bone structure; being more “flattened” outward. Very difficult to differentiate the MTJ. After massage recommend only mobility sessions, then proceeding to functional sessions.

Very pedagogical, superbly illustrated with clear, articulate voice, and moderate speed of speech, and explanations of the Latin expressions, that makes this video a precious tool for understanding the rotator cuff disorders. Great to use in class for first year OT, physiotherapy and sports sciences students! Hats off to this EXCELLENT video. Please, more of that.

I recently went through my third surgery related to rotator cuff repair. One on the left and two on the right. For anyone who hasn't had a shoulder injury and associated pain, remember this, we use those muscles thousands of times a day. And if the injury and pain are severe enough, you will gladly have the surgery. Even with surgery and rehabilitation therapy, it can take months to heal. Please be careful with how you lift and pull. And if you do a lot of physical labor and or exercise, take breaks often to rest those muscles. As you get older it is easier to damage those muscles and the longer it takes to heal.

@@ManimalDetroit Too much hard physical labor. Repeated small tears that were never given a chance to heal properly and then suddenly a large tear that won't heal on it's own.

Good post Terry, appreciate the first hand account from an RC surgery recipient. I read something about a very high percentage of people over 70 have no use of the supraspinatus due to them being torn from what was described in this video. At about 53 years I tore my RH RC (in three places) about 4 years ago and didn't want surgery but got the bursa injection of cortisone and was able to continue with most things including weight lifting, albeit much lighter. But last 3 months I tore it again even worse (falling off the roof), so I am again looking at surgery. The surgeon during the consultation was fairly blase, saying it was a very straightforward operation and could almost be done in day surgery. I wasn't overly impressed with this slightly arrogant display of confidence without even seeing any scans.

@@montinaladine3264 Have the surgery. About three months to fully recover. Mine was day surgery followed up with therapy two days a week. I stopped heavy lifting completely. Ten pound weights only. As long as I don't overdo it, I'm mostly pain free. Find a surgeon that takes your concerns seriously. You will probably need to modify your life style a little, lol. Don't work so hard.

I got this in my recommendations today. Such clear and concise telling with an eloquence that had me enraptured all the way to the end! As a physiotherapist, this is an extremely helpful tool to teach my patients all they need to know when they come to me with shoulder complaints related to the rotator cuff and its most common injuries! Really well made!

I had a terrible torn rotator cuff from a really hard fall, whereas I could barely sleep for two months because of the constant excruciating pain, but I wanted to find a better way and I didn't want to take any pain killers, because of my stance on them. I found a highly qualified massage therapist that found the muscles and tendons, and so forth (tissue? ligaments?) were shortened and stuck together around my clavicle and triceps brachii, while also concentrated attention to the acromion ridge. There are lymph nodes in some of those places, like under the arm, that should not be overworked by massage so it's important to get someone who knows what they are doing. Along with this, I did an ancient Qi Gong, Ba Gua basic beginning exercise from a highly qualified master that I had learned years before, called, 'Serving Tea to the Master,' which involves moving scapula energy around, and I made a natural 100% quick recovery without any surgery or any other physical therapy. Wish I knew it from the onset avoiding the 2 month of agony and torment. I also used generous ice and arnica gel. This moving of the scapula energy, which can be achieved through other qigong, tai chi, and kung fu exercises and practices nicely, heals, treats, cures, and addresses a great many other maladies and injuries, including for the mind and the spirit. THE HEALER IS WITHIN!!

I have recently had a bad fall which resulted in a three part proximal fracture of the humerus in my left arm. This tutorial as been SO useful for me to understand what is going on in my shoulder. "Knowledge is power", as the saying goes!

Great video! Having torn my supraspinatus completely off several years ago and fortunately having a great surgeon/Dr (Mark Frankle), I avoided surgery, but until this video, I never had a full appreciation of the injury, because a picture is worth a thousand words. Thank you.

I just got an MRI result with a supraspinatus tear, and watching this video has really helped me understand exactly where the pain is coming from and why. It's incredibly relieving to have a good understanding of the injury which is also allowing me to organize and put into practice a robust physio treatment which will hopefully lead to good recovery. Thank you again, very well explained and animated. It was particularly important to me to see the common injuries near the end of the video. I'll be referring colleagues who suffer similar injuries to this video and others in your library.

I had a Titanium Humeral Ball installed in 2001 and was told that one of the muscles did not reattach. For 20 years I’ve been looking for a schematic like this one!

Really excellent presentation, thanks for making! I already had a basic understanding from first year anatomy but this gave me a higher level of understanding regarding naming and movement. The only thing I would have liked to have seen more of would be extra info and diagrammatic presentation of various injuries of all of the muscles of the RC, not just the Supraspinatus. Plus various levels of injury such as partial tears to full thickness tears and nerve damage and what movements these compromise and what effects they can have on the whole joint musculature. And how the body compensates etc. But maybe these details have been covered in other AZ videos.

After I separated my shoulder years ago, I never start a workout without warming up my rotator cuff muscles; even if it isn't relevant. I still have a lot of issues with my supraspinatus though, so perhaps I should look into how freyed this old rope really is. Great breakdown overall, a lot of people could use something like this to help them prevent injury.

I damaged mine at 22 and stretched and warmed up everyday I did anything with the upper body. Once it’s hurt, you learn your lesson!

I think I tore a bit of my rotator cuff last year from throwing a football still hurts to this day I can do certain exercises but I cant do most due to pain in my shoulder, I cant lift much of anything over my shoulder.

Separated my shoulder years ago as well I never went to the doctor and ever since I had pain in my shoulder and now it’s given me neck pain and lower back pain and I’ve been in therapy for almost a year and I haven’t fully recovered yet or have a clue when I would be.

I could NOT get my head around the rotator cuff in all my readings. This excellent animated presentation made it easy for even a dweeb like me to understand. It takes a lot of talent and hard work to make something so complicated to be be understandable by most people. My husband has a torn rotator cuff, and the technical report from his doctor was complete "Greek" to me! So, I googled a phrase in his report: "Acute full-thickness tear of the supraspinatus and infraspinatus with retraction to the mid humerus level," and this video came up.

Yep I did 3 out of 4 the biggest biceps tendon was retracted 50 mm ,so in reality it was massive tear but I managed to get nearly all movement back through many many hours days weeks months now into years of rehab it’s a very terrible terrible painful injury but with time it’s possible to get through most of the pain points, but it will never ever come back fully after such injury does occur. I am so glad to see this vid as it made a lot of sense given my injury tear and the subsequent rehab undertaken. Thank you for posting.

Wow! Awesome presentation and illustration of the rotator cuff! I have been experiencing shoulder pain for some months now and have been visiting a PT. Although I am a paramedic, we do not focus on the bones, joints, and nerves as they are not life-threatening emergencies. So I appreciate your concise and simple explanation of the rotator cuff. This along with other videos on RU-vid is definitely helping me understand my own injuries, pain, and A and P. Thank you so much!

Man this is amazingnly presented and so well made. I really hope that all past videos in this channel from many years ago that have to do with muscles and joints are remade in the style of this particular video. Again, fantastic job, this video is for me the definitive way of presenting anatomy.

Great video, 🙏 thanks for the clear explanation. In some fighting sports the shoulder impingement is also very common. Especially because in most of the technical exercises there is a lack of external lateral rotation, which weaken the infraspinatus and the teres minor.

excellent! If I had had such a lesson during my school days in India learning would have made more sense and definitely interesting. I now have a clear layout in my head about the diff bits of the RC and understand which bits are hurting.

Based on my own experience, rotator cuff rehab exercises (mostly range-of-motion with light resistance) will help by keeping the muscles active and strengthening to avoid future injury. Don’t do anything that risks re-injury or causes pain, and don’t mask the pain with anti-inflammatories or analgesics. Also, be patient; the injury will heal with the passage of time.

You need a diagnosis of which muscle/tissues in that area are injured, in order to know which stretches and exercises are necessary for you. The four different muscles in the rotator cuff have different functions and pull/move the humerus in different directions. So it really depends on which one is bothering you. Rotator cuff muscles also aren't the only tissues there as the deltoid group and pectorals also attach at the humerus, among others. Lot of different things it could be. Have a PT examine it would be my advice 😉

Don't ask for advice on the internet mate. What if someone tells you to do exercises that will make the problem worse? Go to a physiotherapist to evaluate your injury.
