Douglas Wisoff, PT and Coach, demonstrates in the "Healing on the Run" video series, how to heal IT Band and other types of knee pain through Form Correction: radiantrunning..... With IT Band pain The main component that is consistently in the picture is a weakness and lack of stability of the lateral hip muscles (Gluteus Medius and sometimes the external rotators as well) on the side that has the knee pain. This causes the hip to wonder laterally on the weight bearing side, and the femur to rotated inwardly at the same time. This problem alone might cause the injury, but usually the runner will also be extending the leg out in front of the body (over-striding) in addition to holding the upper torso high and back. The knee is then dealing with increased impact stress on top of the poor alignment.
The over-stride alone is often the cause of a number of knee problems. When combined with this particular kind of instability at the hip it tends to favor IT Band pain.

I developed severe IT band issues leading up to my marathon and I watched this video a few days prior - I’m sure it’s what saved my marathon debut! I ran, focusing on my form and had no pain during the entire race! It’s incredible! No problems a day later either - THANK YOU so much for sharing!

I really think that your IT band pain was not severe! With severe pain like mine the only thing you can do is stop running immediately, let it heal first and then building up your new running technique very slowly over months and months. I am talking about IT band pain here that 'simulates' a torn meniscus which starts to become pure torture from hell after 10 minutes of running.

@@julienparis3512 Well I ended up with a sports medicine physician. Got a reference from my GP. Instead of an orthopedist he wanted me to take the conservative route. The sports doctor said it is impossible to say what it was, and also a neverending guess. An mri would only make it complexer and put demons in my head. He made a film of my stride. It was obvious that I was a heel striker. I did exercises like jumping on my toes with a straight posture. I build my running in the next months up to half an hour with growing intervals. After 3 months I stopped this revalidation and became a forefoot lander. A half year later I could run 1,5 hours straight. Now because of the forefoot running my heel spur got worse. This was the reason why I devoloped a heel strike technique in the first place. So my advice is run straight, relaxed, on your forefoot, land under your body. I would not recommend barefoot running shoes, but thick soles that dampen (I dont know if my physician would agree). O yes, my physician never encountered an athlete that developed a meniscus tear through (ultra)endurance activities. And personally I now think that so called "it injuries" are fiction. Thank you Douglas, I'm going to work on these tips. I'm 29, I've had IT band issues in my left knee since early March, flared up all of a sudden during a taper in Marathon training. it's been very debilitating physically and mentally to be honest. I know it's flaws in form combined with over training, but now I need to correct it!

Great video. I had always suspected that fixing my running form had helped me get rid of my knee pain but I never knew it was IT band related. Nice breakdown: it was very informative.

I've had this problem for 6 years , pretty much stopped me in my tracks . Tried all the recommended stretches etc literally made no difference . This gives me hope . Thankyou

Giada Trovò Hi , I’ve recently had some breakthroughs with this injury . Two months ago I could maybe manage 15mins and am now able to run for about 45 mins relatively pain free ! I would recommend reading the book Chi running by Danny Dreyer . The key things that seem to work for me are focusing on correct posture and building up the time/miles really slowly (10% per week at most ) I still only run once a week but I absolutely love it , I wish you the best of luck 👍

the chiropractor i went to said my it band is inflamed how much does it effect running because my whole leg just burns up straight away when i do a run for pace

Can you help me out sir? I have this knee pain on outside. I gave 1-2 week break. I used to play football after running. I am not able to run properly and my knee hurts badly if go above 2 km!!! Now... Earlier it was bad as hell. I used to run 5 km .., 5-6 days a week.... Now i not able to take sprint. How can i cure it?

I am a marathon runner from greece and I just want to thank you for that kind of videos. they are very imformative and especially for me they were crucial to overcome and surpass every pain that I had from forefoot strike. I changed that year my strike to midfoot and I cant describe how much that works and feeling right to me. Again thanks a lot and keep it up! @douglas

anyone having this problem look up anterior pelvic tilt or inwardly rotated pelvic tilt. thats causing your it band most likely to hurt or at least it was in my case.

When i start developing flare ups after like 90 minutes, especially when i have an easy/slow run, then increasing cadence and doing a couple of longer/more dynamic swings of the knee usually resolve the pain or help keep it at a low/manageable level. I wish i was way faster for my zone 1/zone 2 work, because my form is better when running faster (my guess). Thanks for sharing these videos.

This is so helpful! Is it all good to massage the IT band? Does that help? Or is rest and icing the only thing that can be done while I can’t run?

I wonder why this never happens to me on trails. It usually doesn't happen on flat ground either, but it does flare up now and then. I've been so careful about form too, it's very hard to know what one is doing wrong at times.

I am having IT pain on my left side. I am defiantly a forefoot runner as I run in sandals or barefoot. I have an anatomically short leg on my right side which causes that foot to tow out and my left hip is pushed out to compensate. I figure this is why i have the IT issue, but I dont know if I can fix it. Any help here would be nice.

Wow I finally found a video addressing running form. But should I use a knee sleeve while improving my form?

I would wear the sleeve if you're used to it and it doesn't inhibit natural movement/ROM. Also if it helps you to be less apprehensive in your movement. If it feels inhibiting in any way then I would put it aside.

I pulled my IT Band while running and now my knee clicks every time I bend it and if I prod that area it feels like fire or a hot electric current goes through it

I wonder if that's what happened to me. 6 months ago my ITB slipped very forcefully across the bone. Now it seems to get stuck and then snap across the bone. It isn't hurting, but when it first happened, I could barely walk for a few days. Now it just feels weird.

Man, I constantly get it pain after about 3miles, and it's extremely frustrating.. seems I have to correct some type of posture I'm unknowingly doing.

What is a good balance between chest being out forward but not too forward that the shoulders go back and down?

Try this...Before you start your run, totally relax your shoulders and chest down on a big out breath. Then staying relaxed start your run easy and gradually pic up your pace staying relaxed, letting the arms swing naturally and not positioning your upper torso in any conscious way. See if you can stay relaxed up on top. If you can't, work on that.

It highly depends on how serious the injury was. After a pretty band IT injury I could run my first 5KM with little pain after 2 weeks of resting and some isometric exercises. The big deal after such exercises is raitining your aerobic fitness (via biking/swimming) and strengthening your core and hips. How is it going for you ?

Christos Alexiou I had to stop activities in July due to chronic knee pain and I'm getting an MRI next week. Turns out it is probably something called "Hoffa's syndrome" which is where the hoffa fat pad in the knee gets inflamed and then impinged between the patella and the femur. Hopefully they can do an cortisone injection and get the inflammation down. (they don't do cortisone for tendons or ligaments anymore due to rupture risk)

Stephen Lewis man, I really hope you get better soon and start again. Cortisone injections will certainly help you but you should also focus on your biomechanics. Strengthen your legs and core to reduce knee strain. Combined with your doctors orders you should be back or at least feel better. 🔥

Although factual, your presentation is not that clear. I couldn't figure out exactly how to correct the issue. Relax the upper half, be straight up/down and don't use heal strikes, but rather flat foot the strike?
