THIS VIDEO IS A RECONSTRUCTION OF THE FOLLOWING SITUATION IN FLIGHT:
07-MAR-2023. A Republic Airways Embraer E175LR (E75L), registration N401YX, performing flight RPA4736 / YX4736 from Washington Reagan National Airport, DC (USA) to Raleigh-Durham International Airport, NC (USA) during taxi was instructed to cross runway 4 at taxiway J but the airplane turned on taxiway F and entered runway 1. Meanwhile a United Airbus A319, registration N819UA, started take-off from runway 1. The Tower controller noticed that and canceled take-off clearance. The landing airplane was instructed to go around from altitude 500 feet.

HOW I DO VIDEOS:
1) I monitor media, airspace, looking for any non-standard, emergency and interesting situation.
2) I find communications of ATC unit for the period of time I need.
3) I take only phrases between air traffic controller and selected flight.
4) I find a flight path of selected aircraft.
5) I make an animation (early couple of videos don’t have animation) of flight path and aircraft, where the aircraft goes on his route.
6) When I edit video I put phrases of communications to specific points in video (in tandem with animation).
7) Together with my comments (voice and text) I edit and make a reconstruction of emergency, non-standard and interesting situation in flight.

controller set them up for failure, but we have to crosscheck better. Fatigue is probably playing a big role in all these. The trips are getting more and more fatiguing. FAR 117 was supposed to help, and in some ways it did. In other ways the trips are worse from a fatigue standpoint. I am sure ATC is having the same issues.

This is on ground control mostly. They assigned them 33 for takeoff and hold short of 1 at F. The ground tells them to cross 4 at J. The pilots just heard cross and didn’t realize they were crossing the wrong runway. Expectation bias strikes again.

Definitely expectation bias. Also, anyone familiar with DCA knows that it is an extremely fast paced airport and any runway clearances must be executed without delay. For example, at the time when this occurred, there were about a dozen arrivals lined up on approach and a queue for takeoff. They heard instructions to cross RWY 4 and did so, but at the wrong intersection (F instead of J). Just thinking from a human perspective here, they probably wanted to get across the runway as soon as possible and didn't reaffirm the clearance with tower to not delay the sequence. After all, they were going to RWY 33 and this was their route, so in they're minds they're probably thinking "oh, he means F". Then ATC has to repeat himself, and now they're rushed and probably ended up catching a case of "Hurry-Up Syndrome". Not saying that this isn't a mistake, but psychologically I can see how it can happen. edit: a word

@@billybuttons4298 It starts with the controller. Pilots have to catch the mistake, but I guarantee the controller forgot he sent them to 33, not 1.. On the route given, they cannot cross 4 at Juliet. When you hear a crossing clearance holding short where you were supposed to be, it’s going to be a decent chance the pilots don’t catch the mistake. Being given the wrong clearance doesn’t put it fully on the pilots by any means.

Ground definitely holds some responsibility, but they weren’t cleared to cross 1. Brickyard should have queried ground and holds some responsibility for sure. Ugly situation..

I don’t put this on the controller. The republic pilots should have known where they were, what they were supposed to be doing vs what they were instructed and recognized that didn’t make any sense.

I was ready to blame the pilot of the e175 100% but then realized that they were following the initial taxi clearance. Ground controller didn't notify them of change in runway or instructions, and the pilots didn't catch the change. Certainly fault on both parts.

The problem is, the pilot is at fault for not following directions. If there is a change from the initial clearance he needs to clarify. Otherwise this happens. 100% preventable if the E175 pilot doesn't cross RWY 1.

The Republic crew was 100% at fault. They received instructions, read back those instructions verbatim and did something totally different.

The crew unfortunately will be 100% culpable since they technically crossed a runway they weren't cleared to.... BUUUT ATC screwed up big time and set the crew up for failure. Not only did they emphasize there was a runway change, they didn't specify the runway in the new taxi instructions (required when issuing a taxi clearance, in this case because it was a new runway being issued from the original clearance). While everyone may pile on the crew and blame them... why not focus on making things better and not throwing pilots under the bus with a "GOTCHA!" situation. ATC failed here, even if the ultimate responsibility for the deviation lies with the pilots.

What they executed was crossing Runway 1 at F, without ever having a clearance to do so. It appears both made errors: The controller for not clarifying that the runway assignment had changed and the E175 crew for crossing Runway 1 without being cleared to do so. It is also possible that the controller was confused and issued the original clearance in error, and then thought he had cleared the aircraft to Runway 1 all along. Without seeing the results of the investigation, it's hard to fully judge.

From 30 years of experience in the FAA as a controller. The cross RWY 4 instruction was a change, but the pilot read it back and did something different. His responsibility was to question the call if it was different than previously told to expect. By accepting the call then doing the wrong thing he has in fact had a pilot deviation. Sorry, the controllers did a confusing thing, but the pilot wil be at fault. Lesson, always verify your clearance with previous clearances and expectations, and always question if they don’t add up.

Shouldn't ground control have told them that there was a taxi clearance change? They were cleared to Rwy 33 and would not be expecting to be crossing Rwy 4 @ J to get to 33. They were already holding short of 4 @ F as they had been instructed to do. I do understand that they read back the clearance and proceeded to do something different, but I can't see how some of the responsibility doesn't get placed on ground control. Just my thoughts. I have no experience as a controller, just a low time CFI trying to use some common sense.

Yeah, it’ll fall on the pilots. It nearly always does, and I guess that’s right if you want to maintain standards. But once again, ATC setup an ill-conditioned situation here with contradictory instructions. Yeah, the pilots should catch that, but let’s not let the first hole in the Swiss cheese entirely off the hook…

THIS! It's the pilot's fault for the deviation, but ATC screwed up big time with the confusing omission of a runway change. So while ATC may not be at fault, they need to do better in the future, from a human factors perspective of avoiding these types of incursions. It isn't about who's at fault: it's about safety and preventing this from happening in the future.

Wouldn't be surprised if the FAA implements a new procedure for planes to cross an active runway that has a plane on that runway or landing on that runway, the current procedures is proving to no longer be adequate enough. EFB's need to have a better live tracking airport diagram for pilots to be more aware of their surroundings. With the influx of new pilots after covid we need new measures to avoid more spikes in incursions.

You can't really let the pilots off the hook here. Yes, ground should have highlighted the change, but the pilots have no excuse for not catching it and requesting confirmation.

A confusion all around, first atc seemed to lost the fact that they were suppose to taxi to rwy 33 (the initial clearance) and then cleared him to cross rwy4 as if he was planning to take off on rwy1. Still, his instructions were not putting him on harm's way. Again, I think a confirmation bias issue made the embraer crew cross at F towards rwy 33, their initial instructions. Situational awareness failed here, they should've stop the plane and double check the instructions with atc. When in doubt, don't. A lesson to learn because it could happen to all of us.

@@featuredview6546 No doubt, the controller also has a piece of the cake and he instructed them to cross rwy 4 at J, but again, that instruction did not put them in harm's way, however that was not the initial clearance instruction and it added to the confusion inside the flight deck. The Embraer crew apparently fell into the confirmation bias syndrome, which unfortunately seems to be more and more frequent these days, they were determined in their minds to cross rwy 4 at F. We are all humans and mistakes are part of us, hence the "human factor", but I still believe whenever you are not sure about an instruction, double check. "Ground, you want us to cross 4 at J instead of F?" At the end of the day, it's not about looking for guilty parties, it's about stopping the Swiss cheese model in time in real time, have the situational awareness and being laser focus during the busy stages will improve our chances of not getting ourselves in bad situations.

That's a tough one for the pilots but they still should have caught the runway crossing clearance was for 4 and not 1. Taxi instructions to 33, the box was setup for 33 and then they throw in a runway 4 crossing to runway 1 at the last second. Equal blame all around? maybe...

Two things about this incursion are frustrating. One, the reenactment doesn't show whether the RPA actually turned left at Foxtrot and was already holding short of 01 when he received the clearance to cross 04 at Juliet. The animation makes it look like the RPA never turned onto Foxtrot and thus should have continued on Juliet to cross 04 but INSTEAD made the left turn onto Fox AFTER getting the updated clearance. But is that actually what happened? Second, if the RPA was already holding short of 01 on Foxtrot, I can see why the clearance to cross 04 could be interpreted as a SIMULTANEOUS clearance to cross 01 followed by 04. If he's already on Foxtrot holding short of 01, how's he supposed to get back onto Juliet? He'd have to enter 01-19 and taxi for ~750m in order to make a left back onto Juliet. Either way, RPA is gonna bear the brunt of the blame for reading back "cross 04 at Juliet" and then proceeding on Foxtrot but if he was already on Foxtrot I can see why he lost situational awareness here. Ultimately, the taxiing SOP at KDCA seems like a huge part of the problem. Glancing at the charts, sending any departing plane to RW33 via Juliet - Foxtrot looks like a recipe for disaster, especially if the airport can turn from 33 to 01 this quickly. For starters, it appears there isn't enough room to cross 01 on Foxtrot and hold short of 04. You'd have no choice but to cross 04 in order to get clear of 01. I'm very curious how often ATC clears pilots departing on 33 to cross BOTH 01 and 04 simultaneously? This could easily lead to the kind of expectation bias that might lead a pilot (like the RPA pilot in this case) to hear "cross 04" and assume it implicitly includes clearance to cross 01 first. Another problem with taxiing on Juliet - Foxtrot - RW33 is that you can't merely cross 04 to stay on Fox. In reality, one would have to enter 04 briefly in order to continue to RW33. Objectively, Juliet - Golf - Hotel - Foxtrot - RW33 makes way more sense, but the intersection of G-G1-H is listed as HS3 so I get why ATC might avoid it. But if J-F-33 is SOP, it's maddening that Jeppesen hasn't listed this as another hot spot. Looks like a good candidate for HS5 on the next AIRAC cycle, especially after this incident.

I live in the DC area and am familiar with the usual ground movements at KDCA. The past few days we have had strong winds outta the northwest which is why some planes have been departing and landing 33. It’s only 5000ft so it’s not used that often. The ground controller initial tells them to taxi to 33 and hold short runway 1 at F. The usual clearance is to taxi to runway 1 via J, cross 4 at J. He told that to Brickyard, mistaking that he had given them 33 and not 1. So tower was wrong but also the crew was wrong because they were not clear to cross Runway 1 and they crossed Runway 1 and 4 at F and not J. Bad situation in general and glad that a accident was avoided

Yeah, keyword "initial". Things change, that's why you write the taxi route on your kneeboard. When the next controller says route again, you look at your kneeboard to check it's not changed. This pilot failed to Read during s readback task. Strangely revealing he is a parrot. He just repeats what comes in his ear. How long has he been a dangerous Parrott? Who knows. Ground the dummy until he learns the basics all over again.

Most definitely Expectation Bias from Brickyard Crew perspective (anticipating RWY 33) additionally the nonconformance uttering and instructions by ATC who Xs 2 uttered "Cross 4 Delay Monn Tower" as expecting by BY 4136 to instantly know that they're being reassigned to another Runway. Failed to read during readback. Should have his license torn up and start again at groundschool. I'm disgusted by the quality of readback procedure.

I'd be curious to hear from a pilot as well. A lot of people are saying we're just hearing about these incidents more often because of channels like this, but it seems hard to believe that it's been like this all along. And if it really was this bad all along, that it's taken until now for the FAA to get together and hold a summit to discuss the issues.

I don't have any stats but I'll make a couple observations... Nobody was hurt and no property was damaged. This incident wouldn't have shown up on the evening news had it happened back when anyone cared about the evening news. There's more visibility now via channels like this one and with the 24/7 cable news networks trolling the Web for anything resembling a story. It can be argued that the system worked properly - when a human error occurred (a very understandable one, given the last-second change of the taxi clearance), other humans (both the pilot of the departing aircraft and the tower controller) noticed immediately and made adjustments. The pilot on final was instructed to go around, which I'm sure he would have done anyway once he got closer and realized the departing aircraft wasn't departing. In other words, as terrifying as this all looks, there's virtually nothing to see here. No harm no foul. Everyone should make a note to think hard about their taxi clearances and be aware of traffic around them, even/especially on the ground. Lessons learned. Moving on.

He assigned rwy33 initially then clearly advised brickyard to cross rwy 4 at j. If there was any question as to why they were told to taxi away from assigned runway they could have asked before proceeding to move. This aint on the controller.

Pilot repeat back to cross RWY 4 not RWY 1. He clearly crossed RWY 1. Even if it wasn't what he expected, his acknowledgement of the order places the blame on him. If he's confused he doesn't move.

The only explanation is that the pilot doesnt know how to readback. The clue is in the name *read* back. Not TalkBack like a parrot. *Read* what is written on the kneeboard. If there is a conflict then he should ask for clarity. The trouble with having all these screens to stare at means he isn't writing his taxi route down on his kneeboard and checking it. He is just being a parrot, repeating what comes in his ear. He should have his license torn up and sent back to ground school to start again. Im disgusted with the quality of piloting.

I the beginning ATC assigned Brickyard 4736 to taxi to Rwy 33 via NFJ. The next clearance from ATC was cross Rwy 4 on taxiway J. Just an odd way to get to Rwy 33 unless they were reassigned to Rwy 1? I listened 3 times and still perplexed. Thanks VASA for another scary, yet informative video.

Initial taxi for 4736 was NJF (rather then NFJ). That's why there are so many comments here about expectation bias; they turned left on F per initial directions (straight across 1) despite 2nd controller telling them to cross 4 at J.

The controller who gave the instruction said Juliet so quickly if I wasn't reading it on screen I wouldn't have known what he said. I know the pilot got it, because the read back was correct, but that instruction was not as clear as it should have been.

So what you're saying is that you couldn't understand what the controller said. You, who I assume isn't a pilot or at least not a 121 pilot, didn't understand the controller. Since you didn't understand, the instructions weren't clear. The pilot, who is an actual pilot, understood the controller. Because guess what...he's a pilot. The instructions were clear for anyone who actually does this professionally. He understood the instructions and read them back verbatim. Just because you, a casual observer, doesn't understand doesn't mean pros don't understand.

Poor communication has been called out before by pilots, there is one video where the pilot asks for clearer communications with a JFK controllers: "clea lad". He made him say it 3 times until he said "cleared to land". Good. In this instance he simply talkedback the route, instead if reading it back off his kneeboard. Completely unacceptable. A complete failure to *read* during a readback. He should have his license torn up.

Key detail, they crossed both 1 and 4, at the intersection of F, 4, and 1. When crossing at the intersection, if you were cleared to cross one, it would be strange (looking at it from their perspective) to not be cleared to cross the other.

Okay, they were distracted. I was based out of DCA for years (ironically I flew that exact tail number a few times) and never made that mistake. Glad tower caught that.

@@JohnSmith-zi9or I know because I've been at that intersection. It's clearly marked and hard to get confused. I think the only time we got confused at National at that intersection was during low visibility situations. DCA is a pretty easy airport to navigate when you are paying attention. But saying that they were distracted is what often leads to confirnational bias because you are not paying attention. You are just accepting that what is in front of you is what you need without trying to verify it.

@@jhmcd2 Then you should know that you don't speculate until we know the FACTS. Suggesting they were distracted is just a damn guess on your part. Assuming the FAA gets their hands on the cockpit voice recorder (good luck), what would you say if it turns out there wasn't any cockpit talk except appropriate conversation? Would you still speculate that they were distracted?

I see can see why the instructions were confusing. They were supposed to taxi to runway 33 but then given a clearance to the block for runway 1. I would agree the pilots should have clarified with the controller instead of assuming what was meant to be said.

When the second call was to cross rwy 4 at Juliet, the pilot should read it back and questioned the controller’s intent given that he initially cleared to Juliet and hold short of rwy 1. That second taxi instruction to cross rwy 4 at juliet was never going to get him to rwy 33. He should have questioned the controller’s intent. He didn’t and proceded to cross rwy 1 in front of an aircraft takeoff roll. He also should check the runway for traffic on it before crossing. ATC can make mistakes and it’s incumbent on pilots to be situationally aware enough to at least challenge ground control.

What a situation. So as a pilot and controller. I can tell you I would of at a minimum questioned the cross 4 at J. Having my airport diagram out, I would have seen no way to get to 33. Now I'm at F waiting to cross 1. Obviously it would appear in the video they are holding short of F. I don't believe that to be the case. Why did the ground controller fired off the clearance? Brickyard read back cross 4 at J. It's a crappy situation and both parties are at fault. There's probably more going on here as well. Woth the recent uptick of runway incidents...the FAA and NTSB will be reviewing this situation. I'm sure more will come out of this. However as fly crew I always welcome you to question me as an ATC. They just had to ask, cross runway 1 at F? If ground was listening they would of caught the error and all would of been well.

The ground controller lost situational awareness and simply passed along a crossing instruction from the local tower controller without speaking up and telling the guy standing next to him that the departure traffic holding short was for for runway 33.

They can handoff to as many controllers as they like, giving different mystery tours round the airport. It's the pilots job to WRITE the instructions and READ them back. He didn't do that because he is a parrot. He thinks it's safe to be a parrot instead. How long has he been a lucky parrot? Who knows, until one day he isn't a lucky parrot. He's a dead parrot.

A change if instruction is valid. Unusual. But valid. What is not acceptable is FAILING to write instructions down to compare. There was a clue with a controller changeover. Perfect opportunity to compare both sets of instructions on the kneeboard. Except this terrible Parrott pilot didn't write it down. Just chirped back what he hears without any mental processing. Disrespectful to the controller by not writing the instruction down, and unprofessional creating a safety failure opportunity.

Still say ATC needs to stop using two different active crossing runways unless it’s operationally necessary. Not permanently but at least pause using them for now. Delays be damned.

(by the way, I know DCA very well, having lived in the area for 20 years and platespotting at DCA for at least 15 years)

The ground controller messed up, told Brickyard the route to 33, which was followed, but then switched it to 1 without properly informing Brickyard of the change. Brickyard should have heard 'Juliet' and gone 'Say again for Brickyard, we are going to 33', but did not, just trundled as instructed earlier to 33. Humans. Not perfect. Both are at fault here.

I have to give predominant blame to the Embraer pilot. The tower instructions may have been somewhat "confusing" but they seemed to have been accurate. Bottom line: the E pilot was about to cross a runway he was not cleared for.

They give out a phone number to call the tower because they deviated from a clearance. Copy a phone number = you messed something up. You call and discuss what happened at some point. But the whole incident is forwarded to the FAA to determine what action against the pilot is appropriate.

The controller screwed up and the pilot didnt question the controller’s obviously incorrect instruction. Total breakdown… this is why ASSUMING is the WRONG THING TO DO!!! Ohhh, that's one's on the controller. He forgot which way the aircraft was going and told them to proceed as if they were heading for 1, but he'd given them taxi clearance for 33 with hold short of 1. So when he told them to cross 4 at Juliet... well they were on Juliet, so that's OK, and they needed to cross 4, so that makes sense. And off they went.

Yeah it may have been different from he original plan, but if you get an instruction and read it back then do something to the contrary, it's on the pilots. And as far as your nonsense talk of them being on J and crossing on 4 and that being okay etc....uh no. The instructions were to cross 4 on J. They crossed 1 and 4 on F. Poor Brickyard didn't stand a chance with those taxi instruction. Granted, questioning the controller would have helped considering he was instructed to cross RWY4 @ J , and he was holding short of RWY 1 @ F. Mostly ground for not paying attention, and a little on the pilot for blindly crossing without paying attention. --- Clear Left , Clear Right , Clear to Cross --- Would have seen United on the Runway.

Was also holding short of 4 on Foxtrot. Without seeing it in person, I cannot tell whether the signs for 1/19 and 4/22 are visible simultaneously, at which point being cleared across one would appear to be a clearance across the other. As well, with the literal two seconds between takeoff clearance and the cancellation, I doubt, if following protocol to a T, that at the point that Republic cleared the intersection, they noticed United holding in position, but without landing lights lit, as they hadn’t yet been cleared for takeoff. At the exact moment United was cleared for departure, Republic was already starting across 4/22.

Ground controller probably forgot they were assigned runway 33 for departure and gave them the standard cross runway 4 since most departures use 1.

What a mess clearing them to 33, hold short of runway 1 at foxtrot. Then he clears them to cross runway 4 at Juliet. Absolutely doesn’t make sense, pilot should have in theory questioned it, but that was either a lapse in memory from controller they were cleared to 33, not 1, or just said the wrong thing via habit.

They do and they throw in distractions while crossing runways such as flight attendants calling the flight deck and what not. It’s a lot more difficult in real life with multiple airplanes talking to controllers, time pressures, congestion at small airports like DCA and LGA, etc. It’s a very busy job on the ground.

Honestly, this is kinda 50/50 blame. Yes, Brickyard accepted the clearance with a readback and should have asked if they had any doubt. Ground gave that instruction way too fast and in a truncated manner - he should have highlighted that it was a change in clearance and done it slowly. This is where delegating a crossing clearance from Local to Ground can be problematic, as that was the active runway
